Under the visionary leadership of Chief Minister Balochistan, Mir Sarfaraz Bugti, with the unwavering support of Health Minister Bakht Muhammad Kakar and the constant guidance of Secretary Health Mujeeb Ahmed Panezai, BINUQ is proud to announce the successful completion of its second renal (kidney) transplant of the year.

In a remarkable act of love and selflessness, a 22-year-old younger brother has given his 25-year-old elder brother the precious gift of life by donating his kidney. The elder brother had endured the hardships of dialysis for the past two years. Both the donor and recipient, belonging to a deserving and underprivileged family from District Naseerabad, are recovering well, Alhamdulillah.

Notably, these critical transplant procedures are being carried out solely by BINUQ’s highly skilled team, without any external assistance.

This essential transplant service, supported by the Government of Balochistan, is now a continuous provision for all transplant patients in the province—entirely free of cost. Patients who would otherwise have to seek treatment in other provinces at ten times the expense can now access life-saving care within Balochistan.

Permcath is cuffed catheter with lower risk of infection and intended to be used long term these catheter can remain in place up to 12 months
Perms catheter is better option for patients on maintenance hemodialysis until permanent Access (AVF) not Created.
We usually use Ultrasound For Vein Puncture and C Arm for proper positioning of Catheter .

50 years old male diagnosed case of APKD and Renal failure.On Maintenance hemodialysis.
Complains of severe Rt sided abdominal pain and heaviness.
He was on regular injectables to control pain.
CT scan showed Bilateral Huge polycstic kidneys with internal haemorrhages.
Right kidney was Pushing Liver Upward and caudally Extending to Pelvis.
Planned for Right Nephrectomy
Kidney was adherent with Inferior surface of liver ,Peritoneum ,Psoas Muscle and IVC.
Difficult Nephrectomy around 30 *15cm kidney removed.

76 years old male with Complains of weak urinary stream,Nocturia sense of incomplete emptying of bladder.
Treated with alpha blockers for more then 7 months by G.P.
Initially showed response but from last two months he feels no improvement with medication.
US prostate with pre and post performed.
Prostate volume 70 grams with significant post void of 180ml.Urine culture was no growth other labs normal and
S.PSA 2.
Counselled for TURP and TURP performed under Spinal Anesthesia.
Foley's removed on 6th Post of day.
Histopathology benign prostatic hyperplasia.
Patient is doing well.

34 year old male History of laparotomy 6 months back post RTA. According to patient bladder and urethral repair was done.(No Record Available).He Was on suprapubic catheter drainage since last six months.Never Changed his Suprapubic catheter.
Presented with Suprapubic catheter Encrustation along with multiple Bladder Stones.
Using same Tract of SPC 30 fr amplatz sheet placed and With the help of pneumatic lithoclast Using Nephroscope complete clearance performed and Suprapubic Change.
Future Plan Antegrade and Retrograde Urethrogram and decide according.

Tumor Surgeries are Always challenging 37
years old lady from Afghanistan diagnosed as case of Renal Mass at Afghanistan. Mentioned as Metastatic disease Inoperable and After performing Trucut Bx they started TKIs.CT triphasic done and diagnosed as large Renal mass along with separate Nodal Mass. Right Radical Nephrectomy done along with Excision of Nodal mass Patient discharged home on 3rd post op day Biopsy awaited.

52 years Old male History of Right pyelolithotomy 20 years
back and lap Cholecystectomy 7 years back.
Presented with Rt Flank pain and on and off haematuria.Workup done and diagnosed as Right
Recurrent renal stones (staghorn) hydronphrotic Kidney and 38% on Renal scan.
Pyelolithotomy performed and Almost 100% clearance achieved.

When primary AVFs (Radio-cephalic, Brachio-cephalic fails Then there is Valuable option of Basilica Vein Transposition in Such patients .
Depends upon the caliber and other Anatomical factors BVT may be single stage or double stage.Arteriovenous fistula ( AVF) is life line of Renal failure patients on Hemodialysis.
When primary AVFs (Radio-cephalic, Brachio-cephalic fails Then there is Valuable option of Basilica Vein Transposition in Such patients .
