: ASC,My names are Maryan Abdi, a Nurse by Profession and an IYCF Counsellor with SORRDO under UNICEF Supported Nutrition Program in the District of Wardhigley, Banadir Region.
: What do you do exactly as an IYCF Counsellor?
: Being an IYCF Counsellor it gives me a privilege to work and pass my knowledge to mothers especially pregnant and Lactating mothers when it comes to Optimal Infant and Young Child feeding Practices.
: What do you mean by Optimal infant and young Child feeding practices?
: Optimal infant and Young Child feeding practices are many among them are ; Early initiation of breastfeeding within one hour of birth and exclusive breastfeeding for the first six months of life, and beyond six months, timely and age-appropriate (in terms of quality and quantity) complementary feeding of children, with continued breastfeeding up to two years of age. This practice remains very critical to child care, nutrition and development.
: What are the challenges facing IYCF Practices in Somalia?
: In Somalia despite adequate Infant and Young Child Feeding practices being crucial for the growth and development of Children, only very few percentage of infants below six months are exclusively breastfed and at the same time very few percentage of mothers breastfeed their infants upto 12 months and it is hardly seen for a mother continuing to breastfeed her child upto 24 months.
: Again in your practice what are the reasons as to why mothers are not doing adequate breastfeeding?
: Mothers in Somalia faces myriads of problem. They carry many responsibility with them; carrying pregnancy, giving birth, caring for their children, fending for them etc.
Having said that some of the reasons from mothers I interacted with are; inadequate knowledge, myths that exist in the community, vigorous advertising of infant formulas and Lack of families support and encourage mothers to breastfeed their infants and ensure the well-being of mothers themselves
: As an IYCF Counsellor what are you doing to change the issues you raised that are hindering adequate breastfeeding?.
: As an IYCF Counsellor am trying to give guidiance and practical help to mothers on IYCF practices.
: Any work-related experience you can share with us?
: In my practice as an IYCF Counsellor I was able to correct many myths related to breastfeeding and also helped many mothers with Infant Young Child Feeding practices through practical teaching and guidiance.
I can remember a mother came to our Ambulatorio facility for delivery and it was her first birth. She was escorted by her mother to the facility. After she delivered we started to do skin contact and early initiation of breastfeeding for her child. Immediately the Mother who escorted the lady got hold of my hand and said” give the child this sugar water, the lady just gave birth and she has no Milk” she said.
I just laughed and told her” but the breast is full of Dambar( Colostrum in English” and she replied by saying “don’t feed the child on that first Milk. Is not good for the child”
From there I realised I have to educate the mother who gave birth and the one who accompanied her on the importance of colostrun and its importnace to the child interms of essential nutrients and immunity. I also counselled them on Exclusive Breastfeeding Feeding, feeding on demand, dangers of artificial milk, how to position and do proper attachement of baby to the breast etc.
Upto now the mother comes to our clinic for immunization and regular IYCF counselling and her child is doing well.
There are also other myths like a mother with small breast can satisfy her child and she should do mix feeding and I was able to talk to them and tell them that breast size doesn’t matter, any mother can produce what can satisfy her child
There are other beliefs that since Mogadishu weather is hot infants less than six months can not survive with water ontop of breast-milk and we are able to convince mothers that breast-milk contain contains enough water that can satisfy the child thirrst and that the infant don’t require any other food or drinlks except breast-Milk.
: Now that WBW is approaching us what is your message to the mothers and other people out there?
Maryan: My message Is this” In our glorious Quran ALLAH tells us in Surah Baqarah Vs 233” And mothers[should] breastfeed their children for a total of two years”
To the mothers out there” Adequate infant and young child nutrition (IYCN) practices are crucial for the growth and development of children. These, include early initiation of breastfeeding and exclusive breastfeeding for the first six months of life, To the fathers and families of breastfeeding mothers out there” Provide families support and encouragement to mothers breastfeeding their infants and ensure their well-being
To the general Public” Let us protect, Promote and support breastfeeding and inturn our children will be protected against Morbidity and Mortality”

SORRDO celebrated WBW with Wardhigley community through the following events.
>Promotion of optimal infant feeding practices at Barwaqo and Ambulatorio MCHs throughout the Breastfeeding Week. Our IYCF Counsellors were mandated to do this activity through-out the week. i.e , exclusive breastfeeding starting within one hour after birth until a baby is 6 months old, Timely introduction of Complementary feeds, while continuing to breastfeed for up to 2 years
>Held one day WBW commemoration at Ambulatorio MCH togetehr with MoH, District Authorities, Community Based Workers, graduates of Mother to Mother Support Groups, youths and other community members Printed breastfeeding Banners, T-shirts, Caps and Head-Scarfs with the 2019 theme and Picture of WBW and the printed materials were used during processions, commemoration, and Breastfeeding promotion sessions at facilities.

Abaay Mahamed Adam, age 22 years is a mother of two young children, Luul who is seven and half month old and Hussein who is 30 months old. Abaay was born, brought up and married in Bardheere before drought forced her and her family( husband and her son) to flee to Mogadishu in search of assistance in 2018. Since then she was living in an IDP camp in Barwaaqo, Wardhigley District.
Abaay was one of our beneficiaries who benefited from Mother to Mother Support group IYCF Counselling training supported through KFW Resilience program in the district of Wardhogley. Abaay was breastfeeding her daughter, Luul,, who was one month old when she was enrolled to the MTMBSG training session, a session where both theoretical and practical IYCF counselling are given in a structured way. The planned IYCF session was given once every week for eight weeks.
Just like her first born, ,Hussein, Abaay was giving mixed feeding i.e powder milk, Sugar water and breast-milk to her daughter. Abaay and her other group members under-went critical IYCF sessions (both theoretical and practical) and many gained knowledge and changed their atitude towards breastfeeding practices
Abaay, with the knowledge and support she got from IYCF counsellors and group members she was able to understand many misconceptions related to breastfeeding and also understand the benefits of breastfeeding to her child well. She was able to stop mixed feeding and concentrate feeding her child on breast-milk alone upto when her child reaches six months. Abaay also started her daughter on complementary feeding on times and still continues to breastfeed her daughter to date.

“ Previously I use to believe that my child cannot get satisfied with breast-milk alone and that giving suagar water and other breast-milk substitute can satisfy her hunger cues” she says “ but now I know that even if a mother has small breast she can breastfeed and satisfy her child well so long as a child is breastfeed well and on demand” she says. My child is now seven and half month old and very healthy and strong, I havent stopped breastfeeding her though I have introduced complementary feeds as per the advise from our IYCF Counsellor” she says
Abaay also does manual jobs like laundary to complement her family income and that has never distracted her from breastfeeding her child.
“ I do laundery at our neighbour sometimes as a source of income and always I carry my child with me to make sure whenever she needs breastmilk she gets it always” she says
Early initiation of breastfeeding and exclusive breastfeeding for the first six months of life, and beyond six months, timely and age-appropriate (in terms of quality and quantity) complementary feeding of children, with continued breastfeeding up to two years of age remains very critical to child care, nutrition and development.

HUMAN INTEREST STORY
REDUCING CHILD MORTALITY AND MALNUTRITION IN THE DISTRICT OF WARDHIGLEY.
Khalid Qassim Ibrahim is a 3 year old boy born to a very poor family who lives in Hantiwadaag waxda of Wardhigley District. His Mother Fartun Hassan and Father Qassim are jobless. Qassim is a porter while Fartun is a house wife. The family also has another 7 children to feed and with no steady income the family faces constant challenges to feed their kids.
Khalid was not getting the balanced food nutrients he is to get at his age and over sometime he started loosing weight and reached a point where his whole body were consumed by severe wasting and his health deteriorated for worse.
Khalid was taken to a stablization centre and because of his condition he was admitted for 5 days where he underwent medical treatment and Nutrition Rehabilitation. He was then discharged through an OTP program run by SORRDO.
Khalid came to our OTP on 29th Sept 2019 with the following anthropometric measuremnet ;
MUAC 10.2 CM, Weight 8.1 kgs, Height 75 cms.

Luckily Khalid’s appetite was good and was enrolled to OTP and put on RUTF, given systematic treatment. Fartun, khalid’s mother was happy and opportimistic that her child, khalid health will improve and he will play like his peers. Khalid got the treatment he most needed and his mother promised to feed the child well on the RUTF given and to follow other Homecare care message given i.e. hygiene, Systematic drug usage and follow-up for subsequent visit.
Two months laiter Khalid’s health and his general body condition has grately improved and has undergone complete recovery. On 17th Nov 2019, Khalid was discharged from OTP with the following Anthropometric recording; Muac of 12.6, Weight of 9.7 kgs and Height of 76 cm, thanks to the holistic IMAM services made available through joint WFP/UNICEF Resilience Programme in many facilities in Banadir District.

“Thanks GOD my son has improved very well ever since he was given the RUTF, he has undergone complete recovery, he can now afford to play with his pears. Thanks to all who have made this help possible for my child and many other children benefiting from this services” she says.
