ในการนี้ผู้ที่ประสงค์จะใช้ประโยชน์จากข้อมูลเวชระเบียนดังกล่าว

To comply with Ministry of Public Health’s regulations,Kasemrad Sriburin Mae Sai
announces the annual destruction of expired medical records of 10 year.
All medical records of Out-patients were dated back 2024 - 2014
will be removed and destroyed.
For those who wish to keep your own recods, please contact
Kasemrad Mae sai Hospital medical record department by 1-30 September 2025.

Your identity document such as indentification card
or passport is required for the processing.
