A 3-year-old child was admitted to the pediatric ward complaining of dizziness and fatigue. The patient also has a history of anemia. A complete blood count (CBC) was performed, and the results showed mild leukocytosis of 17 and anemia with a hemoglobin (Hb) level of 5.43.

Definition
Hairy cell leukemia (HCL) is a rare, indolent lymphoproliferative neoplasm of mature B cells with a distinct clinical presentation that includes peripheral blood cytopenias, splenomegaly and a small number of circulating neoplastic cells with hair-like cytoplasmic projections.

Histopathology
Extensive marrow fibrosis as demonstrable by reticulin stain; however, neoplastic hairy cells, as described in the peripheral blood description, can be seen. (first image)

Peripheral smear
Pancytopenia with rare hairy cells that are medium in size with moderately abundant pale blue cytoplasm, reniform nuclei, open chromatin, absent nucleoli and a characteristic serrated cytoplasmic border. (last three images)

A blood group test is a lab procedure used to identify a personโ€™s blood type based on the presence or absence of specific antigens on the surface of red blood cells. The main system tested is the ABO system (A, B, AB, or O) and the Rh factor (positive or negative).

How to read result
If there's an agglutination reaction

In Anti-D, it's O positive
In none of them, it's O negative
In Anti-A and Anti-D, it's A positive
In Anti-A only, it's A negative
In Anti-B and Anti-D, it's B positive
In Anti-B only, it's B negative
In Anti-A, Anti-B and Anti-D, it's AB positive
