gait analysis, etc. Hillel has been treating ingrown toenails (and also other general podiatric foot-care) for more than 20 years, having worked intensively for many years in the nail surgery clinic of the Israeli army, various kupat cholim clinics, including the kupat cholim clalit diabetic foot-clinic, and in private practice.

לגבי ציפורן חודרנית, זה לא רק מה שמוציאים שחשוב, אלא מה שנשאר בפנים שבאמת משפיע!!

הרבה פעמים אנשים מגיעים אלי שסובלים מציפורן חודרנית אחרי שהיו בטיפול במקום אחר, ועדיין סובלים מכאבים, למרות שהראו להם חתיכת ציפורן מאוד גדולה שיצא. בבדיקה ראשונית הרבה פעמים אין מה לראות שיכול להסביר את הכאבים הממושכים. אבל כשאני בודק יותר עמוק אפשר לראות שהמטפל חתך את החלק הבעייתי של הציפורן באמצע, והשאיר את החלק השני תקוע עמוק בתוך האצבע, או בצד של האצבע
עמוק בתוך העור, שכמובן עכשיו עושה את הטיפול הרבה יותר בעייתי. מה שהיה יכול להיות טיפול מהיר ואפקטיבי ופשוט יותר עכשיו הפך להיות יותר בעייתי ועמוק יותר ולפעמים כאובה יותר ממה שהיה צריך להיות.

אז בבקשה, ברגע שיש לכם ציפורן חודרנית, לא לחפור לבד, ולא ללכת למטפל לא מקצועי, ישר לפודיאטור מורשה.

IT’S NOT ONLY THAT WHICH WAS REMOVED THAT MATTERS, BUT MORE IMPORTANTLY, THAT WHICH IS LEFT INSIDE THAT REALLY COUNTS.

I often see patients who come to me following treatment for an ingrown toenail somewhere else complaining that they are still in pain, despite having been shown the large piece of nail that was removed. On initial examination there is often nothing to see that is causing the pain. However, on deeper inspection, I often see a section of nail deeper inside the toe that has either been split in half midway with the remaining section left inside even deeper, or the nail has been split in half lengthwise, with the remaining part hiding deep under the side of the toe. Either way, the pain is still there, despite a large section of the nail having been removed. This is because the person doing the treatment has not correctly removed the offending section of the nail.
In this case the treatment that needs to be done to correct the damage is much more invasive than had the patient come right away.
Often a treatment that would have been quick and relatively simple, due to bad treatment elsewhere, becomes more invasive and complicated.

GET IT RIGHT THE FIRST TIME!! GO DIRECTLY TO A LICENSED PODIATRIST.

הרבה אנשים, במיוחד נשים, חוששים לעשות ניתוח שורש לצמיתות עבור ציפורן חודרנית בגלל ששמעו שאחר-כך זה לא נראה יפה. מעדיפים לסבול מכאבים באופן כרוני במקום ללכת לפודיאטור ולעשות ניתוח שורש עם צריבה.

רואים בתמונות כאן אצבעות של אישה שהייתה אצלי לפני כחמש שנים עם בעייה של ציפורן חודרנית כרוני בבוהן ימין שלה , אספקט ימין בתמונה. עשיתי לא ניתוח שורש עם צריבה לצמיתות. התרפא ללא כאבים וגם נראת בסדר גמור. גם אחרי חמש שנים זה לא חזר לחדור פנימה.

אחרי כמה זמן האישה עברה לגור בעיר אחרת יותר רחוקה והתפתחה לה ציפורן חודרנית בבוהן ברגל השנייה. אבל הפעם היא הלכה לקופת חולים שלה בעיר החדשה שלה, ועשתה ניתוח שורש לא אצל פודיאטור. היא הופתעה מאוד לגלות שלא כל אחד עושה את הניתוח אותו דבר. היא סבלה מאוד אחרי הניתוח , ועכשיו מפריע לה מאד איך האצבע נראית, כפי שרואים בתמונה.

לומדים מכל זה שחשוב מאוד להבין שניתוח שורש לציפורן חודרנית זה דבר לא פשוט ולא כל אחד ואחד עושים את זה אותו דבר. כל אחד עושה את זה בדרך שלו. טיפול על השורש של ציפורן יכול לגרום לכאבים לכל החיים וגם לבעיות אחרות כמו איך זה נראית.
חשוב לבחור טוב מאוד איפה עושים את הניתוח הזה, אפילו אם צריכים לשלם קצת יותר על זה.

Very often I hear about people who are hesitant about permanent nail root surgery because they have heard from friends that the results are not aesthetically appealing. They would rather suffer from pain on a regular basis than having it removed permanently once and for all. These pictures are taken of a lady who came to me suffering from chronic ingrown toenails on her right big toe. I performed permanent nail root surgery on the problematic side ( right side of the nail in the picture), it healed nicely and looks aesthetically appealing. This picture was taken five years later, still no sign of recurrence of the ingrown toenail. Subsequently the lady moved away to another city further away and developed an ingrown toenail on the other foot. This time she didn’t go to a podiatrist but a non podiatrist on her health fund ( kupat cholim) performed the nail surgery. The patient suffered terribly post surgery with tremendous pain, and she is very upset with how it came out afterwards.

The lesson learned is that not everyone performs nail surgery the same. Especially when treating the root of the nail, it can affect the way it looks and feels forever. Permanently. Choose very carefully where you do nail surgery, even if you need to pay a little extra.

I saw a lady this week who had had her ingrown toenail treated many years ago by having the whole nail surgically removed,not by a podiatrist. Seen here is the result, forever.

Another reason to choose very carefully where to have your ingrown toenail treated. It’s not always necessary to have surgery on the nail, and when it is indeed called for, it’s very rare that the whole nail needs to be removed. A podiatrist will remove only the side that is problematic, together with the root. However, it’s very important to know how to kill the root and to do so in such a way so as to prevent regrowth of the ingrown toenail but at the same time preserving the aesthetic structure of the nail and toe.

היית אצלי השבוע אישה שעשתה ניתוח לציפורן חודרנית לפני הרבה שנים, לא אצל פודיאטור. עשו לה עקירה מליאה. זה התוצאה . לצמיתות.
עוד סיבה לחשוב טוב מאוד לפני שבוחרים אצל מי לעשות טיפול לציפורן חודרנית.

לא תמיד יש צורך לעשות ניתוח, וכשיש צורך בניתוח, נדיר מאוד שצריכים לעקור את כל הציפורן. אפשר לעקור את הצדדים בלבד, אבל חייבים לדעת איך להמית את השורש על ידי צריבה , באופן שימנע גדילה מחדש של הציפורן, אבל בלי נזק אסתטי לאצבע.

היום היה לי ביקורת של מטופל ( חייל מהשטח), שעשיתי עליו טיפול לפני כמה חודשים עבור יבלות ויראליות על כפות הרגליים. הוא סבל למשך יותר משנה/שנתיים מיבלות ויראליות לא רק על כפות הרגליים אלא גם בידיים, וניסה הרבה טיפולים עד שהוא הגיע אלי.

אבל הטיפול שעשיתי עליו, needling, עובד על מערכת החיסון לעזור לגוף לייצר נוגדנים נגד הוירוס בכל הגוף.
אז למרות שעשיתי עליו טיפול רק בכפות הרגליים, זה ריפא גם את היבלות הויראליות שהיו גם בידיים .

כך שהיום בביקורת, לא היה סימן של היבלות, לא רק בכפות הרגליים אלא גם בידיים.

I had a follow up on a patient ( a very grateful soldier) who came to me a few months ago suffering from the discomfort of several warts, having had several failed treatments. He had warts not only on his feet but also on his hands. In fact he had had many treatments over the past one to two years for the warts, especially the warts on his hands. Being a podiatrist I obviously only treated the warts on his feet. However, the treatment that I did, needling of the warts, is a treatment that induces the immune system to make antibodies against the virus that causes the warts to form, so in effect the result as seen today was that the warts had healed up, both on the feet and also the hands. Needless to say that the patient was very grateful.

Here is a before and after picture of a young person who suffered for years from ingrown toenails and had several unsuccessful surgeries and total nail removals, only for the situation to get even worse, as can be seen in the before picture.
The after picture shows a fully healed situation, as well as can be expected from the permanent damage caused by unprofessional previous treatment.
Lesson learned. When dealing with ingrown toenails go directly to a qualified,registered podiatrist who is experienced with ingrown toenails.

בחור שהגיע אלי אחרי שנים של סבל מציפורניים חודרניות והרבה ניתוחים ועקירות, ( שהנזק מהטיפולים רואים ), וכאן רואים תמונה של אחרי החלמה מהניתוח שאני עשיתי לו.
כאן רואים עוד סיבה ללכת ישר לפודיאטור ולהימנע מבעיות כאלו מטיפולים לא טובים.
