We would like to present three consecutive cases of scalp avulsions treated with replantation carried out by reconstructive and microsurgical team NIO Gliwice in the years 2017-2021. The superficial temporal vessels were the recipient vessels of the first choice. In all cases the interposed vein grafts were applied to obtain tension-free anastomoses. The most common postoperative complication was a partial necrosis within nuchal region (angiosome supplied by transverse cervical vessels).

We would like to present a case of 11 year old boy with osteosarcoma of tibia. After excision of the osteosarcoma tumor there was around 20 cm of missing bone tissue. Reconstruction was preformed using double fibula flap. One (ipsilateral) was harvested as pedicled and the other one (contralateral) was anastomosed with anterior tibial artery and vein using coupler technique. Both fibula flaps, connected with titanium plates, after osteosynthesis in the recipient site, provided complete support for the body weight in the lower limb. The postoperative course was without complications

The reconstruction was performed by combining the experience of the NIO Recon Team Gliwice and the surgical team from Institute of Mother and Child in Warsaw.

We would like to present two cases of a 65 and 67 year old patient with advanced squamous cell carcinoma of the lower lip. After tumor resection, the defect was reconstructed with free functional gracilis flap and covered with split thickness skin graft. In both cases pedicle was anastomosed to the facial vessels and the motor nerve from gracilis muscile was anastomosed to marginal mandibular branch of facial nerve (CN VII). In the comments section you can find a video with drinking test during late follow up, which confirms functional reconstruction.

We would like to present the case of a 46 year old patient with dermal Cylindroma of the scalp (Turban Tumour). After total resection, the defect was reconstructed with omental flap and then covered with numerous split thickness skin grafts. The pedicle was anastomosed to the superficial temporal vessels.

Reconstruction – A. Maciejewski MD, PhD, D. Walczak MD

Type of reconstruction –

1. Omental flap
2. Split thickness skin grafts

We would like to present the case of a 53 year old patient with advanced squamous cell carcinoma of the nasal septum mucosa. After total nasal resection, the defect was reconstructed with RFFF and two auricular flaps, anastomosed together as a artificial sequential chimeric flap. The pedicle was anastomosed to the facial vessels. In the second stage of reconstruction, nasal tissue was covered with forehead flap in order to obtain the best aesthetic effect.

Resection – C. Szymczyk MD, PhD
Reconstruction – A. Maciejewski MD, PhD

Type of reconstruction –
1. Artificial sequential chimeric flap – RFFF (Radial Forearm Free Flap) + double AFF (Auricular Free Flap)
2. Forehead flap

We would like to present a case of a 72-year-old male with malignant melanoma recurrence in the parieto-temporal region and metastases to the left cervical lymph nodes. The resection included a considerable part of the scalp, the superficial lobe of the left parotid gland and levels I-IV of the left cervical lymph nodes en bloc. An ALT flap was raised using the KISS technique and both skin paddles were rearranged side-by-side to match the exact size and contour of the defect. The pedicle was anastomosed to the superficial temporal vessels.

We would like to present the case of a 51-year-old female with radiation necrosis after breast cancer treatment. Complete defect of the chest wall with exposure of the pleura and pericardium (see the video in comments). Primarily, the defect was reconstructed with pedicled TRAM flap complicated with partial necrosis of the zone IV. Subsequently, the attempt of residual wound closure with NPWT and skin-grafting was unsuccessful. Contralateral pedicled IMAP flap was applied to cover the remained part of the defect. After the wound has healed completely, the flap modeling was performed and defined the inframammary fold. Patient is already scheduled for the NAC reconstruction (artificial one in the photo).
