ℹ️ Cholangioscopy is an endoscopic technique that allows direct visualization of the biliary tree (intrahepatic and extrahepatic bile ducts).
🔹 Indications for Cholangioscopy
Cholangioscopy is indicated for both diagnostic and therapeutic purposes.

1️⃣ DIAGNOSTIC Indications
A. Indeterminate Biliary Strictures
• When imaging (ERCP, MRCP, CT) and/or brush cytology or biopsy are inconclusive.
• Obtain targeted biopsies under direct vision.
B. Evaluation of Intraductal Lesions
• Characterization of intraductal masses, polyps, or papillary tumors (e.g., intraductal papillary neoplasm of the bile duct – IPNB).
• Determining extent of tumor involvement before surgery.
C. Unexplained Filling Defects
• Clarify the nature of filling defects seen on cholangiography (stone, blood clot, sludge, tumor, or parasite).
2️⃣ THERAPEUTIC Indications
A. Bile Duct Stones (Difficult or Large Stones)
• Visualization-guided lithotripsy: Useful when conventional ERCP techniques fail (e.g., impacted or intrahepatic stones).
B. Guide wire placement across difficult strictures unable to manage by ERCP
C. Removal of migrated or fractured biliary stents or other retained materials.
D. Biliary Cast or Blood Clot Removal
