What are qualifications of healthy s***m?

Semen Analysis is a method to identify s***m count, s***m motility and s***m abnormalities in order to diagnose infertility in males and primary preparation for having babies.

For appropriate s***m quality, we follow the criteria of World Health Organization (WHO).

Firstly, we will examine s***m volume which should be more than 1.5 milliliters. Next, we will examine s***m count which should be more than 15 million per milliliter.

Moreover, we will examine the percentage of s***m motility which should be more than 40% and select those with progressive motility which should be more than 32% and normal morphology should be more than 4%.

In summary, the key qualifications which affect fertility are s***m volume, s***m count, s***m motility and s***m morphology.

Khun Naritsara (32 yrs.) and her husband. After trying to have a baby for a long time, they were not successful since her husband underwent chemotherapy before. Trisomy 13, 18, 21! How do chromosome abnormalities affect pregnancy?!

In general, people have 23 pairs of chromosomes or a total of 46 chromosomes. Abnormalities of chromosomes can occur in various forms such as trisomy, monosomy, and microdeletion.

These abnormalities are frequently found on 13th, 18th and 21st pairs of chromosomes.

For all 3 pairs of chromosomes, the abnormalities of chromosomes can indicate genetic abnormalities of embryos. This is for the case that a mother sends her embryos for chromosome testing.

Please have a look at this, if 13th, 18th and 21st pair of chromosomes are abnormal, how will it affect a baby?

An abnormality of the extra 13th chromosome is called Patau syndrome. (Trisomy 13). It is the cause of cleft lip and cleft palate, microphthalmia, low-set ears, extra fingers or toes (polydactyly), hearing loss, and intellectual disabilities.

An abnormality of the extra 18th chromosome is called Edwards syndrome. It is the cause of a small abnormally shaped head, small jaws, low-set ears, cleft lip and cleft palate, overlapping fingers, defects of the lungs, kidneys, and stomach/intestines, and low IQ levels.

An abnormality of the extra 21th chromosome is called Down syndrome. It is the cause of a flattened and small head, almond-shaped eyes that slant up, a flattened bridge of the nose, low-set ears, small mouth, atrial septal defect, developmental disabilities and low IQ levels.

For ICSI program or In-vitro Fertilization, it has technology which is called Preimplantation Genetic Testing : PGT. Normally, we will remove embryo cells during blastocyst to identify genetic defects.

We will select only healthy embryos to transfer into the uterus. This method will help reduce the risk of babies’ abnormalities and avoid termination of pregnancy.

What should women eat after “Egg Retrieval”?

Apart from guidelines after egg retrieval, another thing that women often forget is food. Let’s see what women should eat after egg retrieval.

First of all, women should eat all 5 categories of food and the food must be cooked, fresh and clean. Avoid preserved food. We recommend “Egg White” due to high protein called albumin. This albumin helps retrieve fluid from the abdomen back to blood vessel.

Eating egg white is suitable for the patient who has a lot of eggs by ovarian stimulation or egg retrieval process. High egg count makes high estrogen levels. So, the patient will have the excessive abdominal fluid. As a result, abdominal distension or intestinal flatulence may happen.

Therefore, protein from egg white helps regain fluid, relieve flatulence and reduce ascites. Women can eat egg white for 2-3 eggs per meal. The egg yolk can be removed due to high cholesterol. It is mostly recommended to eat only egg white as it contains high protein.

For those who prepare for embryo transfer, we would advise to eat yogurt or drink fermented milk. Because lactobacillus will make a suitable environment for embryo implantation. However, please consume in moderation to avoid having diabetes.

Khun Chonnikarn (38 yrs.) and her husband. They had attempted to have a baby for over 1 year. Since she is almost 40 years old, she decided to consult with Dr. Mai at Prime Fertility Clinic. What is closed embryo culture system?

There are two different methods of embryo culture as below:

Open System: Embryo culture is performed in an in-vitro or a plate and a culture medium directly exposes to the environment inside an incubator.
Closed System: Embryos are placed in microdroplets of culture media covered by the culture oil.

Benefits of Closed System:

The culture oil can protect droplets of culture media from pathogen and microorganism floating inside the incubator.

Prevent fluid evaporation of a medium droplet

Slow down the CO2 diffusion from culture media

Increase the stabilization of temperature, pH and osmolarity

Do ge***al warts affect fertility?

Ge***al warts are a skin and tissue disease caused by human papillomavirus or HPV which is related to the virus that cause cervical cancer.

Generally, ge***al warts can spread through a direct contact and are a s*xually transmitted disease as well.

The symptom is the appearance of small polyps on skin or tissue. These can be found in both males and females but we often find more symptoms in females. The disease makes some warts on v***a, va**na, perineum, top of thighs or even a**s.

Having ge***al warts affect fertility or not? Perhaps yes. In case a patient has seriously suffered from the disease for a long time and the ge***al warts become very huge then obstruct or block the v***a.

If the v***a is blocked, the patient will have problem during s*xual in*******se. Or a huge va**nal wart will disturb s***m pathway to approach cervix. Under the serious circumstance, the patient will experience infertility.

However, people normally don’t wait until the disease becomes that much severe. The disease itself doesn’t cause infertility. But its effect happens when the baby is born. As the newborn is risky to get infection to the eyes, nose, mouth or even neck and vocal cords. Nowadays, there are vaccines that can prevent ge***al warts or pathogenic HPV strains.

Why can’t you wear makeup on the embryo transfer day!?

Have you ever wondered why doctor doesn’t allow patients to wear makeup when they are coming for embryo transfer? It’s because to apply nail polish, spray perfume, apply lotion, apply powder may affect the quality of embryo.

To avoid makeup on the embryo transfer day due to the chemical inside nail polish or even some chemical in various cosmetics can spread out.

These substances affect embryos or gametes (both eggs and s***m). Since an embryo is very sensitive. In case of chemical exposure of embryos, the quality may drop or the embryos may die.

Therefore, the decline in embryo quality makes the less chance of endometrial attachment, implantation or even poor embryo development consequently.

However, you can take a shower, wash your hair or soap yourself due to these are considered as washing activities.

Thus, on the embryo transfer day, women should refrain from chemical exposure and avoid using chemicals as much as possible, e.g., perfume, nail polish, makeup, lotion or whatever volatile substance. Mostly the doctor will suggest avoiding during this time.

What are mosaic embryos? How do they relate to pregnancy?
After receiving the embryo’s chromosomes testing report (PGS, PGT-A), most of couples expect to get euploid embryos while aneuploid embryos are expected to happen as little as possible. In fact, there is another type of embryos that some people have ever heard. It is “Mosaic Embryos”.

What are mosaic embryos? How do they relate to pregnancy?

Mosaic embryos mean embryos that contain a mixture of normal and abnormal cells making them unidentifiable exactly whether they are normal or abnormal. Mosaic embryos come from the abnormal cell division of embryos. As a result, some cells have excessive chromosomes while others lack of chromosomes.

Concerning question is if mosaic embryos can be used for embryo transfer?
This issue is very important. A patient needs to understand that the report of mosaic embryos is unclear. A mosaic embryo can be normal or abnormal so that possible subsequent risks and conditions after pregnancy will be considered.
