Orthopedics is a field full of innovation
Here is my version of a technical anterior cruciate ligament (ACL) reconstruction, presented within the concept developed by Robert Śmigielski, Rainer Siebold, Christian Fink.
Our paper published in 2023 demonstrates a simple yet precise technique using: standard arthroscopic instruments, 1–2 drill bits (5 and 5.5 mm),
aiming guides available in any ACL reconstruction set. Smith+Nephew, Marc Chamieh , Wassim Mourad
The use of the Ultra Star system by Smith & Nephew enables graft fixation on the cortices, allowing for anatomical reproduction of the flat femoral footprint and the C-step in the tibia.
This technique leads to improved clinical outcomes and more stable results.

Last week was a truly busy and inspiring time — two capitals in just a few days: Rome and Istanbul.
First, I attended a conference on cartilage, and then I had the honor of being an invited guest at the XVII Turkish Congress of Sports Traumatology, Arthroscopy, and Knee Surgery.
I shared our achievements, methods, and techniques developed at Rehasport Clinic | Poznań in the treatment of , , and joint injuries.
Meeting friends and colleagues in these two beautiful cities of the world gives me new energy and motivation for further creative work.

✨ Sharing a milestone from my surgical journey ✨
Several years ago, together with Ciemniewska-Gorzela MD PhD, and later modifying the technique alongside Paweł Bąkowski, MD PhD, we developed a surgical method that has saved the function of many patients.
This technique allows us to reconstruct the tendon in the most challenging situations – repeated ruptures, lack of tendon continuity, severe atrophy – when traditional options offer no solution.
We proved that patients can walk without crutches and start exercising as early as 2 weeks after surgery, while in other techniques they remain immobilized for months.
Today, together with Bartosz Kiedrowski and Jakub Kaszyński MD PhD, Paweł Bąkowski, MD PhD , Naczk MD PhD Rehasport TEAM we are proud to present the functional outcomes as well as the rehabilitation protocols we designed. These methods ensure that patients not only recover but can also return to sport.

This week we had the pleasure of welcoming distinguished scientists Artur Banach and Ross Crawford from The Prince Charles Hospital, Brisbane (Australia) to our operating theatre at Rehasport.
A year ago, we jointly published the first article in a series in Arthroscopy, Sports Medicine, and Rehabilitation, addressing one of the major causes of patellar cartilage injuries. Our ongoing collaboration now focuses on finding the most effective treatment strategies for patients.
Our work is bringing tangible results – both in the form of successfully treated patients and the development of innovative surgical techniques that address the underlying biomechanical challenges.
Spending time together in the operating theatre was truly inspiring. For us as scientists, such collaboration and discussion are the best ways to exchange ideas, challenge perspectives, and seek the truth.

Silnik gigantycznego statku zepsuł się i nikt nie mógł go naprawić, więc zatrudnili inżyniera mechanika z ponad 30-letnim doświadczeniem.
Bardzo dokładnie obejrzał silnik, od góry do dołu. Po obejrzeniu wszystkiego, inżynier wypakował torbę i wyciągnął mały młotek.
Delikatnie w coś uderzył. Wkrótce silnik znów ożył. Silnik został naprawiony!
Tydzień później inżynier wspomniał właścicielowi statku, że całkowity koszt naprawy gigantycznego statku wyniósł 20 000 dolarów.
„Co?!” powiedział właściciel. „Prawie nic nie zrobiłeś. Daj nam szczegółowy rachunek".
Inżynier odpowiedział mu: „Odpowiedź jest prosta:”
Pukaj młotkiem: 2 dolary
Wiedza, gdzie pukać i ile: 19 998 dolarów
Ważne jest docenianie czyjejś wiedzy i doświadczenia... ponieważ są one wynikiem zmagań, eksperymentów, a nawet łez.
Jeśli wykonam pracę w 30 minut, to dlatego, że spędziłem 20 lat, ucząc się, jak to zrobić w 30 minut. Jesteś mi winien lata, a nie minuty.

This weekend, we held another SNACE cadaver course — this time for doctors from the Czech Republic. 🇨🇿
It was a great pleasure to lead the course for our Czech colleagues together with my friends from Plzeň, Petr Zeman, and from Prague, Jakub Kautzner.
A big thank you, as always, for the excellent organization and cooperation to the Czech team — Petra Ghanem and Ivona Felbingerová, to the Warsaw LAB team, and of course to Dorota Wojciechowska, the brain behind the operation.

Persistent deep ankle joint pain during or after sports?
When symptoms don’t improve and the pain continues to increase, one possible cause is damage to the articular cartilage of the talus.
Here’s an example from the case of a professional football player:
Despite undergoing ankle arthroscopy, debridement of the cartilage defect, and removal of osteophytes – he still could not return to sport. The pain persisted.
✅ Our proposal: cartilage reconstruction using the + technique.
Thanks to a posterior arthroscopic approach, the entire reconstruction can be performed fully arthroscopically, without the need for open joint surgery or tibial osteotomy – which are still used in many centers to access the talus.
Minimally invasive, function-preserving, and anatomically precise.

🔍 What happens when a large cartilage defect is improperly treated with microfractures?
The answer is unfortunately clear: persistent pain caused by bone marrow edema and subchondral cyst formation.

💡 Here’s the solution:
Do not perform microfracture in defects larger than 1 cm.
Instead, use the AMIC+ technique – a reliable, evidence-based approach that promotes true cartilage regeneration.

🇱🇻🇵🇱 Today marked the first day of our two-day advanced surgical course on the treatment of knee and shoulder injuries, organized for our orthopedic colleagues from Latvia.
This outstanding Surgeon-to-Surgeon educational program, hosted by Rehasport and Smith+Nephew Medical Education, follows the curriculum of the School and Akademia Rehasport , creating the perfect platform to share expertise and introduce the latest surgical techniques.
I had the pleasure of working alongside my colleagues Jakub Naczk and Patrick Buliński to teach our approaches to:
Knee joint surgery,
ACL reconstruction,
Lateral meniscus stabilization (including repair of bucket-handle tears),
Arthroscopic treatment of osteochondral defects using + technique – with cancellous bone grafting and collagen membrane (by ),
And hip arthroscopy.
Tomorrow, our Latvian colleagues Janis Voznesenskis and Zangus Pilaps will participate in surgical procedures led by Professor Przemyslaw Lubiatowski.
