My patients with sciatica feel worst in the morning or when sitting, standing, coughing or sneezing. 🤧 It’s a debilitating condition and a literal pain in the butt! 🍑

Sciatica refers to pain that radiates along the path of the sciatic nerve, which branches from your lower back through your hips, buttocks and leg.

🔍 Finding out the true cause of sciatica can be tricky. There are five structures that can irritate the sciatic nerve. The structures are

1) a disk herniation

2) a tight muscle

3) an irritated joint

4) a narrowing of the space where the nerve roots exit the spin

5) a space-occupying lesion (any kind of growth, such as a tumor) impinging on the sciatic nerve

A MRI scan is often used to identify the cause, but should really only be used as a guiding tool. 🤔

A detailed clinical assessment to identify structures that reproduce symptoms will be more accurate and useful in determining the cause. 💯

These symptoms are actually very common, but not normal. 🙅🏻‍♀️ It can occur at any time in life for a woman, but commonly presents during pregnancy, post natal and menopause.

🏃🏻‍♀️Leaking urine or faeces when exercising, coughing or sneezing

😖 The need to go to the bathroom very suddenly or the desire to go to the bathroom is too strong

👩‍❤️‍👨 Pelvic or va**nal pain during sexual in*******se

🌚 Difficulty controlling the release of urine, faeces (poo) or flatus (wind)

🥦 Having constipation; difficult to poo and don’t go very regularly

🏋🏻‍♀️Feeling a bulging and heaviness in the va**na and or feel something coming down when lifting

✨ Don’t be embarrassed to speak to someone about it!

The pelvic floor muscles (PFM) can be weak, overstretched, slow to work, too tight or torn just like other muscles of your body.

🤰🏻Pregnancy & childbirth can cause problems for the PFM especially if you have had an assisted va**nal birth, an episiotomy or a very large baby.

🥴 Chronic constipation causes you to strain to empty your bowels on a regular basis, which leads to over-stretching and weakness of the PFM. Difficulties with emptying may be due to poor relaxation of the PFM.

🏋️‍♀️ Heavy or repeated lifting increases abdominal pressure which may put your PFM under strain

👿 Being very overweight may increase the pressure on the PFM

😫 Menopause results in a decline in circulating oestrogen, putting postmenopausal women at risk of PF dysfunction

✨Always try to avoid unnecessary strain over your PFMs. If you have to lift in your job or daily routine, get advice about safe lifting or use equipments to help.

The biggest misconception about pelvic floor muscles is that everyone’s pelvic floor needs to be tighter and that if you have a pelvic floor issue, you should strengthen them with pelvic floor exercises known as KEGELS. 😱

This isn’t necessarily the case, because for some women who have a really, really tight pelvic floor, the tightness of the pelvic floor can cause urinary issues (such as urge to urinate or painful urination), constipation or tailbone pain. ☹️

Doing more KEGELS exercises will only aggravate the situation. ❌

It’s important to see a professional to ascertain if you have a tight or a weak pelvic floor before starting on any KEGELS exercise.

Each woman’s pelvic floor is unique. Unless there is pain, women are often not educated about their pelvic floor, what it is or what it does. ☹️

The pelvic floor is a 'sling' of muscles, a bit like a small muscle hammock that runs between the p***c bone in the front, and the tailbone at the back. A woman's pelvic floor muscles support her womb (uterus), bladder, and bowel (colon).

There are two types of pelvic floors:
🔒Hypertonic
🔓Hypotonic

If the pelvic floor is hypertonic, it means the pelvic floor muscles are too tight, this can lead to physical discomfort including sexual dysfunction. ⚡️

If it is hypotonic the muscles are too loose, which often occur post childbirth, specifically va**nal birth. 🤰🏻

Every women deserve to know the implications for these different types of pelvic floors and how they can impact them. 💡

If you are new to tennis 🎾 or any racquet sports 🏸,

Get a qualified coach to recommend you
1. The grip size of your racquet
2. The weight of your racquet

Incorrect equipment can greatly increase the risk of having a tennis elbow, and this can definitely be prevented easily 👌🏼

Pro tip from Darren, Head coach & founder of Play! tennis

🎾 When in doubt, always choose a smaller grip size (you can always make it larger with an overgrip)

🎾 Generally, females can use a racquet that weighs less than 300g, while males can go up to 320g

Many people don't seek treatment when they first experience pain because
1. they hope the pain would go away on its own 🙏🏻
2. they are too busy 👩🏻‍💻
3. they've become used to their pain 🙀
4. they manage their symptoms with medications💊
5. they don't know where to seek help from 🆘

Unfortunately, people who wait too long to get treatment can find themselves with more serious concerns down the line and limited treatment options. If you are experiencing pain for more than 3 months, it's best to seek help or ask for advice!! It could be your nearest doctor, physiotherapist, personal trainer or even a friend. Just ask, let someone know, you're not alone. 🤗

We don’t stretch what we need to stretch, we stretch what we can stretch. 💡

Some muscles are just biomechanically awkward to stretch. Although these muscles can be elongated, they can’t be elongated enough to create the satisfying sensation of good stretch ‼️

With only a few major muscle groups that are stretchable, stretching as a treatment is a major limitation. ☹️

The hamstrings, hip flexors, quadriceps, calves, pectorals are just a handful of muscles that are indeed pleasant to stretch and may have therapeutic value. However, there are still many important muscles left out…

Stretching is a pleasant ritual for many people, myself included. It’s simple and it feels good. 👍🏻 When asked to explain a stretching habit, many people struggle. 🤔 They have a hard time articulating clear and specific goals.

Everyone just “knows” that it’s a good thing, and they haven’t really thought about why. 🤷🏻‍♀️

A trigger point or a muscle knot is a sensitive area in the muscle or connective tissue (fascia) that becomes painful when pressed. Pressing on a trigger point can cause referred pain and can help identify the source of pain. 🎯

When you have 'knots' in your muscles - stretching is not an effective strategy to regain flexibility in your muscles - Watch the video to find out why!!! 👀

What's your definition of being physically strong or fit? 🤔

The old me would think that I'm strong if I could deadlift a 100kg. I'm fit if I looked ripped..

But now, being physically strong to me means that even after a day of work and exercise, you can wake up feeling good about your body. You shouldn't feel like your body has been beaten up. You shouldn't feel like you need more sleep.🧟

Your body is not a machine. It's not meant to do the SAME thing everyday. If you're sitting down for long hours daily, incorporate regular walks & movement in your routine. 🏃 If you're exercising daily, include rest days for your muscles to recover. 💆🏻

Balance is the key. Doing too much of something good will in fact become a bad thing. 👿
