What if the human hand carries an ancient blueprint? ๐ŸŠ๐ŸฆŽ๐Ÿ–๏ธโœจ

Some peeks into my journey on reptilian dissection to better understand children born with rare congenital differences of the upper limb. ๐Ÿ’›

If reptilian forelimbs preserve older muscle patterns, then studying them may offer clues to the evolutionary and developmental origins of these human hand anomalies. Sometimes, to understand what happens before birth, we have to journey far back through the story of life itself. ๐Ÿงฌ๐ŸŒฟ

Back through development.
Back through anatomy.
Back through evolution. โœจ
Because the hand may hold more than function.
It may hold history.
It may hold memory.
And within that memory may lie answers, healing, and hope. ๐Ÿ–๏ธ๐Ÿ’ซ

This is what makes anatomy so extraordinary:
In every structure, there is a story.

Happy to share our article in Orthopaedic Surgery describing a novel two-stage temporary ectopic implantation strategy for complex hand reconstruction after devastating bilateral upper-limb trauma โ€” a reconstructive pathway designed for the rare circumstance in which conventional immediate replantation is not feasible, yet meaningful recovery remains worth pursuing.

The significance of this work lies not only in technical ex*****on, but in outcome: restoration of protective sensibility, grasp, pinch, and eventual return to occupation. We hope this technique adds meaningfully to the armamentarium of original microsurgical reconstruction methods for carefully selected cases.

What lingers most is not only the technical originality of the operation, but the philosophy beneath it: when the usual options fall away, thoughtful reconstruction can still offer dignity, function, and hope.

Beyond anatomy and technique, this is a story about refusing to let catastrophe have the final word.

The SGH Hand family is delighted to share the publication of our study on trapeziometacarpal joint arthroplasty with the Touchยฎ prosthesis in the Asian population ๐Ÿ’™โœจ

This is a particularly meaningful milestone for our department, as Singapore was the first country in Asia to offer the Touchยฎ prosthesis, and this work from our centre represents the largest case series in Asia to date.

This work highlights something we care deeply about: Asian anatomy matters ๐Ÿ–๏ธ In our series, the mean trapezium dimensions were 14.9 mm in width, 12.1 mm in depth, and 8.8 mm in height, substantially smaller than values reported in Caucasian populations. In practical terms, this meant that almost all cases required the smallest available implants, and it reinforces how anatomy directly affects implant selection, surgical technique, and patient suitability.

These differences are not just academic โ€” they change how surgery is planned and performed. A smaller trapezium leaves less room for safe cup positioning, makes bone preservation even more important, and increases the need for meticulous technique. Our experience also showed that plain radiographs may not always tell the full story: in one patient, the trapezium appeared suitable on X-ray, but intra-operatively the narrowest articular surface measured only 7 mm, making cup placement unsafe. This is why careful selection and more precise preoperative assessment are so important ๐Ÿ™

Looking ahead, this is exactly where our future work will be centred ๐Ÿ“š We will continue to focus on these anatomical differences through prospective data collection, improved preoperative assessment, and the hope for implant designs better suited to Asian patients ๐ŸŒโœจ

So happy to co-author this paper with on Proximal Row Carpectomy: Open Versus Arthroscopy ๐Ÿซถ๐Ÿป๐Ÿ“

This paper frames PRC as a motion-preserving, time-tested wrist salvage procedure for a wide range of degenerative and traumatic conditions, while also exploring how arthroscopic PRC is emerging as a promising alternative to open surgery.

One of the biggest takeaways for me: Just because a technique is newer and less invasive doesnโ€™t automatically make it better.

Arthroscopic PRC offers real theoretical benefits โ€” smaller scars, less soft-tissue damage, faster recovery, and direct visualization of joint surfaces โ€” but the review also emphasizes that open PRC still has the stronger long-term evidence base, while arthroscopic PRC remains limited by a lack of high-quality long-term studies.

What makes this even more exciting is that the conversation is evolving. The paper also highlights how modifications like capitate resurfacing and soft tissue interposition grafts may expand PRC indications, even in selected patients with early capitolunate arthritis.

Grateful for the chance to contribute to a piece that looks not just at how we operate, but how we think about surgery.

Small incisions are exciting.
Good evidence still matters.

Celebrating a meaningful collaboration across borders ๐Ÿคโœจ

We are proud to share our published article in Seminars of Plastic Surgery on โ€œUpper Extremities Peripheral Nerve Injuries and Reconstructionโ€ where all the authors far and near are part of the International Microsurgery Club family! This paper highlights real clinical cases of iatrogenic upper extremity peripheral nerve injuries and emphasizes the importance of early recognition, timely intervention, and dedicated rehabilitation for better patient outcomes.

Later this year, Iโ€™ll also be starting my peripheral nerve traveling fellowship with the Chang Gung/Wan Fang team, and I honestly couldnโ€™t be more excited. It feels incredibly special that this publication is part of the beginning of that journey, and Iโ€™m so thankful for the chance to keep learning and growing with people I respect so much.

After three years of research, reflection, and relentless work โ€” itโ€™s finally here. โœจ
Today, Iโ€™m deeply grateful to share that our paper, โ€œCongenital Muscular Pseudohypertrophy of the Upper Limb: Morphology, Anatomy and Surgical Guidelines of a Unique Entity,โ€ has been published in Plastic and Reconstructive Surgery (PRS) โ€” one of the highest-impact journals in our field.
This is more than a publication. It is the culmination of years of quiet perseverance, growth, and belief in a process that often felt slow, uncertain, and unseen.
Our study represents the largest reported series of this newly documented congenital abnormality and proposes a surgical framework for care โ€” an effort to better understand a rare condition that shapes form, function, and futures.
Behind these pages lies nearly two decades of collective clinical experience, countless hours spent revisiting operative notes, patient data, and anatomical dissections, and a shared commitment to bringing clarity to something rarely described in the literature.
For me personally, this was the most difficult and life-changing paper I wrote during fellowship.
What began as curiosity became years of analysis, collaboration, and persistence across centres in Singapore, Edinburgh, and Beijing. Iโ€™m profoundly thankful to my co-authors, mentors, and most of all, the patients whose journeys made this work possible.
To every young surgeon and researcher: trust the slow burn.
The long nights, the setbacks, the revisions, the waiting โ€” they are not detours. They are part of the path.
And sometimes, the most meaningful work takes the longest to bloom.
