RH incompatibility, is a sad reality in Pakistan. It is avoidable by giving anti D prophylaxis between 28-32 weeks of gestation, single dose of 300ugm is good enough followed by a repeat dose after delivery of baby, if baby is RH positive!!!
Inuterotransfusion is the only treatment available for RH incompatibility.
It’s very important to have a steady hands with continuous visualization of needle!!!

Autosomal recessive polycystic kidney disease (ARPKD) is a rare genetic disorder that affects 1 in 20,000 children. A fetus or baby with ARPKD has fluid-filled kidney cysts that may make the kidneys too big, or enlarged. ARPKD can cause a child to have poor kidney function, even in the womb.
It can be diagnosed before 15 weeks by prenatal genetic testing.
Un fortunately, Pakistan is a country where genetic testing is neither funded by the government nor insurance!! Diagnosis by ultrasound can be made around 22 weeks and later!!
Sadly, we have to rely on scan for diagnosis and by that time the emotional bonding so strong!!!

Role of Genetic sonogram

A 38 year old woman referred to us at 28 weeks of gestation with early onset fetal growth restriction, fetal renal pelvicalcyceal dilation, persistent choroid plexus cyst and polyhydromnios.
Her age related risk for trisomies was 1 in 207
By adding above soft markers as a part of genetic sonogram increased her risk for Trisomies to 1 in 41.
We did her amniocentesis and the results showed Trisomy 18, Edwards syndrome.
Screening for Trisomies in first trimester is done universally but in some cases genetic sonogram can be helpful in risk assessment of Trisomies.

Congenital diaphragmatic hernia (CDH) occurs when there is a hole in the diaphragm, which is the thin sheet of muscle separating the chest from the abdomen The majority of CDHs occur on the left side. The hole allows the contents of the abdomen (stomach, intestine, liver, spleen, and kidneys) to go up into the fetal chest. They should be delivered in a perinatal center with a Level III intensive care nursery

In the best cases, some infants do very well with treatment after birth, surgery and care in an intensive care nursery On the other end of the spectrum, babies with severe CDH and very small lungs can have difficult challenges after birth, and some will not survive.

Another case of TRAP (Twin Reverse arterial Perfusion) delivered today. The little one weighed 1.5 kg whereas the acardiac monster weighed 3.75kg.
Cord Occlusion is one and also the right management but in centers where it’s not done or are referred late can be managed expectantly by taking into account hemodynamics of the normal fetus.

Combined or Triple test ( False positive rate of 5%) and NIPT are all for screening only and if result shows high risk for trisomies they need to confirmed by either CVS or Amniocentesis depending on gestational age.
It’s a relief for the couple when results turn out negative for trisomies.

Congenital complete heart block usually occurs as an isolated anomaly but can be associated with other congenital problems with the structure of the fetal heart.
This patient was referred from Gujranwala for prenatal assessment at 32 weeks of gestation. On assessment fetus had complete heart block leading leading to fetal pericardial effusion (fluid around heart). Fortunately, there was no fetal ascitis. We decided to deliver the fetus immediately after steroid cover to give max chance of survival. Otherwise it would have lead to hydrops and fetal/neonatal death. Doing some Rights: right prenatal diagnosis, right time to deliver and right place to deliver ( NICU has paediatric cardiac support ) can contribute to survival.
Little Miss, weighing 1100gm is celebrating her 30 days of life today. ❤️

We present a case referred to us with STAGE 2 early onset FGR at 29 weeks of gestation with absent end diastolic flow.
We monitor STAGE 2 FGR in our unit with daily dopplers and prolonged pregnancy for about 3 weeks. We had to take decision to deliver at 32 weeks of pregnancy with no interval growth. Steroid cover with dexamethasone was completed 24 hours before planned delivery.
Little Miss kour was born weighing 1100gm. She stayed in nursery for 7 days and later celebrated her blessings in gurd dawara 😊
