This patient underwent lumpectomy in periphery for a small lump without addressing da axilla. Post operatively margins came out positive with skin involement and multiple axillary nodes. So we did MRM with level 2 axillary dissection and preserving da major neuro vasculer structures. We are also planning for an LD flap once her chemo is completed in collaboration of our skillful plastic surgeon dr Zohaib Hidayat in sha ALLAH

Case report

This patient presented with history of ingestion of a nail in Ramzan. She was advised to wait for spontaneous pasaage of the nail but it never occurred rather she developed severe abdominal pain during last 2 weeks. She didn't have any other local signs or systemic toxicity. Initially she was reffered to gastroentrologist for endoscopic retrieval but they refused to do endoscopy as in x ray it was in da region of terminal illeum/ICJ. Also she had affordability issues and was brought through by 2 members of an NGO so CT couodnt be done as well as any other fancy investigations or treatment modality so it was decided to go in. Upon exploration via a grid iron incision it was found to be stuck in a broad based Meckel's diverticilum about 2 feet proximal to ICJ and was localized by omentum. It was removed through a small entrotomy which was closed layer on. She has recovered well and improving

اس مریض کے پتے میں پتھریاں تھیں اور ساتھ میں ھرنیا کی بھی تکلیف تھی اور موٹا پا بھی تھا جس کی وجہ سے اس کا پیٹ بھت زیادہ لٹکا ہوا تھا اور چلنے پھرنے نماز پڑھنے میں دشواری ہوتی تھی تو ایک ھی آپریشن سے اس کے سارے مسئلے حل کرنے کی کوشش کی یعنی پتھہ بھی ہٹا دیا ھرنیا کا آپریشن بھی ھو گیا اور لٹکے ھوے پیٹھ سے بھی نجات مل. گئ. یاد رھے کہ یہ سارا کام مریض کے لیے صحت کارڈ کے ذریعے بالکل مفت کیا گیا
