ประเภทของงูพิษในประเทศไทย แบ่งตามลักษณะของพิษงูเป็น 3 กลุ่มใหญ่ๆ ดังนี้:
1. งูพิษต่อระบบประสาท (Neurotoxin)
งูในกลุ่มนี้มีพิษที่ส่งผลต่อระบบประสาท ทำให้เกิดอัมพาตของกล้ามเนื้อและระบบหายใจ ซึ่งอาจเป็นอันตรายถึงชีวิต งูที่อยู่ในกลุ่มนี้ได้แก่:
งูเห่า (Cobra)
งูจงอาง (King cobra)
งูสามเหลี่ยม (Banded krait)
งูทับสมิงคลา (Malayan krait)
งูพิษต่อระบบโลหิต (Hematotoxin)

2. งูในกลุ่มนี้มีพิษที่ส่งผลต่อระบบโลหิต ทำให้เกิดความผิดปกติของการแข็งตัวของเลือด เลือดออกในอวัยวะต่างๆ และอาจนำไปสู่ภาวะไตวาย งูที่อยู่ในกลุ่มนี้ได้แก่:
งูแมวเซา (Russell’s viper)
งูกะปะ (Malayan pit viper)
งูเขียวหางไหม้ (Green pit viper)

3. งูพิษต่อระบบกล้ามเนื้อ (Myotoxin)
งูในกลุ่มนี้มีพิษที่ส่งผลต่อระบบกล้ามเนื้อ ทำให้เกิดการสลายของกล้ามเนื้อและอาจทำให้เกิดภาวะไตวายจากการอุดตันของไมโอโกลบิน งูที่อยู่ในกลุ่มนี้ได้แก่:
งูทะเล (Sea snake)

การจำแนกประเภทของงูพิษตามลักษณะของพิษช่วยในการวินิจฉัยและรักษาผู้ป่วยที่ถูกงูพิษกัดได้อย่างเหมาะสม

แนวข้อบ NL step 1 (ศรว) เรื่อง Snake Bite งูกัดเป็นเรื่องที่เราพบได้บ่อยทางคลินิก เราแบ่งงูเป็น 2 กลุ่มหลักๆ คือ Neurotoxic snake และ Hematoxic snake ซึ่ง Hematoxic snake มีหลักๆ 2 ชนิดคืองูแมวเซาและงูเขียวหางไหม้ โดยพิษของงูแมวเซาจะมีลักษณะเด่นคือนอกจากอาการเลือดออกแล้วยังทำให้เกิดไตวายเฉียบพลัน (Acute kidney injury) ได้อีกด้วย

P: palpitation ใจสั่น
H: Headache ปวดศีรษะ
E: Excessive sweating เหงื่อออกมาก

นอกจากนี้ยังมี sign and symptom 6 P of pheochromocytoma
6 P ได้แก่

P Pressure (hypertension)
P Pain (headache)
P Perspiration (sweating)
P Palpitation
P Pallor
P paroxysm

Rule of 10s: Pheochromocytoma
กฎ 10% ของ pheochromocytoma
10% of pheochromocytomas found incidentally and not associated with hypertension. 10% of pheochromocytomas were extra-adrenal (paragangliomas), 10% were bilateral, 10% were malignant, 10% occurred in children. 10% of these tumors were familial that was changed. Genetic involvement in tumors is closer to 30% and 50% of familial pheochromocytomas are bilateral. 20-40% of paragangliomas (extra-adrenal) are malignant.

ลองฝึกทำข้อสอบ NL step 1 หมวด endocrine system โจทย์ข้อนี้ต้องคิด recall 2 ชั้น

ขั้นแรก เป็นการ recall เกี่ยวกับกลุ่มอาการของ Cushing's Syndrome

ขั้นที่สอง ต้องรู้ว่า Cushing's Syndrome
พยาธิสภาพอยู่ที่ใด หลักๆ มี 3 ที่ คือ
1. adrenal cortex ชั้น zona fasciculata
2. anterior pituitary gland ชั้น par distalis
3. ectopic ACTH มักเป็น paraneplastic syndome เช่น brochial carcinoid tumor, small cell lung cancer, medullary thyoid cancer และ pheochromocytoma เป็นต้น

เคล็ดลับ !!!
Cushing's Syndrome สรุปเป็นคำกลอนได้ (คำคล้องจอง 3 พยางค์) ดังนี้
หน้าจันทร์แรม (moon face)
แก้มอ้อยเชื่อม (hyperglycemia)
เอื้อมไม่ไหว (proximal muscle weakness)
ยายมีหนวด (hirsutism)
ปวดกระดูก (osteoporosis)
ลูกทะเล (sodium retention, hypertension)
เท่เหมือนควาย (buffalo hump)
ลายหน้าท้อง (purplish striae)
หมองสกิน (steroid acne)
กินจนอ้วน (truncal obesity)
ด่วน mismense (abnormal menstruation)

เคล็ดลับ !!!
“Salt, Sugar, S*x: The deeper you go, the sweeter it gets”
และ G F R
ดังนั้น cortisol เกี่ยวกับ sugar ซึ่งเป็น ชั้นที่ 2 ของ adrenal cortex ตรงกับ อักษร F ย่อมาจาก Fasciculata
