Mr …, 42 years old, government service holder, normotensive, nondiabetic, nonsmoker, hailing from
…, presented with severe weakness with occasional dizziness for … months. He finds it difficult to
perform regular daily activities. The patient also feels a lump in the left upper abdomen which is
associated with heaviness and occasional dull pain for … months. He also complains of loss of
appetite, loss of about 20 kg of body weight, palpitation and occasional gum bleeding for … months.
There is no history of fever, bowel or bladder abnormality or other bleeding manifestations. He was
treated with ranitidin, iron tablets and vitamins. There is no such illness in his family.
On examination—general examination (mention as above).
Systemic examination (mention as above).
My diagnosis is hepatosplenomegaly with severe anemia for which I have some differential
diagnoses:
✓ Chronic myeloid leukemia
✓ Chronic liver disease with portal hypertension
✓ Myelofibrosis
✓ Tropical splenomegaly syndrome

Today I see patient came to outpatient clinic , only his CBC has thrombocytosis 880 others are normal

( إِيثَارُ الْعُزُوبَةِ لِلطَّالِبِ وَتَرْكُهُ التَّزْوِيجَ )

- الْمُسْتَحَبُّ لِطَالِبِ الْحَدِيثِ أَنْ يَكُونَ عَزَبًا مَا أَمْكَنَهُ ذَلِكَ؛ لِئَلَّا يَقْتَطِعَهُ الِاشْتِغَالُ بِحُقُوقِ الزَّوْجَةِ وَالِاهْتِمَامِ بِالْمَعِيشَةِ عَنِ الطَّلَبِ:

- عن أَحْمَد بْن الْمُغَلِّسِ، قَالَ: سَمِعْتُ بِشْرَ بْنَ الْحَارِثِ، يَقُولُ: «لَا تُؤثِرُوا عَلَى حَذْفِ الْعَلَائِقِ شَيْئًا، فَإِنِّي لَوْ كُلِّفْتُ أَنْ أَعُولَ دَجَاجَةً لَخِفْتُ أَنْ أَصِيرَ شُرَطِيًّا فِي الْجِسْرِ، وَمَنْ لَمْ يَحْتَجْ إِلَى النِّسَاءِ فَلْيَتَّقِ اللَّهَ وَلَا يَأْلَفْ أَفْخَاذَهُنَّ».

- عن خَلَف بْن تَمِيمٍ، قَالَ: سَمِعْتُ إِبْرَاهِيمَ بْنَ أَدْهَمَ، يَقُولُ: «مَنْ تَعَوَّدَ أَفْخَاذَ النِّسَاءِ لَمْ يُفْلِحْ»

- عن الْحَسَن بْن عَلِيٍّ - يَعْنِي ابْنَ عَفَّانَ الْعَامِرِيَّ الْكُوفِيَّ - يَقُولُ: سَمِعْتُ ابْنَ نُمَيْرٍ، يَقُولُ: قَالَ لِي سُفْيَانُ: " تَزَوَّجْتَ؟ قُلْتُ: لَا، قَالَ: مَا تَدْرِي مَا أَنْتَ فِيهِ مِنَ الْعَافِيَةِ.

- عن أَبي مَنْصُور الْحَارِث بْنَ مَنْصُورٍ، يَقُولُ: سَمِعْتُ سُفْيَانَ الثَّوْرِيَّ، يَقُولُ: «إِذَا تَزَوَّجَ الرَّجُلُ رَكِبَ الْبَحْرَ، فَإِذَا وُلِدَ لَهُ كُسِرَ بِهِ».
