Pinhole Surgical Technique
Just a couple of 2-3mm pinholes.
This treatment modality is done to gain root coverage, thicken the amount to soft and hard tissue for patients that present root exposure.
Often patient like these complain of root sensitivity and are advised to brush with a softer toothbrush and utilize root sensitivity toothpaste.
Other patients receive restorative treatment by removing tooth structure and adding composite filling materials to cover up the areas of root sensitivity.
If the issue is lack of soft tissue covering a root, why not consider soft tissue alternatives for root coverage.
There are other options. This is the least invasive available.

Soft tissue development is crucial for implant hygiene and esthetics. This can be done in a delayed approach, although in this case patient had molars removed and everything was done the same day. Same day tooth removal with surgical implant placement. Bone grafting and soft tissue emergence profile was developed in a single surgical procedure.

Don’t wait till it’s too late.
Tooth replacement is not something you do for vanity, not all dentist aren’t trying to trick you into replacing teeth in a big financial conspiracy.
Tooth replacement is needed on most occasions to avoid tooth movement , rotation, migration all of which may lead to tooth loss.
Teeth look for a way to accommodate and in many occasions tooth movement can compromise previously good healthy teeth.

Gingival flap rotation is necessary in most dental implant cases in order to increase the amount of attached gingiva and depth of vestibule around a submerged dental implant. This increase in tissue is done without the need for soft tissue augmentation procedures. Bone profiling is done to avoid forcing the healing abutment to seat inadequately causing impingement on the supracrestal attachment fibers of the gingival contributing to screw loosening and bone remodeling. Giving the patient a more hygienic implant crown connection and allowing the dentist to create long term hygienic restorations.

72 hour Post Operative visit. Pinhole Surgical Technique. Minimally invasive soft tissue grafting procedure for mucogingival defects. Patient reports only taking pain medications the first day. Zero pain reported after that. Noted extraoral swelling and intraoral gingival swelling is anticipated and normal as well. Pinhole perforations are nearly fully healed.

Implant Parallelism is key to make restorations work long term.

Today we place two implants for two different teeth in the same quadrant with one natural tooth in between them. Tomorrow these implants may be utilized to replace the tooth in between them if this tooth in the future needs to be removed. Having alternatives is good. Never look at short term victories when it comes to dental implants. Keep your options open.
