هذا المريض عمره ٥٢ سنه ، وصل الى الطواريء حيث تم تشخيصه باحتشاء في عضلة القلب وتم تحويله فورا الى معمل القسطرة حيث اجري له قسطرة للشرايين وتبين وجود انسداد شديد في الشريان الجذعي الرءيسي الايسر وهبوط حاد في الضغط حيث تم وضع مضخه مساعده لعضلة القلب وتحويله الى غرفة العمليات مباشرة وتم اجراء عملية قلب مفتوح طارءة وزراعة الشرايين ، والحمدالله المريض حالته مستقرة بعد العمليه
Here is 52 years old male patient, presented to ER with chest pain and dynamic ECG changes. He was in cardiogenic shock , Cath done which revealed subtotal Acute occlusion of the distal left main artery,IABP and Inotropic support were initiated, then shifted to OR and Emegecy CABG done , the patient did well after surgery overnight, this morning we were able to remove the IABP out and he is doing great, thankfully to Almighty god

هذا المريض البالغ من العمر 70 عاما ، حضر الى الطواريء وكان يعاني من الدوخه وفقدان الوعي لمدة قصيرة ، وتبين لاحقا انه يعاني منذ مدة من الم في الصدر من وقت الى اخر ، بعد الفحوصات اللازمة تبين وجود تضيق شديد في الصمام الابهر وتوسع شديد في الشريان الابهر الصاعد ومهدد بالانفجار والوفاة الحتمية ان حصل هذا لا سمح الله، تم اجراء عملية كبرى لتبديل الصمام الابهر وتبديل الشريان الصاعد ونقل شرايين القلب الرئيسية الجذعية الى الشريان الاصطناعي الجديد والحمدالله حالته ممتازه وسيغادر المستشفى غدا بعد ستة ايام من اجراء العملية
This is 70 years old male patient, presented via ER with complaints of dizziness, syncope for short time and history of chest pain since long time. After thorough investigations found that he has severe Aortic Valve stenosis with PG more than 90 mmgh and dilated Ascending Aorta =5cm impending rupture,done for him BENTAL procedure and for discharge tomorrow on day 6 post operation

هذا المريض البالغ من العمر ٦٥سنه تم له اجراء قسطرة قلبيه لزراعة جهاز في الوريد الأكبر في القلب لتحسين الترويه لعضلة القلب وهذا الجهاز حديث لعلاج الحالات التي لا يمكن عمل لها عمليه زراعة شرايين او شبكات وهو حل حديث ونتائجه جيده بتخفيض الآلام ، ولكن بعد نهاية زراعة الجهاز تبين لأطباء القلب التداخلي ان هناك تسرب للدم من الوريد الأكبر الى التجويف حول القلب وكان فريق الجراحه قريب من الحدث وتم استشارتنا فورا وقررنا اجراء عمليه عاجله لان ألقلب بداء يختنق والضغط يهبط باستمرار ، لذلك وأثناء تحضير العمليات أدخلنا انبوبه الى التجويف حول القلب لنسحب الدم ونرجعه الى القلب عن طريق أنبوب موجود أصلا في وريد الرقبه وواصل الى القلب وبهذا استطعنا ان نصل به الى غرفة العمليات على قيد الحياة وهو بضغط منخفض جدا ولكن تم اجراء العمليه بسرعه فاءقه وتم باذن الله وقف النزيف والسيطره عليه وإصلاح الاصابه واليوم هو اليوم الخامس بعد العمليه وخرج الى بيته والحمدالله

This is 65 years old male patient, presented for coronary sinus reduced device insertion, this device newly invented to block forward flow of blood in the coronary sinus and to augment the back flow to the sub endocardium and this found to be reducing the Angina pain in patients who are not fit for neither surgical nor PCI ,Intervention
But after inserting the device the interventional cardiologist has noticed that a blood rapidly collecting in the pericardium, due to coronary sinus injury, we were consulted and immediately decided for surgical intervention, but take him a life while preparing the OR we have inserted a pig tail catheter in the pericardium and started to suck blood which was already heparinized and transfused it via internal jugular line which was in place to the heart , for two purposes, to prevent tamponade and to give his own blood as it was no time to prepare blood , within 10 minutes he was on OR table and two minutes to go in , the the bleeding site identified the tamponade relieved and the coronary sinus been repaired, patient discharged home on day 5 post op

59 years old male patient, presented complaining of dizziness and chest pain. Echocardiography revealed sever bicuspid Aortic valve stenosis, max gradient 90 and mean 65 , coronary angiogram showed tight proximal LAD stenosis, Echo and CT Angiogram showed ascending aorta aneurysmal dilatation , we did for him ascending aorta replacement , Aortic Valve replacement using size 25 mm mechanical valved conduit and re implantation of left and right coronary buttons , then we did Lima to LAD anastomoses beating heart on pump, the patient did extremely well post operation.
هذا المريض عمره ٥٩ سنه كان يشكو من الام في الصدر ودوخه، تم اجراء الفحوصات اللازمه وتبين وجود انسداد في الصمام الأبهر وتوسع في الشريان الأبهر الصاعد وايضاً انسداد في الشريان التاجي الأمامي ، مما استدعى اجراء عمليه كبرى لتبديل الصمام الأبهر والشريان الأبهر وإعادة الشرايين التاجيه الرءيسيه على الابهر الجديد وايضا زراعة شريان تاجي جديد حيث استغرقت العمليه ١٢ والحمدالله وبفضل من رب العالمين تكللت بنجاح كبير

هذه السيدة الفاضلة تم تحويلها الى عيادة جراحة القلب ن عيادة أمراض القلب وتعاني من تضيق في الصمامين الأبهر والمايترالي وتوسع في الصمت ثلاثي الشرفات ، على اثر ذلك تعرضت الى جلطة دماغية وتم تبديل الصمت المايترالي بصمام معدني واواصلاح الصمام ثلاثي الشرفات ، اما الصمام الأبهر فكان صغيرا لذلك تم تكبير الشريان الأبهر الصاعد بإضافة رقعة من نسيج الكتان الخاص وتوسيع جذر الأبهر وبعدها تم زراعة الصمام المعدني وإغلاق الشريان الأبهر ، افاقت من التخدير بنفس اليوم وبعد سبعة ايام غادرت المستشفى برعاية الله هذا اليوم السبت بصحة ممتازة والحمدالله.

This 39 years old lady referred to my clinic with a diagnosis of severe mitral valve stenosis,severe Aortic Valve stenosis, and moderate Tricuspid valve regurgitation. With history of old brain infarction. I did for her mitral valve replacement with mechanical valve, then discovered the Aortic valve with a small annulus so annular enlargement performed using double weaver heamosheiled patch and top a mechanical valve size 21 mm was implanted then completion of ascending aorta angioplasty done and finally the tricuspid repair done on beating heart, after 7 days the patient discharged home (today),with great god tremendous help.

هذه سيده عمرها ٥٨ عام، اجريت لها عمليه سابقه في مركز اخر قبل ثلاث سنوات ، راجعت عيادة القلب تشكي من دوخة وضيق في النفس وإعياء عام ، وبعد الفحوصات السريرية والشعاعيه تبين وجودتوسع شديد في الشريان الابهرالصاعد وصل الى ٧ سم ومهدد بالانفجار حيث يوجد ضعف شديد بجدار الشريان الأبهر ، اجريت لها عملية اعادة فتح الصدر وازالة الشريان الأبهر الصاعد كاملا وازالة الصمام القديم وتبديل الشرايين الرئيسية للقلب المرتبطه بالأبهر ، اسم العمليه بنتال ومعظم مراحل العمليه تمت بتبريد كلي للجسم الى درجة حرارة ١٨ الى ٢٠ درجة مءويه مما أتاح توقيف القلف والرئتين وتوقيف مضخة الترويه لمدة أربعين دقيقة وبعدها تمت عملية الإحماء واستمرت العمليه كاملة حوالي ١٠ ساعات . الحمدالله النتيجه ممتازه تم فصلها عن الأجهزه في اليومالتالي وبعد عشرة ايام غادرت المشفى بصحة جيده وامس راجعت العياده احسن بكثير بفضل من الله ومنه
58 years old female patient, had Aortic valve replacement 3 years ago, in a different place , presented to cardiology clinic with complain of dizziness ,shortness of breath , and general fatigue, diagnosed as severely dilated ascending Aortic Aneurysm, 7 CM in diameter with impending rupture, we did for her Redo operation and removed the old mechanical valve , re implanted left and right main coronary arteries and replaced the ascending Aorta, (Bental ), under total circulatory arrest. Patient did very well , discharged home 10 days later in very good condition
