A patient presented with pain in tooth #47 . Radiographic examination revealed a periapical lesion on both mesial and distal roots, a separated instrument within the mesial canal system, and evidence of external root resorption.

Treatment involved retrieval of the broken instrument, removal of old gutta-percha from the canals , and thorough irrigation with sodium hypochlorite. Obturation was completed using a bioceramic sealer and the single cone technique.

At the 1-year follow-up, the patient was asymptomatic, and radiographs showed complete periapical healing.

Challenging Endodontic Case – 6-Month Success 🔹

A patient was referred with severe pain in the lower right first molar after a colleague faced difficulty removing old root canal filling and a separated instrument was left inside the ML canal and extends beyond the apex. Radiographs also showed severe external root resorption.

✅ The fractured instrument was successfully retrieved, canals were prepared to full working length, while the old gutta-percha in the mesial root was intentionally preserved to avoid further damage to surrounding bone.
✅ Calcium hydroxide dressing was placed for 2 weeks.
✅ Final obturation was done with MTA apical sealing , single cone and bioceramic sealer .

At the 6-month follow-up, radiographs revealed complete healing of the periapical lesion 🙌

A patient was referred to me by a colleague with a separated instrument in the DB canal of an upper first molar.
The initial treatment attempt showed a false pathway due to trying to bypass in the different direction, which unfortunately caused a perforation.

In this case, I carefully redirected to the correct canal path and confirmed the position with radiographs and apex locator. After several attempts, I was able to successfully bypass the instrument.
The canals were then obturated using 3D obturation, and the internal defect was sealed with MTA.
The case was completed in a single visit with final coronal restoration placed.

A patient presented with a fractured lower first molar that required full coverage restoration. Although the tooth was asymptomatic and showed no signs of periapical pathology radiographically, a non-surgical endodontic retreatment was performed prior to crown placement as a preventive measure.

Clinical Note:
It is highly recommended to perform root canal retreatment for any previously treated tooth that presents with structural compromise (e.g., fractures) and requires a crown, even in the absence of symptoms or radiographic signs of failure. This ensures optimal long-term success and minimizes the risk of future complications beneath the final restoration.
