A 50-year-old male patient presented with complaints of weakness of all four limbs. He was a software developer by profession. On examination power was 3/5 at C5-c7. Tendon jerks were brisk and Hoffmann sign was positive. A diagnosis of spondylotic myelopathy was made. Keeping in view the multiple level issue and ossified posterior longitudinal ligament the patient was operated using posterior approach with laminectomy at C3, 4, 5, and C6 with lateral mass fixation from C3 till C7. The recovery was uneventful and postoperatively patient had full power and had marked improvement in upper limb function. Our life has been made a little comfortable with oscillating drill which is of great help around the lateral mass and prevents tissues being rolled and avulsed. I am grateful to the team who looked after the patient not only during surgery but also provided excellent post op care in intensive care unit.
