This 55 year old gentleman came to us with low back pain and severe bilateral leg pain secondary to degenerative disc disease at L5/S1 level with listhesis. The L5-S1 motion segment has distinctive anatomy and receives a higher degree of mechanical stress and loads compared to the segments above.These characteristics may make L5-S1 susceptible to traumatic injuries, degeneration, disc herniation, and/or nerve pain.
Lumbar interbody fusion procedure performed: A degenerated disc is removed and L5-S1 vertebrae are fused together with implants interbody cage and pedical screws.

This lady came to us with pain in her arm and neck which was unbearable for her, she had clear signs of Spinal Cord Compression. Dr. Athar M Siddiqui, Consultant Spinal Surgeon National Hospital Defence, talks about the case of a 70-year old male patient presented to us with Acute on chronic progressive bilateral leg paraparesis. His history of weakness spans over a year that was progressively getting worse. On top of it, he had a fall 4 months ago following which his leg movements completely stopped bilaterally, with just sensation below D7 level.
When we met him in OPD, his MRI scan shows traumatic cord compression at D7/8 Level on a background of infective erosion of vertebral Body and disc space most likely TB. He was been given a 10-20% chance of neurological recovery following surgery, however, the surgery was performed to clear the infection, stabilize spine, and prevent further kyphotic deformity for his rehabilitation in the future.
Following surgery by the grace of GOD, his legs started moving and eventually at 6 weeks mark as his wound completely heeled, his spine stabilized, and power in his key myotomes in lower limbs improved to 4+/5 BILATERALLY in L2/L3/L4/L5/S1 from almost 1/5. This is an excellent recovery and a case report on its own. Further transpired that timely intervention can change the patient's situation with the right treatment in expert and trained hands.

Loss of dexterity i.e fine motor day to day skills in performing tasks with hands and mind like buttoning up a shirt, brushing the teeth, difficulty in properly eating food, losing grip in holding a teacup, etc.
These were clear signs of Spinal Cord Compression. Subsequently, MRI Spine confirmed a compressed cervical spinal cord at 3 levels. A minimally invasive Cervical Spine Cord Decompression was done using an anterior approach. Blood loss was less than 100 ml and she was on her feet with a steady gate from day 2 post-op. Her motor skills are recovering as we speak and she is extremely grateful and pleased with the outcome of her challenging surgery and timely management.

This 72 year old woman had a fall at her home and she sustained a fracture in her lumbar spine and was unable to walk. She was taken to a big renowned hospital locally, where she was advised to wear a jacket and a prolonged bed rest was suggested, which was inconvenient for her and she was brought to Dr. Athar Muniruddin Siddiqui.
Dr. Siddiqui after taking her other medical conditions (i.e. diabetes, high B.P etc) under consideration advised a minor surgical procedure under local anesthetic, which not only improved her mobility and relieved pain but saved her from the risk of other complications such as bed sores, chest infection, clotting and inconvenience of prolonged bed rest. The procedure took 10-15 minutes in total and she walked out of theatre on her feet and there is just a needle sized scar at her back. Given the skeletal immaturity and by looking at her MRI scan lumbar spine and nature of the curve, if not intervene surgically the curve will continue to progress and will be causing disability, disfigurement, and heart and lung function compromise in future.
Hence, deformity correction surgery was performed using CE Marked approved international deformity correction system and neuro-monitoring. Full correction on both AP and lateral planes achieved. Postoperative x-rays are reflective of excellent correction.
By the 5th post-op day our patient was home and walking with no change in neurology. We are Alhamdullilah very pleased with the outcome of the Surgery as it’s an extensive and comprehensive international standard spinal surgery and is available in very few centers within the country.

He had a traumatic injury to lower thoracic and upper lumbar spine following a road traffic accident when a rickshaw ran over him.
His L1 vertebrae completely crushed at the time of the accident causing compression at conus medullaris. This gave him bilateral lower limb weakness. More pronounced in the left leg and progressive traumatic kyphoscoliosis. Meaning both bending and tilting of the lower lumbar spine. Causing pain and disability.
As he was presented with progressive weakness and deformity, Dr. Athar and the team focused on to correct his deformity, decompress and stabilize the traumatic spine surgically. Immediate appropriate spinal surgery was performed. The neurological deficit was over 30 days old and generally doesn't recover after 48 hours. By the grace of GOD, he recovered in less than a month following surgery.
A very pleasing outcome for such a challenging injury.
