عاوزين نشوف Terminology of tooth preparation عندي مصطلحات لازم اعرفها زي:-
🔴ال simple tooth prep
دا لم ال prepration يضمن surface واحد..مثلا ال occlusal surface وبختصرو بس "O"
🔴ال compound tooth prep
دا لم ال preparation يضمن لي 2 surface..مثلا ال mesial and occlusal surface وبختصرو بي "MO"
🔴ال complex tooth prep
دا لم ال preparation يكون في 3 surface او اكتر..مثلاال mesial, occlusal, distal وبختصرو بي "MOD"
📌نشوف ال Tooth preparation walls :
عندي internal wall و extarnal wall
✅ال external wall:-
دا ال surface البيمتد لي ناحية ال external tooth surface 💃
وبياخد اسم ال tooth surface زاتو..مثلا buccal او lingual او mesial او distal
✅ال internal wall :-
دا ال surface مابيمتد لي ناحية ال external tooth surface
عندي مثلا زي ال
*ال pulpal wall :
دا بيكون perpendicular علي ال long axis of tooth وبيكون occlusal to pulp 💃
*ال axial wall:
دا بيكون parallal لل long axis of the tooth 💃
*ال enamel wall:
دا جزء من ال external wall بيكون consisting of enamel 💃
* ال dentinal wall :
دا جزء من ال external wall بيكون consisting of dentine
🔴ال floor او ال seat :
دا بيكون horizontal و perpendicular علي ال occlusal surface
مثلا زي ال pulp floor وال gingival floor 🔴ال line angle :
عبارة عن junction of tow surface
✅ ال internal line angle :
دا هو ال line angle بيكون عندو apex points علي ال tooth
✅ال external line angle :
دا هو ال line angle بيكون عندو apex points بعيد من ال tooth
🔴ال point angle :
دي عبارة عن junction of three surface 💃
🔴ال cavosurface angle :
عبارة عن junction of a prepared wall and external surface of the tooth 💙
وهو بختلف حسب
ال location of tooth.. ونوع ال restorative material واتجاه ال enamel rods in the perpared wall 💃 📌نشوف ال classification of tooth preparation 💙
اساسا التقسيم دا عن طريق ال G.V black علي حسب ال diseased anatomic area لي 6 classes 💃💃
✅class 1:-
دا بيشمل كل ال pit and fissure preparation.. فقسمناهو لي ٣ قروبات :-
١.ال preparation on occlusal surface of premolars and molars
2.ال preparation on occlusal Two thireds of the facial and lingual surfaces of molars
3.ال preparation on plaatal surface of maxillary incisors
✅Class2:
دا هو ال preparation on the proximal surface of posterior teeth
✅class3 :
دا هو ال preparation on proximal surface of anterior teeth بس مابتشمل ال incisal angle
✅class4:
دا هو ال preparation on proximal surface of anterior teeth و تشمل ال incisal angle
✅class5:
دا هو ال prepartion on the gingival or cervical thired of the facialor lingual surfaces of

في البوست الفات كان وقفنا في ال factors of tooth preparation في البوست دا نتكلم عنهم واحد واحد :
✅ال general factors:-
ودي بحد زاتها فيها ٣ حاجات:
🔴ال diagnosis :-
لازم قبل نعمل اي restorative procedure اعمل examination..اشوف ماعدهو السبب الخلاني اعمل restoration..هل هو caries او fracture او esthetic او function ولا شنو بالظبط 💙💙✅
بعد كدا اشوف حالة ال periodental وال pulp وهكذا يعني الانسجة المحيطةةلي بال tooth 💙
بعدها اشوف ال occlusion.. وال esthetic.. وهل ال patient دا patient caries risk?!
واشوف هل ال tooth ديزاتهاا داخلة في خطة علاجية تانية مثلا هي هي abutment ولا لا وهكذا 💙
🔴ال knowledge of dental anatomy :
عشان افهم ال tooth preoaration لازم اكون عارف ال anatomy of tooth 💃💃يعني لازم اعرف ال size وال position بالنسبة لل pulp وطبعا عارفين انو حجم ال pulp بنقص مع العمر 💃ولازم نعرف ال thickness بالنسبة لل enamel وال dentin 💃ونعرف علاقة ال tooth زاااتهاوال supporting tissues 💃
🔴ال patient factors
طبعا دا مهم شديد لانو بحدد لي انا مفرض اعمل ياتو restorative treatment.. عن طريق اني اعرف عمر ال patient واعرف ال dental health بالنسبة للمريض دا واعرفةالحالة الاقتصادية ليو "economic status " 💃
✅ال conservation of tooth strucure :-
بقدر الامكان احاول اني اخلي الو restoration يكون small لي ؟!
لانو كل ماكان ال remove لل tooth structure اقل بالتالي حااقلل امكانية انو يحصل damage لل pulp💙💙
وانو كل م كان ال prepartion بالنسبة للسن smaller الحاجة دي بتسهل اني ارجع ال restorative material..وحيكون stronger فبالتالي حيقاوم ال fracture 💙

✅ال restorative material factors :
انا بختار ال restorative material علي حسب ال economic وال esthetic بالنسبة لل patient..وقابلية اني اقدر اعمل "isolate" عزل للمنطقةةالجراحية.. وعلي حسب امتداد ال lesion او ال defect 💙

في البوست دا عاوزين نشوف ال fundamental of tooth preparation بمعني اساسيات او ال gied line البعمل بيها تجهيز لل tooth عشان اخت restorative material 💙💙
نحن زمان كنا بنقول cavity preparation وكان بنقصد بيها علاج ال caries او ال cutting او ال preparation لاجزاء ال tooth بحيث انو تستقبل لي restorative material ✅
اما حاليا فبنقول tooth prepartion لانو لقينا انو العلاج مابيكون فقط لل caries انما في اسباب تانية اقرب مثال ال truma 💔
📌من هنا بقدر اعرف ال tooth prepartion واقول انو هو ال

تغيرات مكانيكية لمشاكل ال tooth عموما عشان تستقبل restorative material يرجع ليها شكلها ووظيفتها وجمالها ❤
📌طيب انا عشان اعمل restorative محتاج لي شنو؟!
✅انو يكون في fractured teeth
✅او يكون في dental caries lesion
✅او يكون في restoration محتاجة اعمل ليها repair
✅او يكون في esthetic اراعيها زي ال fluorosis
✅او يكون في congential malformation تشويه خلقي عاوز اصلحو 📌طيب الهدف من ال Tooth preparation شنو ؟!
الهدف اني اعمل remove لكل ال defects واني ازود ال pulp بال protection الضروري 💙
وانو لم اعمل restoration لازم اراعي اني مااشيل اكتر من الاجزاءالمطلوبة عشان احافظ علي ال tooth 💙
وانو ال preparation دا يسمح لي اني اخت restorative material ترجع لي ال esthetic وال function 💙
وانو لم اعمل preparation انو م يحصل fracture لل tooth ولا لل restoration بسبب ال masticatory force 💙

مواصلين في كلامنا عن ال perevention of dental caries ❤
ونتكلم عن ال Fluoride exposure :-
ال Fluoride مهم جدا في حكاية ال caries prevention 💙لانو بيشتغل anti-caries effect عن طريق3 mechanisms مختلفة هم
📍انو بيحسن ال remineralization في حالة ال incipient caries ✅
📍انو بيحسن ال precipitation بالنسبة لل fluoroapetite بالتالي حيكون اكتر مقاومة لل demineralization 💙💙
📍انو بيمتلك antimicrobial activity
اذا كان ال fluoride low concentrations حيمنع البكتريا انها تتشكل في ال extracellular وال intracellular polysaccharides 💙💙 فبالتالي حيقلل لي او بيعمل reduces لل bacterial adhesion وبحد لي او يقلل من ال microbial metabolism بين ال host's meals 💙
اما اذا كان ال fluoride high concentration
ف ال Fluoride ion مباشرة حيسمم لي بعض من ال oral microorganisms ودي بضم لي ال MS البتسبب لينا ال caries ✅✅

📌ال Fluoride sources :-
ممكن نتحصل علي الفلوريد من ال diet ✅
وال Tooth pastes " معجون الاسنان"
ودا بيساعد لي ال tooth انها تكون cleaning and polishing 💙
عندي اربعة مصادر للفلوريد ممكن استخدمها بشكل يومي وروتيني هم
*ال sodium fluoride بختصرو بي NaF...
*ال Amine fluoride بختصرو بي AmF...
*ال sodium monofluorophosphate بختصرو بي MFP...
*ال stannous fluoride بختصرو بي SnF2...
🔵معظم معجون الاسنان بيحتوي علي sodium monofluorophosphateو او sodium fluoride بتركيز من 1000 لي 1450 ppm 💙
في ال Low dose بالنسبة للاطفال الاكبر من 7 سنين بتحتوي علي كمية اكبر من 500 ppm..عشان يقلل خطر ال Fluorosis ❤❤

3.ال systemic fluoride او ال fluoridated community water system
دي سبب موثر لانو بزودنا بي systemic و topical effect
ال water Fluoridation التركيز بتاعها لم يكون 1ppm دي بتعمل caries reduction بنسبة 50%💙
4.ال Mouth rinses او ال rinsing solution :
التركيز بتاعها بعتمد علي الاستخدام...بمعني لو كان اسبوعيا التركيز حيكون 0.2%.. ولو كان يوميا التركيز حيكون 0.05
اكتر solution مستخدم هو ال sodium fluoride ❤

5.ال professional topical applications:
ودا عن طريق ال dentists وبيكون ي
ي اما في شكل varnishes زي الطلاء او المناكير 💅..او يكون في شكل gels.. والكلام دا بكون من ٣-٦ شهور حسب ال patient caries risk 💙💙

انواع التخدير ......بطريقة مبسطة.

ماذا تعرف عن التخدير !!؟
التخدير هو عبارة عن فقدان جزئي او كلي للاحساس سواء كان مصحوبا بفقدان الوعي ام لا .

💧ماهي انواع التخدير ؟

✅التخدير العام ( general anesthesia ):
فيه يتم دفع الدواء المخدر عن طريق الوريد .

يسبب التخدير العام فقدان كامل للوعي حيث ان المريض لا يشعر بالالم ولا يتذكر الاحداث والتفاصيل التي تحدث خلال العملية .

✅ التخدير النصفي (spinal anesthesia ) :
يتم حقن المادة المخدرة مثل الليدوكايين في سائل النخاع الشوكي
لا يفقد المريض وعيه في هذا النوع ، ولكن يفقد القدرة على الاحساس والحركة في نصف جسمه السفلي .

✅ التخدير الموضعي ( local anesthesia ) :
تتم عن طريق دفع الدواء المخدر في الجلد ويؤثر على منطقة محددة من الجسم ولا يفقد المريض وعية في هذا النوع ..💙
