Understanding Neck Pain: Could It Be Your Facet Joints?

Not all neck pain comes from a slipped disc. A very common but often overlooked cause is facet-mediated pain — pain arising from the small joints at the back of your neck.

Here is what you need to know:

What are facet joints?
They are small joints that guide movement of your neck. With age, posture issues, or injury, these joints can become irritated or worn out.

How does facet pain feel?
• Aching or stiffness in the neck
• Pain going to the shoulder or between the shoulder blades
• Worse with looking up, turning, or bending to one side
• Better with rest or gentle flexion
• Usually no numbness or tingling in the arms

Why is this important?
Facet pain is often mistaken for “disc disease” or “muscle spasm.” Correct diagnosis prevents unnecessary medicines, wrong injections, or even unnecessary surgery.

How is it diagnosed?
A specialist identifies the pain pattern and may use a small numbing injection (medial branch block) to confirm the source.

How is it treated?
• Targeted physiotherapy
• Posture correction
• Medial branch blocks
• Radiofrequency ablation for longer-lasting relief
• Surgery is rarely needed

The History Behind Colles’ Fracture

Most orthopedic surgeons diagnose a Colles’ fracture within seconds—but the history behind it is far more remarkable than the quick label we use today.

In 1814, long before X-rays existed, Abraham Colles, an Irish surgeon, published a short but groundbreaking paper titled:
“On the fracture of the carpal extremity of the radius” in the Edinburgh Medical and Surgical Journal.

With nothing more than clinical observation, Colles described:
• The classic dinner-fork deformity
• Dorsal displacement and angulation of the distal radius
• Preservation of pronation and supination despite deformity
• The tendency for the deformity to persist, yet function to remain surprisingly good
• Its high prevalence in elderly women after falls on an outstretched hand

He defined—accurately and completely—a fracture pattern 80 years before radiography.
His description was so precise that modern imaging simply confirmed what he had already concluded from careful examination alone.

Today, over 200 years later, every wrist X-ray with dorsal angulation pays silent tribute to the clinical brilliance of Abraham Colles.

Dupuytren’s Contracture

If you’ve noticed a thickened cord or a finger slowly bending into your palm—and you can’t fully straighten it—you may be dealing with Dupuytren’s contracture.

This isn’t caused by overuse. It’s a gradual thickening and tightening of the palmar fascia, often linked with:
• Family history
• Diabetes
• Chronic alcohol use
• Smoking
• Northern European ancestry

Early signs are easy to ignore, but the progression is not. Once the finger starts contracting, it rarely reverses on its own.

• Usually painless
• Affects ring and little fingers most
• Early treatment gives better outcomes
• Options include collagenase injection, needle fasciotomy, and fasciectomy depending on severity
