Extreme Obesity Rising Faster in US

Speaking about their work at a JAMA media briefing, lead author Ted D. Adams, of the School and also of Intermountain Healthcare at Salt Lake City, said the rate of extreme obesity is rising faster in the US than the rate of moderate obesity.

But unfortunately, tackling extreme obesity with lifestyle changes doesn't work very well in the long term for extremely obese patients, and neither does drug therapy.

This leaves surgery as the only effective option to help severely obese people sustain weight loss, says Adams. But, as he and his colleagues note in the background section of their paper:

"For this high-risk population, however, the number of studies reporting long-term weight loss following bariatric surgery are limited and generally have incomplete follow-up."
The Study
For their study, Adams and colleagues examined links between Roux-en-Y gastric bypass (RYGB) surgery and weight loss, type 2 diabetes, and other health risks, 6 years after surgery.

The participants were 1,156 severely obese people aged between 18 and 72. Severe obesity is defined as having a Body Mass Index (BMI) of 35 and over. A person's BMI is their weight in kilos divided by the square of their height in meters. 82% of the participants were female, and the average BMI was 46.

The participants were in three groups (one intervention group and two control groups): 418 who sought and received RYGB surgery, 417 who sought it but did not receive it (Adams said the main reason was on grounds of insufficient insurance coverage), and 321 who were randomly selected from a population-based sample.
The Results
The results showed that the average unadjusted weight loss in the surgical group was 27.7% between the beginning of the study and year 6, while over the same period, control group 1 (the ones who sought but did not have surgery) gained an average of 0.2% in weight, and control group 2 (the random population sample) did not gain or lose weight.

The authors write:

"At 2 years, 99 percent of surgical patients had maintained more than 10 percent weight loss from baseline and 94 percent had maintained more than 20 percent weight loss," plus:

"At 6 years, 96 percent of surgical patients had maintained more than 10 percent weight loss from baseline and 76 percent had maintained more than 20 percent weight loss."

As well as sustaining weight loss, the surgery group showed favorable improvements in other indicators.

Remission rates for type 2 diabetes over the 6 years were significantly higher in the surgery group than in either of the two control groups: 62% compared with 8% in control group 1 and 6% in control group 2.

Incidence of type 2 diabetes was also significantly lower in the surgery group: 2% versus 17% and 15% in control groups 1 and 2 respectively.

This favorable pattern was also reflected in other health measures, for example, remission rates for high blood pressure, low HDL-C (the "good" cholesterol), high LDL-C (the "bad" cholesterol), and high triglycerides were higher in the surgery group than in the two control groups, with improvements in high blood pressure and low HDL-C being especially significant.

The authors remark that improvements in metabolic and cardiovascular risk profiles were sustained in the surgery group over 6 years, while:

"In contrast, cardiovascular and metabolic status of severely obese control participants generally worsened during the 6-year period."
Importance of Findings
"These findings are important considering the rapid increase in total numbers of bariatric surgical operations performed in the United States and worldwide, and may have significant ramifications for the projected 31 million US individuals meeting criteria for bariatric surgery," conclude the authors.

In an accompanying editorial, Anita P. Courcoulas, a bariatric and general surgeon with University of Pittsburgh Medical Center, comments that an important feature of the study is that although the weight loss waned in years 2 to 6 in the surgery group, the control of the health risks remained "very good".

"These findings are important because they show in a RYGB cohort and control group with nearly complete follow-up at 6 years that weight loss and associated health benefits following RYGB are durable," writes Courcoulas.

"The mortality rates in this study were too small to assess statistically, but serve as a reminder of an uncommon but important outcome needing objective monitoring," she adds.

Grants from the National Institutes of Health/The National Institute of Diabetes and Digestive and Kidney Diseases and the National Center for Research Resources helped fund the study.

10 Easy Tips to Jump Start Your Energy Levels

Feeling tired with little or no energy? Join the club! Most people are always tired from sun up to sun down. Most of us simply accept this condition as a way of life. But honestly, being tired all the time is a signal from the body that something is inherently wrong.

Unfortunately, most people just dismiss these warnings and comfort themselves with “I just need some time off. Or, it’s not a big deal, I feel like this all the time”. Most never listen to the body, they do not address the problem and matters only get worse.
It’s kind a sort a like a leaky faucet. It is irritating and there is something wrong. But, you try to buy yourself more time. Most simply hope and pray the leak will go away. However, it only gets worse and as time goes on it becomes more expensive to fix. Most people treat their bodies the same way. And yes, the same is true with your body and your energy level. Feeling tired all the time is simply the body trying to tell you something and if you don’t fix it, it will only get worse.

1. Breathe Deeply

Oxygen is an excellent source of energy, but we do not take advantage of this God given source. The art of Yoga uses this system to energize and strengthen the body. Fresh air also helps rid the body of waste materials too. Start each day with this exercise and it will give your brain and muscles and instant boost.

Take a step outside if you can. Or, sit down in a comfortable spot with your back straight and head level. Keep your mouth closed and inhale deeply through your nose. Try to bring the air deep down into your tummy. If you are doing this correctly place one hand on your tummy the other on your rib cage and you should feel them expand. Try to hold your breath while you count slowly to 16. Repeat this up to 10 times. It doesn’t get any easier than this.

2. Drink Plenty of Water

Dehydration is a common energy drainer. How much water do you drink each day? Try drinking a large glass when you wake up and any time you feel like you need a pick me up. If you do not feel thirsty try to drink a minimum of eight 8 oz. glasses of water every day.

3. Supplement Your Diet

You may be suffering from malnutrition. Your body may simply be missing out on some of the required key nutrients, minerals and vitamins that it needs. Exactly what vitamins and minerals you should have every day? Learn more Running on empty so to speak can hurt. If you are not getting the daily requirements in your diet, consider taking essential vitamin supplements. I have found Iron and vitamin Bs to be the biggest culprits. 8. Stand or Sit Up Straight

Posture is key. Good posture improves energy levels. Do not sit slouched over. Stand tall, shoulders back, tummy in, or from a sitting position do the same.

9. Manage Your Weight

If you are over weight you might need to shed a few pounds. Carrying around the excess weight can make you very tired. Losing weight is simple, just take in less than physically exerted. 10. Take a Nap

Here’s a shocker! A short 20-30 minute nap will do wonders for you. It will revitalized your mind and your body especially right after lunch. Things to Know Before You Have Plastic Surgery

1. Health: Am I healthy enough to have plastic surgery?
Before even thinking about plastic surgery, you have to ask yourself if you’re healthy enough for it. Most people are great candidates for plastic surgery, health-wise. Make sure you disclose all of your medical history to your plastic surgeon. Hiding health problems because you think it will prevent you from having plastic surgery only hurts you. Your plastic surgeon can’t give an accurate assessment if you hide your health problems. And you won’t have a leg to stand on if something goes wrong! Lying to your surgeon will basically excuse them from responsibility if there are complications. True, certain health problems may prevent you from having plastic surgery, but wouldn’t you rather be alive as you are than a cosmetically-enhanced co**se?

2. Procedure: What is the name of the procedure and what does it entail?
Knowing the name of the procedure is different from knowing what it actually entails. This does not mean that you need to know the particulars, such as the type of scalpel your plastic surgeon is using, but you should know if the procedure is going to achieve the result you want or is the correct procedure for the body part for which you are seeking enhancement.

3. Risks: What are the bad things that can happen in connection to the procedure?
Things usually go well with plastic surgery. However, you need to know the possible complications that come with the particular procedure(s) you are having. Infection is just one complication that could occur. Also, you will have scars with plastic surgery. The scars may not always end up as a nice fine line. Sometimes scars can end up raised or widened. These are only some of the complications you should know about before having plastic surgery.
Another risk is that despite perfect technique or what is considered a good result by a plastic surgeon and his or her colleagues, you may not be happy with the final result. This occurs when the limitations of the procedure are not discussed thoroughly before surgery.

4. Benefits: What are the good things that can come from the procedure?
What are you hoping to gain from your surgery? Obviously, an improvement in the contour of a particular body part is expected. Perhaps you may even gain a boost in self-esteem. If you are expecting anything more than that, you should know the limitations of your procedure.

5. Limitations: What kind of result(s) can you expect from plastic surgery?
Your plastic surgeon can only control what happens inside the operating room. No matter how great a job your plastic surgeon does, it will not guarantee a job promotion, a relationship, or an improved social life or social status. Plastic surgery will not cure depression or eating disorders.
Also, if you aren’t willing to accept the procedure your plastic surgeon is recommending to get the best possible contour –- either because you’re worried about longer scars or a longer recovery –- then you have to be willing to accept a lesser result. There is almost nothing more frustrating to a plastic surgeon than a patient who expects the world, but is not willing to accept the plastic surgeon’s recommendation to achieve the best possible result. Not following post-surgery instructions can also cause a bad result.
Finally, if your goal is to look like a Barbie or to have a “perfect” nose or pair of breasts, you may be highly disappointed. Either you may end up looking too “plastic” or you may not think you look “good enough.” Your goal should be improvement. If “perfection” is attained with your surgery, consider that the icing on the cake or the cherry on top!
6. Alternatives: Are there any other ways to get the result(s) you want?
An important thing to ask yourself is: Are there other ways to get the result you desire without undergoing plastic surgery? This is important to know because plastic surgery is not without risks. Could you improve your diet? Could you move your body a little bit more and get more exercise? If you’ve given other alternatives an honest try, then plastic surgery may be a good alternative for you. Not having surgery at all is also an alternative.

7. Placement of scars: Where will your scars be? Will you be able to hide them?
You should know where your scars will be placed. Yes, you should expect scars with plastic surgery! This is especially important if you wear low-cut pants or shirts, bikinis, etc., or if your line of work involves skin exposure (such as actors, dancers, or models). Don’t be afraid to ask your surgeon where the scars will be. Don’t believe the hype that plastic surgeons perform “scarless surgery.” Plastic surgeons know where to hide or camouflage scars! Plastic surgeons also take pride in their incision closures, as we know this is what the patient often uses to define whether or not a procedure is successful.
8. Expense: Do you have the room in your budget to have plastic surgery?
Plastic surgery is not cheap! Whether you are paying cash or you are financing your surgery, you have to really determine if you can afford plastic surgery. Are there other things in your life that will take a big hit if you put money toward plastic surgery? This is especially true if you are financing your surgery. Long after you’ve recovered, are you willing to still be paying off your surgery? You’ll still have to pay, even if the result is not exactly what you wanted.

9. Revisions: Might you need more than one procedure to get the result you want?
You may need revision surgery. Revision surgery is not uncommon with plastic surgery. Revision surgery involves undergoing surgery again to improve upon prior surgery. It can be a small procedure that requires only local anesthesia or it can be a bigger procedure.

10. Recovery: Will you be able to take the appropriate time off to recover?
You will need time off to recover after plastic surgery. Depending on your procedure, you may need days or even weeks to recover. Depending on your job, even after the initial aches, pains, and bruising subside, you may still have restrictions ordered by your plastic surgeon. If you work, can you afford to take time off of work? This includes stay-at-home moms, as their work is so varied in the course of the day. If you choose to use your vacation time, you have to decide if it’s really worth exchanging rest and relaxation for recovery time.
