Fatphobia in society needs to be challenged and changed. It creates stigma and prevents people from feeling accepted and valued. Early medical help should be encouraged for everyone, regardless of body size. Health care must be accessible and free from bias. Promoting respect and support leads to better health and well-being for all.

Definition and Causes:
πŸ”΄ Obesity is a chronic, progressive disease involving excess body fat.
πŸ”΄ It raises the risk of serious health problems across multiple systems.
πŸ”΄ Causes include:
πŸ”΄ Genetics, environment, medications, medical conditions, and lifestyle
πŸ”΄ Key contributors: poor diet, low activity, stress, and hormonal factors

Limitations of BMI:
🟠 BMI alone doesn’t measure fat distribution or metabolic risk.
🟠 Use BMI+ for better risk assessment, including:
🟠 Waist circumference
🟠 Waist-to-hip ratio (β‰₯0.85 women, β‰₯0.90 men)
🟠 Waist-to-height ratio (>0.5)
🟠 These help detect central obesity and predict cardiometabolic risk

Complications of Obesity:
🟑 Increases risk of:
🟑 Type 2 diabetes – due to insulin resistance
🟑 Obstructive sleep apnea – fat narrows airway
🟑 Cardiovascular disease – heart attack, stroke, hypertension
🟑 MASLD – liver fat accumulation, inflammation, fibrosis
🟑 Osteoarthritis – joint stress from excess weight
🟑 Also linked to infertility, GERD, cancer, and depression

Approach to Obesity Care:
🟒 Use respectful, person-centered care free from weight stigma
🟒 Follow the 5 A’s framework:
🟒 Ask – get permission to discuss weight
🟒 Assess – BMI+, complications, and behaviors
🟒 Advise – explain risks and benefits clearly
🟒 Agree – set shared, realistic goals
🟒 Assist – provide tools, referrals, and follow-up

Treatment Options by BMI Category:
πŸ”΅ 25–26.9 kg/mΒ²: Lifestyle therapy
πŸ”΅ 27–29.9 kg/mΒ²: Add meds if complications exist
πŸ”΅ 30–34.9 kg/mΒ²: Lifestyle + pharmacotherapy
πŸ”΅ 35–39.9 kg/mΒ²: Add surgery if complications
πŸ”΅ β‰₯40 kg/mΒ²: Lifestyle, pharmacotherapy, and surgery recommended.

Obesity is not just a global problem, but one that is hitting Singapore hard.

According to the latest Ministry of Health data from 2022, nearly 12 out of every 100 adults aged 18 to 74 in Singapore were obese - double the rate from 30 years ago.

However, obesity is not a flaw or a simple lifestyle choice, but a complex, chronic disease that is often misunderstood and unfairly judged.

Obesity is associated with more than 230 medical conditions, including heart disease, stroke, Type 2 diabetes and fatty liver, according to doctors.

Metabolic and bariatric surgery, including Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG), is highly effective in achieving T2D remission.

The DRI (Diabetes Remission Index) and W-DRI (Weight Loss-Adjusted Diabetes Remission Index) models accurately predict T2D remission post-MBS, enabling personalized patient care and informed decision-making.

World Diabetes Day "Breaking Barriers, Bridging Gaps”.

It serves as a reminder to all of us of the need to collectively work to overcome the growing burden of diabetes globally and in WHO South-East Asia Region.

The theme emphasizes the importance of tackling the challenges faced by individuals, communities, and health systems to access diabetes prevention and management.

Latest estimates for 2022 indicates that diabetes affects nearly 246 million people 30 years and above in the WHO South-East Asia Region.

The outcomes of untreated and uncontrolled diabetes range from heart attacks, strokes, kidney failure, blindness, and amputations.

These complications not only impose an emotional and financial burden on individuals and families but also significant financial strain on healthcare systems.
