A young boy with complaint of generalized abdominal pain from last 4 days .History examination and radiological finding suggestive of perforated appendix with mild free fluid in pelvis.laprscopy done about 500 ml pus in abdomen with appendix perforated at tip. Appendectomy done with abdominal lavage . Alhamdolillah pt fine on follow up visit .May Allah bless the pt complete recovery .ameen.

Maintenance of blood supply to distal part until proper expertise or to get the time to save the distal part from ischemia blood supply can be maintained with help of simple drain connecting both proximal and distal part.A young boy with RTA injury to upper limb muscles nerves and brachial artery cut. Artery repaired with reverse great saphenous vein graft median nerve also repaired bone was intact.before repair blood supply was restored through drain to prevent ischemia then reverse vein graft applied .May Allah bless pt with complete recovery.ameen.

Difficult case of laproscopic cholecystectomy of pt from dubai for empyema gall bladder at same time pt had pancreatitis managed for pancreatitis when enzymes settled lap chole done in same admission .
