اوضحت دراسة بريطانية أن أكثر من 50% مراجعي عيادة تقويم الأسنان يحضرون بدوافع تجميلية ناتجة عن سوء ارتصاف الأسنان و خاصة الأمامية منها. إلا أن لتقويم الأسنان أهمية تتخطى حدود الأهداف الجمالية إلى تأمين عمل وظائف النطق و المضغ بشكل سليم, بالإضافة إلي حماية النسج الداعمة المحيطة بالأسنان. فسوء الاطباق الناتج عن تراكب الأسنان قد يودي لاحقا إلى مشاكل و الام على مستوى المفصل الفكي الصدغي, وتراكب الأسنان يؤدي إلى صعوبة تنظيفها بالشكل المطلوب و بالتالي ظهور افات لثوية و نخور سنية.

بكل اعتزاز وفخر شرفني أن أمثل سوريا الحبيبة والرابطة السورية لتقويم الأسنان بأعضائها ومجلس إدارتها في أكبر محفل علمي في اختصاصنا وهو المؤتمر العالمي لتقويم الأسنان الذي نظمته الجمعية العالمية لتقويم الأسنان WFO في البرازيل 22-25 /10/2025 .
كانت فرصة أكثر من رائعة للقاء الأصدقاء والأحبة من حول العالم ونتبادل الخبرات والتجارب لنرتقي بمهنة تقويم الأسنان لأعلى مستوى.

Dr. Luai Mahaini:

Consultant Orthodontist, German Board Certified (Facharzt), holds a PhD in Orthodontics from the University of Munich, Germany. He has over 20 years of clinical and academic experience in Germany, Syria, the United Arab Emirates, and Saudi Arabia.

He currently practices at White Smile Clinic in Al Shamkha, Abu Dhabi, and supervises postgraduate studies at Ajman University.

Previously, he held academic positions at Damascus University, Qalamoun University, and the International University for Science and Technology, and practiced clinically at MVZ Sigmaringen Clinic in Ulm, Germany.

Dr. Mahaini is the author of scientific books in the field of orthodontics and has numerous publications in peer-reviewed scientific journals. He is also a certified lecturer in mini-implant orthodontic systems in the Middle East and has received awards for clinical excellence.

Abstract:

The management of vertical dimension remains one of the most complex aspects of orthodontic diagnosis and treatment planning. Vertical discrepancies influence not only facial esthetics and occlusal function but also the stability of orthodontic outcomes. This lecture explores the multifactorial nature of vertical dimension control, addressing the interplay between skeletal growth patterns, dentoalveolar compensation, and soft tissue balance.

Common clinical challenges such as anterior open bite, excessive lower facial height, and posterior vertical excess will be discussed with emphasis on evidence-based treatment modalities. Strategies including vertical control mechanics with fixed appliances, temporary anchorage devices (TADs), clear aligners, and orthognathic interventions will be analyzed.

تأثير النموذج الوجهي، والنموذج الشكلي والمسببات المرضية في اختيار نوعية علاج الصنف الثالث

حين تطرح معالجات حالات الصنف الثالث يمكن التفكير بما يلي:
١- من الصعب معالجة معظم حالات الصنف الثالث لأنها ذات طبيعة وراثية وإن كانت من سبب وظيفي.. فهل إعادة التأهيل مجدية في هذه الحالات!!..
٢- كيف يمكن ان يؤثر النموذج الوجهي على قرار نوعية المعالجة التقويمية؟
٣- هل إعادة التأهيل الوظيفي مجد عند الكبار؟
٤- هل يمكننا إرجاع القوس السنية السفلية؟ وهل من الأفضل ان نتجنب الدوران الخلفي للفك السفلي (عكس عقارب الساعة) لمستوى الأطباق.
٥- هل يمكن ان نعتبر بروز الفكين هو عبارة عن اضطراب وظيفي؟ وهل العمل على تصحيح الوظائف الحيوية قد يكون مفتاح النجاح للمعالجات التقويمية؟
٦- هل لشكل الوجه والحفرة الفموية علاقة مباشرة بالنموذج الوجهي؟ ام بالأصل العرقي، ام بالمنظومة الوظيفية ككل؟

محاوله الإجابة على هذه الأسئلة عبر الخبرة السريرية لمعالجة عدد من حالات الصنف الثالث المختلفة في عرض شيق آمل أن يشكل إضافة جميلة للموضوع.

Dr Waddah Sabouni

Specialist orthodontist, received his DDS degree in 2002 from Aleppo University then his orthodontics degree in 2006 from Paris V University.
He did a master's in lingual orthodontics in 2009 and then a postgraduate in orthognathic surgery in 2011.
He is a member of the French Orthodontic Society, American Orthodontic Society, World Orthodontic Society, and is a board member of the French Aligner Society and European Align Society.
He is an assistant professor at the University of Dijon, La Reunion University, and the European Master of Aligners.
He has published over 30 peer-reviewed journal papers, in addition to a number of conference papers/abstracts and book chapters.
Dr Sabouni is an international speaker for Align Technology, a Diamond apex Provider since 2013, a member of the Invisalign Elite Club, and was the winner of the Align Technology International Research Awards in 2015.
He has private practices in the south of France and the UAE.

Abstract:

During this Lecture, Dr. Sabouni will be covering various aspects of clear aligner treatment.
From the scope of malocclusions that can be treated with aligners,
to different interdisciplinary treatments such as cases with extractions,Impacted canin or orthognathic surgery treatments, this conference will be exploring the broad aspects of the applicability of clear aligners within the orthodontic practice.
Learning objectives;
· Understand the various type of cases that can be treated with clear aligners today and how the advance in technology has broadened their applicability
to a wide range of malocclusions.
· Acquire knowledge about treatment considerationswhen working with patients with periodontal conditions.

The impacted maxillary incisor presents a significant clinical challenge with implications for aesthetics,function, and patient psychology.
Successful management requires a methodical, approach.
We will outline a systematic pathway from early diagnosis, highlighting the critical role of clinical examination and advanced 3D imaging (CBCT) to the formulation of a tailored treatment plan. We will explore the spectrum of management options, including interceptive measures, and orthodontic traction mechanics.
The objective is to provide clinicians with a clear, evidence-based framework to predictably guide impacted incisors into a functional and aesthetically pleasing position within the dental arch.

الملخص:
مع تقدم التقنيات الحديثة و طرق العلاج المختلفة فإن نجاح العلاج التقويمي يبقى معتمداً بشكل أساسي على التشخيص الصحيح للحالة، و ما هذه التطورات إلاّ وسائل علاجية للوصول إلى نتائج مُرضِية مبنية على التشخيص و التشخيص التفريقي الصحيحين.
يحتوي التشخيص الكامل على عدة ملفات يجب على اختصاصي تقويم الأسنان الخوض فيها و الإجابة على العديد من الاسئلة عبر الفحص السريري الدقيق، فحص للأمثلة السنية التقليدية او الرقمية، فحص الصور داخل و خارج الفموية، فحص الصور الشعاعية و التحاليل السيفالومترية، و من ثم القيام بوضع مخطط علاجي افتراضي يبين ما يجب عمله للوصول إلى نتيجة تحقق الغاية المطلوبة على المستويات الوظيفية، التجميلية، الإطباقية، و النفسية. بالإضافة إلى ما سبق تحقيق الثبات الأفضل على المدى المتوسط و على المدى الطويل.
تعتبر الصور الشعاعية و التحاليل السيفالومترية من أهمّ الوسائل التشخيصية في تقويم الأسنان و لقد قدم لنا الحاسوب و التطبيقات اللوغاريتمية المتوفرة الكثير لتسهيل هذه المهمة، و لكن هل يمكننا أن نثق ثقة تامة بهذه الوسائل المساعدة؟ هل التحاليل القائمة على الدراسات الإحصائية صحيحة بشكل كامل و لا تقتضي المراجعة؟ محاضرتي تجيب على هذا السؤال و توضح كيف نستطيع ان نرفع من مصداقية التحاليل اثناء دراسة الحالة لإعطاء التشخيص الصحيح الذي يضمن نجاح العلاج التقويمي بغضّ النظر عن الوسيلة العلاجية المتبعة.
