Alhamdulillah. Website " payam e Sehat Foundation, Mohammad Meher Elahi, free clinic for heart and diabetes is being launched.

Population at large is at hightened risk of heart and diabetes related ailments. Prevention is always better than cure. We aim to assess patients at the earliest stage of the disease, so it can be nibbed in the bud.

Diabetes is a disease which needs constant struggle to keep it under control. As the coronary artery disease starts at the appearance of fatty streaks in coronary arteries, in a child as young as 10-12 years old. Similarly diabetes get its roots from metabolic syndrome, starting from obesity.
Now every household is witnessing excessive consumption of fast foods plus sedentary life style. Even school going children prefer to sit and play video games rather than playing football, cricket on the ground. Harsh reality is that most school premises does not have playgrounds. The do annual sports day in some nearby hired playground. So, children are not regularly given chance to be physically active, even in schools.
Diabetes require constant watch on their intake of food quality as well as quantity. Portion size play an important role in managing diabetes. Energy intake and energy consumption must be balanced, to keep weight under control. This will reflect in the control of the diabetes.

A female, 30 years old, with Down's syndrome. Known case of type 1 diabetes, diagnosed since 3 years.
Her appearance is like an early teenager, regarding height and weight. Typical slanting eyes, loose skin (web neck)on sides of neck, single palmar crease, short and broad hands, small fingers. Widely separated 1st and 2nd toes.
She was already on oral hypoglycemic agents. But her blood sugar was never optimally controlled. The patient was not controoling her dietry restrictions.
With high suspicion, we performed blood test for C peptide proteins. This test distinguishes between type 1 and type 2 diabetes. She turned out to be type 1 diabetes mellitus.

Mr AFZ came to the clinic with a shocking story. He lost both his upper arms in the line of duty. He was an emloyee of Karachi Electric Supply Corporation, while working on the electric pole, he was electrocuted. Became unconscios, when re gained consciousness his both hands were blackened. Later both his arms were amputated. He was sole Bread winner for his family. Had very small kids. Due to financial hardships he could not bear the stress. Developed diabetes mellitus. It was the time, we just had started this free clinic. He got enrolled for diabetes. Since that time his kids taking care of every of his need. Now the kids are grown up, doing some petty jobs to provide for the family.

"Prevention is better than cure."
It is so true when it comes to foot problems of a diabetic. Diabetic foot is a devastating condition. It encompasses corns, calluses, flat foot, ingrwing toe nails, high arch, pressure sores, ulcers, osteomyelitis, charcot foot, etc. etc.
One or the other of these condition develops, persists, and if left untreated, the ultimate result is loss of limb or part of it. May endanger the life even.
So inspection with great detail and correcting and treating any problems as and when they arise, is the key of saving the limb, and hence keep him/her in queue of a common patient rather than being sympathized by every body seeing them and causing severe depression and anxiety in these patients.

Tolosa- Hunt syndrome is an eye problem, secondary to diabetes and hypertension. Cavernous sinus is inflamed and cranial nerves passing through it is affected, leading to drooping of eyelids and vision is impossible. If treatment started earliest within hours, then it can recover also.

Alhamdulillah, it is more than 5 years now. Payam e Sehat Foundation, free clinic for heart and diabetes was established. Simultaneously NICVD accepted our offer to open a chest pain unit in the same clinic. People in the vicinity are enjoying benefits for not only controlling their diseases but having access to chest pain unit as and when required.
