Collaborative Joint Supportive Supervision Strengthens Mobile Outreach Health Services
May 4th, 2026.
The County Health Department (CHD), in collaboration with Yo’Care South Sudan, conducted a joint supportive supervision visit to one of the Integrated Medical Outreach sites operated by the Yo’Care mobile medical team under the NEAR Change Fund Emergency Project funded by NEAR Network, with Amalna as the Grantee and implemented by Yo’Care South Sudan in Duk County, Jonglei State.
The visit was led by the CHD Director, Mr. Simon Arak, who was joined by the Health Project Officer, Mr. Jacob Garang Awuoldit. During the supervision, the team engaged directly with patients and frontline health workers who were delivering services at the outreach site.
The purpose of the visit was to assess the quality-of-service delivery, assess adherence to clinical standards, and the level of client satisfaction among community members receiving care. The supervision team also reviewed how integrated primary healthcare services were being provided at the site.
The team observed a strong level of commitment and professionalism among the outreach staff. Services provided included outpatient consultations, immunization, and nutritional assessment. These services remain essential for communities that face long distances, transport challenges, and limited access to health services.
Beneficiaries and the CHD expressed appreciation to Donors and Yo’Care for their support, noting that the outreach services have improved access to essential healthcare at the community level. Many community members continue to benefit from the mobile health services, especially those who live far from health facilities.
Yo’Care’s outreach model continues to play an important role in improving service utilization, strengthening community trust, and bringing healthcare closer to the people who need it most.
Through close collaboration with the County Health Department and other partners, Yo’Care South Sudan remains committed to supporting quality, accessible, and people-centered health services for vulnerable communities.
