This statement is from Francisco Santos Silva, who adds, " fortunately, many of these situations, due to their low pathological relevance, are self-limiting, with regression often occurring almost spontaneously "
However, others, whether traumatic or mainly degenerative in nature, with a higher incidence in patients over 45/50 years of age, present as partial or total tears of varying degrees of extent in one or more rotator cuff structures.
Any of these clinical situations in general, in addition to the functional impairment they cause, also entails very high economic and social costs, which are important to pay close attention to in current times.
The natural history of rotator cuff tears is quite clear and defined, showing that small partial or complete tears, over time, develop into larger injuries, sometimes compromising the entire structure and consequently triggering worsening pain symptoms, as well as drastic loss of function of the entire upper limb and compromising the quality of work and social life.
Francisco Santos Silva also states that " evidence has consistently shown that early surgical repair of small injuries, whether partial or complete, is associated with a better healing process, lower rates of re-tearing, improved postoperative function, and signif**antly greater abduction strength capacity, when compared to the repair of large or massive tears "
Large tears, due to the incapacity they cause, often result in reverse-type arthroplastic surgery, which currently still has complication rates of over 50%, in addition to the exaggerated financial costs to society.
It's important to emphasize that not all patients with a shoulder cuff tear require surgical treatment, as other commitments or contingencies don't always indicate it.
However, the truth is that an early diagnosis and surgical repair, while the injury is small and the biological regeneration capacity is most effective, yield the most appropriate final results from a clinical and functional standpoint.
Francisco Santos Silva takes the opportunity to remind us that " early diagnosis is crucial and should be achieved exclusively with the help of MRI "
