🌐Wanna be… : Type 2 Dermal insertion structures คืออะไร⁉️
โดยทั่วไปเราเข้าใจว่าใบหน้าของเรามี Layer หลักๆอยู่ 5 ชั้น (skin,subcutaneous,SMAS/muscle,deep fat and bone) ซึ่งใน paper นี้จัดเป็น Type I : Layer by layer structure…เเต่ concept นี้อาจจะใช้ได้ดีเฉพาะส่วนของ lateral side of face เช่น temporal area, zygomatic area เเละ subzygoma area เป็นต้น
เเต่⁉️ทางด้าน medial side of face จาก concept เดิมอาจจะอธิบายได้ไม่ชัดเจนนัก ผมเลยอยากพาทุกคนมารู้จักกับ…🔽🔽🔽
🌟Type 2 Dermal insertion structures โดยมีเเบ่งได้ทั้งหมด 4 type
✅ Type 2a : muscle - dermis
✅ Type 2b : muscle - collagen - dermis
✅ Type 2c : bone to dermis
✅ Type 2d : intermediate facial layers - dermis
🌐โดย Type 2a เเละ 2b ก็คือ mimetic muscle บนใบหน้าของเรานั่นเองครับ เเต่จะมี insertion ในลักษณะที่เเตกต่างกัน
✅ Type 2a : muscle - dermis เช่น Lip levetor muscle ได้เเก่ Zygomatic major/minor, LLS, LLSAN โดย origin ของเค้าอยู่ที่ zygomatic bone เเละ maxillary bone เเต่ insertion จะไป attach ที่ dermis ตรงบริเวณของ Nasolabial fold เเละ merge ไปกับ OOM (peripheralis part), อีกตัวอย่างเช่น Corrugator supercilii muscel ที่มี dermal insertion ตรงส่วนใกล้ๆกับ vertical line of mid-pupil
📍ในด้านของ clinical application : 🔺เพิ่มความเเม่นยำในเรื่องการฉีด Toxin เช่นการฉีด ID ตรง tail of corrugator muscle หรือ การฉีด ID to superficial subq. บริเวณร่องเเก้ม เพื่อเเก้ไข gummy smile ที่เกิดจาก Zygomaticus minor, LLS เเละ LLSAN ตามลำดับ
✅ Type 2b : muscle - collagen - dermis เช่น DAO muscle (ดังรูปที่3) จะพบได้ว่ามี fibrous tissue(collagen) ขึ้นมาเกาะตลอดเเนวการวางตัวของ muscle ซึ่งจะเเตกต่างกันระหว่าง DAO ที่อยู่ lateral ต่อ marionette line เเละ DAO ในส่วน medial ซึ่งจะมี adhesion ที่เกาะเหนียวมากกว่า lateral (มี DLI วางตัวอยู่ด้านใต้ต่อ DAO) , อีก 1 ตัวอย่างซึ่งสำคัญมากๆ⁉️ก็คือ Platysma-SMAS ที่จะมี dermal inserion ด้วย collagen ในส่วนของ lower face and neck area
📍ในด้านของ clinical application : เนื่องจาก type 2b มี collagen มาเป็น dermal insertion ดังนั้น collagen stimulating treatment จะเข้ามามี role มากขึ้น (จากเดิมที่เราเคยใช้กลุ่ม filler มาฉีดเเก้ไขส่วนของ marionette line หรือ labiomental crease) เช่น EBD(ถ้ากลุ่ม USG ระวัง marginal mandibular br. of facial nerve ด้วยนะครับ) , *Biostimulator, Thread เเละ Toxin(ซึ่งก็ต้องระวัง DLI muscle โดยเเนะนำให้เน้นฉีดไปที่ DAO ส่วน lateral part เป็นหลัก)
🌐 ส่วน Type 2c เเละ 2d ก็คือ True เเละ False ligament ตามลำดับนั่นเองครับ ซึ่งถ้าไป match กับ paper ที่ผมเคยเเชร์ก่อนหน้านี้ที่กล่าวถึง New concept of ligament จะพบว่ามีทั้ง
✅ Superficial retinacular cutis : Vertical alignment ซึ่งผมเริ่มเห็น EBD บางตัวที่สามารถส่ง Energy เเบบ vertical ได้บ้างเเล้วครับซึ่งทำให้เกิดการกดตัวของ Superficial retinacular cutis ในเเนวตั้งได้นั่นเองครับ
✅ Deep retinacular cutis : Horizontal alignment ทำให้เกิดในส่วนของ sub-SMAS gliding plane ซึ่งเป็น concept หลักที่ใช้ในฉีด filler เพื่อ lifting ใน class ของ Wanna be (inboxได้น้า🥰)
