Stress leakage may be driven by coordination issues, pelvic floor muscle tone changes, hormonal changes pre-period and even in perimenopause and menopause, load management issues, breathing mechanics, or a combination.

Each needs a different approach. A pelvic floor assessment identifies which ones are
relevant for you.

Pelvic floor stimulation devices work. For certain women, with a specific type of stress leakage caused by urethral sphincter deficiency, they are a solid option. But leaking has more than one cause.

A device that induces muscle contractions cannot assess which one is driving yours.

🤨A pelvic floor with increased muscle tone may cause leaking.

🤨 An overactive bladder may cause leaking.

🤨 Prolapse may cause leaking.

🤨 Altered lumbopelvic control may cause leaking.

🤨 Ribcage stiffness may cause leaking.

None of these respond to the same treatment.

If you have done a course of treatment and you are still leaking, you are not a treatment failure. You are someone who needed an assessment first.

If period pain is something you manage every month, a TENS machine is worth knowing about.

TENS sends small electrical pulses through the skin that trigger your body’s natural pain relief response.

💫The evidence for high-frequency TENS in period pain is strong. It may reduce how much medication you need and extend the period of relief.

However, the settings matter; use it for at least 20-30 minutes at a time. More than once a day if needed.

The wellness internet is not wrong.

Fibre, protein, lifting, cardio, sleep, social connection, most of it is genuinely good advice. And most of it takes time. Lifestyle changes compound.

The women who feel the difference are the ones who kept showing up. But keeping showing up is hard when your body keeps getting in the way.

When running means leaking.
When lifting means pain.
When leaving the house means planning your route around toilets.

That is not a motivation problem. That is a signal worth paying attention to.

One in 3 women leak when they sneeze, cough, or exercise. Most have adapted around it.

Extra pads, avoiding the trampoline at the kids’ party, skipping the run.

Stress urinary leakage is not inevitable. Surgery is rarely the first step. Pelvic floor physiotherapy is the evidence-based starting point.

The 6-week OBGYNE check is doing exactly what it’s designed to do.

Depression screening. Healing stitches. Birth control. Cervix closed. Pap smear if needed. Done.

It is not designed to assess your pelvic floor, screen for prolapse, check your abdominal separation, look at your C-section scar mobility, or tell you whether your body is ready to run, lift, or have s*x again.

That’s not a criticism of the appointment. That’s just what it is.

A women’s health physiotherapy assessment at 6 to 8 weeks is an hour long (and tbh can span a couple of sessions depending on complexity!). It covers your body from the inside out. It’s the appointment most women don’t know exists until something goes wrong.

You waited. You advocated for yourself. You went under. You woke up on the other side still hurting.

Here is the part most women are not told: by the time surgery happens, your nervous system has spent years adapting to persistent pain. The pelvic floor has been bracing. Pain pathways have shifted. Surgery removes the lesions. It doesn’t automatically undo any of that.

Do you know the difference between lubricants?

Some help with friction whilst others may moisten the tissues. They don’t help with muscle tension, nerve sensitivity, or hormonal changes to vaginal tissue.

If s*x is still painful, your body is asking for something more specific.
