Typical features include:
• Red, fleshy appearance
• Bleeding with minor touch

Diagnosis: Behçet disease✅

1. Recurrent oral ulcers (canker sores)

Painful, recurrent aphthous ulcers are the most common and earliest feature of Behçet disease.

They often look like ordinary canker sores but recur frequently.

2. Ge***al ulcers

Behçet causes recurrent ge***al ulcers that are usually deeper than oral ulcers and may heal with scarring.

The combination of oral plus ge***al ulcers is a key diagnostic clue.

3. Uveitis (eye involvement)

Behçet is a vasculitis, so it commonly affects the eyes.

Uveitis can be anterior or posterior and may threaten vision if untreated.

Eye disease strongly supports Behçet over simple aphthous conditions.

4. Erythema nodosum–like lesions

Painful, red, tender nodules on the legs are classic skin findings.

They reflect underlying inflammation of blood vessels.

Putting it together

The triad of oral ulcers + ge***al ulcers + eye inflammation, along with erythema nodosum, is classic for Behçet disease and is unlikely to be explained by a single infection or isolated condition.

Pathophysiology (brief)

Behçet disease is a chronic, relapsing, multisystem inflammatory vasculitis.

It affects small and large blood vessels.

More common along the “Silk Road” (Middle East, Mediterranean, Central and East Asia).

Key teaching pearl

> Recurrent oral ulcers alone are common.
Add ge***al ulcers and uveitis → think Behçet immediately.

Next steps (clinically)

Diagnosis is clinical (no single definitive lab test).

Assess for:

Eye involvement (urgent ophthalmology referral)

Neurologic or vascular disease

Treatment depends on severity and may include:

Colchicine

Corticosteroids

Immunosuppressants or biologics

One-line exam answer:

> “Recurrent oral and ge***al ulcers with uveitis and erythema nodosum are diagnostic of Behçet disease.”

Absent Abdominal Reflex on the Left Side in a Young Patient

The superficial abdominal reflex is a simple bedside neurological test, but it carries important clinical value.
Loss of this reflex on one side should never be ignored. It may suggest an underlying neurological problem such as spinal cord pathology, nerve involvement, or an upper motor neuron lesion.

Careful clinical examination often gives the first clue before imaging and advanced tests confirm the diagnosis. Early recognition helps in timely referral and management.

Clinical skills still matter. Always compare both sides.
