*INAUGURATION OF PROF. DR. AAMIR MAHMUD IJAZ THORACIC SURGERY SKILLS LAB*

Department of Thoracic Surgery, King Edward Medical University Lahore. Official was established by Dr. Ijaz ul Hassan in Mayo Hospital, Lahore in 1959. This was the first dedicated department in this discipline to be established in Pakistan. Later on in 1995, a dedicated Teaching Unit affiliated with King Edward Medical College was established, with Dr. Aamir Mahmud Ijaz, then Assistant Professor of Thoracic Surgery, as its Chairman.

A 33 bedded department situated on the upper storey of the historic Mayo hospital Inpatient block known to all by the name of Gharri(گھڑی)ward, this unit gives 24/7 emergency cover to Thoracic Surgical patients, along with 4 OPD days and three elective OT days, out of which one day is reserved for Bronchoscopies and cases operated under local anesthesia, and two days reserved for major Thoracic Surgical undertakings requiring General Anesthesia. The department is currently being headed by Dr. Syed Muhammad Raza Naqvi, Associate Professor of Thoracic Surgery King Edward Medical University, with Postgraduate trainees in both MS as well as FCPS Thoracic Surgery training programmes, receiving their specialized postgraduate training in this technically demanding surgical discipline.

Today marked the achievement of yet another milestone in this direction, when, funded by Professor(retd) Dr. Aamir Mahmud Ijaz in its entirety, a state of the art Thoracic Surgery Skills Lab was inaugurated by the Vice Chancellor King Edward Medical University, Professor Dr. Khalid Masood Gondal. The occasion was also graced by Professor Dr. Aamir Mahmud Ijaz, Dr. Asghar Ali Chaudhry, Professor Dr. Saqib Saeed, Dean King Edward Medical College and CEO Mayo Hospital, Dr. Munir Ahmed Malik MS Mayo Hospital and Dr. Irfan Azmatullah, Chairman Department of Cardiac Surgery, King Edward Medical University.

Fortunately it was our Head of Department, Dr. Syed Muhammad Raza Naqvi's Birthday as well, so a Bithday cake cutting ceremony was also been graced by the Worthy VC KEMU PROF. Dr. Khalid Masood Gondal , Prof. Dr. Aamir Mahmud Ijaz, Dr Asghar Ali Chaudhary, MS Mayo Hospital and CEO Mayo Hospital.

18 years old female presented as an outpatient with presenting complaints of bilateral upper limbs pain and numbness for past one month.

Nerve conduction study (NCS) showed right Brachial plexopathy.

CXR neck and upper chest showed bilateral Cervical Ribs.

Cervical ribs are supernumerary or accessory ribs arising from the seventh cervical vertebra. They occur in ~0.5% of the population, are usually bilateral, but often asymmetric , and are more common in females.

Although cervical ribs are usually asymptomatic, they are the most important anatomic rib variant clinically, because they can cause thoracic outlet syndrome by compression of the brachial plexus or subclavian artery/vein.

Right sided Cervical Rib was excised via supraclavicular approach. Patient remained uneventful post operatively and was discharged home. She followed up in Outpatient Department after 6 weeks and her numbness and pain of right sided upper limb vanished away post operatively.

Her left sided Cervical Rib was excised via supraclavicular approach after 2 months of 1st surgery. Her post op recovery remained uneventful and she was discharged home 3rd post op day. No numbness and pain in both upper limbs is observed after her surgery now.

45 yrs old female admitted through OPD with the presenting complaints of Shortness of breath, Cough and left sided chest pain for 1 month. Shortness of breath was usually encountered at night time and was relieved by sleeping in left Lateral position. Her CXR showed left sided lower lobe atelectasis with Bronchiectasis.

CT Scan Chest showed "A well defined avidly enhancing soft tissue density mass measuring 13x1.1cm is seen in carina extending into left main bronchus".

Flexible Fibreoptic Bronchoscopy was planned. Endobronchial Tissue Biopsy of the mass was sent for Histopathology which confirmed our Diagnosis of Endobronchial CARCINOID tumor (Well Differentiated Neuroendocrine Tumor).

~~~~~LEFT HORIZONTAL (ACCESSORY LINGULAR) MINOR FISSURE~~~~~~

Lung fissures are a double-fold of visceral pleura that either completely or incompletely invaginate lung parenchyma to form the lung lobes.

Each lung has an oblique fissure separating the upper lobes from the lower lobes and the right lung has a horizontal fissure that separates the right upper lobe from the middle lobe.

There are numerous accessory fissures that are common anatomical variants.

Accessory fissures of the lung usually occur at the borders of bronchopulmonary segments. They are common normal variants but are less commonly seen on imaging.

Some of the more common accessory fissure include:

☆ Azygos fissure: most commonly seen accessory fissure
☆ Left horizontal fissure
☆ Inferior accessory fissure
☆ Superior accessory fissure

Young female, presented with complaints of fever, right sided chest pain and productive cough for past 6 weeks.

CT scan Chest with IV contrast showed right hydropneumothorax with thickened parietal pleura and diseased lung.

Flexible Fibreoptic Bronchoscopy was performed for Bronchoalveolar lavage. MTB gene expert of Bronchoalveolar lavage came out to be negative.

Pleural Tissue Biopsy was planned via Right Thoracotomy under General Anesthesia.

Patient presented to us with complain of productive cough with yellowish color sputum since 10 years fever off and on.

Patient is having history of TB 9 year back for which patient have taken 9 month course of ATT.

On examination : course crepts right lower side of chest , decrease air entry on right side with displaced apex beat.

PFTs shows moderate severe bronchodilatation with ( FEV1

FAREWELL TO LEGEND OF PROF AAMIR MAHMOOD IJAZ sb.

Professor Dr. Aamir Mahmood Ijaz Professor of Thoracic Surgery KEMU was born on 20th of September 1961 to a family of renowned medical professionals. He passed his Matriculation exam in 1976 from Divisional Public School and after that he passed his Intermediate exam in 1978 from Divisional Public College. He got admission to the prestigious King Edward Medical University Lahore. Official following in the footsteps of his maternal grandfather, teacher of teachers and the pioneer of the art and science of General Surgery, Professor Dr. Ameer ud Din, and his father, the pioneer of the discipline of Thoracic Surgery in Pakistan, Professor Dr. Ijaz ul Hassan. He knew from the very first day that he had some real big shoes to fill in, and he did exactly that after graduating in 1985, as he started his journey to become a just like his father. He completed his post graduation in this discipline in 1992. After working for some time as Senior Registrar in the Thoracic Surgery Department, King Edward university Mayo Hospital Lahore, he left for the United Kingdom for further training in this rare specialty, where he spent three years of vigorous training in the best thoracic surgery centres under the mentorship of one of the the best thoracic surgeons in the world, including Professor Dr. Peter Goldstraw. He returned to Pakistan to serve his own people in his parent institution and was appointed the in Punjab, and he was posted in the Thoracic Surgery Department, King Edward university Mayo Hospital Lahore, where he established the first teaching unit in this discipline in Punjab, as the Head of Department in 1995. He single handedly nurtured this specialty with very limited human resource and facilities, and his untiring efforts bore fruit as this department started flourishing. He remained the Head of this department for 26 years, right until the day of his superannuation on 19th of September, 2021. During his career in thoracic surgery, his noteworthy contributions include training postgraduate trainees in the art and science of Thoracic Surgery, introduction of minimally invasive Thoracic Surgery, establishing a full faculty department by getting the regular seats for Assistant Professor, Associate Professor and Professor approved by the Health Department Government of the Punjab after a long perseverant effort that began in 2004 and bore fruit in 2018. Under his leadership, this department got recognized for in Thoracic Surgery, in 2020, with special contribution and assistance of our worthy Vice Chancellor, Professor Khalid M Gondal (TI).

Professor Dr. Aamir Mahmud Ijaz has been a kind, gentle and extremely supportive human being towards his colleagues and peers, his trainee residents and junior doctors, para medical staff and especially towards his patients. He has been known in this institution as a humble individual with an extremely pleasant personality, who is always willing to help anyone who is in need in any and every possible way. He was given a graceful and well deserved farewell on on the occasion of his superannuation. The ceremony was hosted by the Thoracic Surgery Department, King Edward university Mayo Hospital Lahore in the conference room of Surgical Tower Mayo Hospital Lahore.

The Vice Chancellor Professor Dr. Khalid M Gondal sb graced this ceremony with his presence as the Chief Guest, alongside Dr. Iftikhar Ahmad, , and a very close friend of Professor Dr. Aamir Ijaz, Professor Dr. Syed Asghar Naqi, , Professor Dr. Zara Ishrat of the , Professor Dr. Farrukh Afzal of the , Professor Dr. Mahjabeen Masood of the Interventional Radiology KEMU - Mayo Hospital Lahore , Dr. Irfan Azmatullah Associate Professor and Head of Cardiac Surgery Department mayo Hospital Lahore, Dr. Rana Wajid Ali, and Dr Salman Kazmi along with other members of staff, to name a few.

Professor Dr. Aamir Mahmud Ijaz was presented with shields of honor by the worthy Vice Chancellor and by the Department of Thoracic Surgery King Edward Medical College. Thoracotomy & Excision of Schwanomma was done.

A schwannoma is a tumor that grows in the sheaths of nerves in your peripheral nervous system(the parts of your nervous system that aren’t in your brain or spinal cord). Schwannomas are usually benign, meaning they’re harmless. In rare cases, they can be malignant, or cancerous. Malignant schwannomas are also called soft tissue sarcomas. Schwannomas usually don’t produce symptoms until they become large enough to put pressure on the nerves around them. You may feel occasional pain in the area that’s controlled by the affected nerve.
