A 50 Years old male presented with Recurrence of Low Grade Glioma involving R motor cortex . The previous surgery was done in 2012 in Karachi and patient was remained well for 10 years . Now he has Power 4/5 in Left hand with Focal fits . Functional MRI for Motor Mapping of left hand and left foot was done to analyse the relationship of functional activated areas with the Tumor to make surgery safe and to achieve gross total resection with out worsening neurological defect .

A 50 years old Male presented with history of low grade glioma operated in 2012 which turns to be oligodendroglioma WHO 2 . Now their is recurrent and surgery was planned.

A Funtional Mri was done to Map patient R Motor Cotex - Left Foot Area .

Red Dot is the Patient R Hand Activation area Just post to the Tumor during Functional Imaging.

And when we put corticospinal tract by doing DTI its the same area where patient has its R Hand Control in left motor cortex.

Now Orange region is the Left Motor Cortex along with corticospinal tract .

This is First Time in Pakistan that you are abel to Navigate Motor cortex Responsibel for R Hand Movement in real time intraoperativly .

2nd Case :-
Exoscopic Neuronavigated , Tractographic Excison of Low Grade Glima of Left Hemisphere near the Eloquent area of Speech and Right Hand .Aidded by Functional Neuroimaging for speech and Motor cortex along with 3D Reconstructed strutural images for Brocas .

Two Functional Tasks Was Performed to locate Speech of the patient and the Right Hand area through functional neuro imaging which is in gyrus located posterior to the tumor .

Tumor is like a Rose in the bushes . You have to steal the Rose from the bushes

Red Is the Tumor , Blue is corticospinal tract and yellow is the rest of the tractography including unbundled fibers .

Tumor is supromedial to Broca . Synaptive Medical
