Indwelling Pleural Catheter(IPC) Relieves Dyspnea More Than Talc Pleurodesis

Barbara Boughton

June 1, 2012 (San Francisco) — The first randomized controlled trial comparing indwelling pleural catheters (IPCs) with talc pleurodesis for malignant pleural effusion suggests that IPC is superior for controlling dyspnea and reducing hospitalizations, researchers reported here at the American Thoracic Society (ATS) 2012 International Conference. The study was also published online May 20 in JAMA.

"Indwelling pleural catheters put the patient in control of their symptoms in what is essentially a palliative situation, but there's also an ongoing nuisance factor. A reminder of the patient's ongoing malignant disease remains hanging out from [his or her] chest," said presenting investigator Jamal Grayez, MB, B*h, from Horton Hospital in Banbury, United Kingdom.

Guidelines recommend chest tube and pleurodesis as first-line treatment for pleural effusions, and talc is considered the most effective of these agents. Until now, IPC has been considered second-line therapy.
