What are causes of degenerate cells in blastocyst?

Embryo cells that get damaged or cell death in blastocyst stage during the assisted reproductive technology or embryo which is from natural fertilization. It can occur from 2 natural processes which are Necrosis and Apoptosis.

The damages that occur from Necrosis process are involved in swelling of cells and breakage of cell walls. The damages that occur from Apoptosis process is involved in development stage of cell including few cells and egg shell which is called Zona Pellucida: ZP

Also, cells will stop developing in a blastocyst stage which is due to the shrinkage of cells. It leads to damages in nucleus area, cell wall area and DNA.

When is the right time for fertility checkup?

Fertility checkup is considered as an important element for a couple wishing to have a baby: there are male fertility checkup and female fertility checkup.

For female fertility checkup, it is recommended to get a checkup on day 2 or day 3 of menstrual cycle since the o***y will be stable. It is a suitable period for ultrasound scan to check ovarian cyst.

Also, it is to check antral follicle count together with the condition of the uterus.

For male fertility checkup, there is no specific period. However, men should refrain from ej*******on 2-7 days before the checkup to keep s***m at its best quality. So that we can check whether s***m quality is good or not.

Therefore, if you want to have a baby, it is recommended getting a checkup 3 months in advance to reduce any risks that might occur during pregnancy.

What is s***m freezing? Who is suitable for this program?

It is a process in which s***m will be collected and proceeded to freezing step. In general, we will spin s***m first to help select s***m with good quality and good motility. Then, selecting the good ones for preservation.

What is it for? It is to preserve s***m in terms of quality.

Who is suitable for s***m freezing? Firstly, it is suitable for men who will receive medical treatments. For example, in case of cancer, an orchiectomy may need to be performed. S***m production will be insufficient. This program allows s***m to be frozen for future use.

In addition, men who will receive chemotherapy around pelvic area which will damage s***m production, we recommend to do s***m freezing program.

Moreover, there is a social factor that men are unavailable to collect s***m for ICSI or IUI at the moment. The s***m will be frozen and thawed on the procedure day for women.

The best way is to use fresh s***m since frozen s***m quality might be dropped by 40-50% which means some s***m die during the process. Even though half of s***m don’t survive during freezing step, it’s still enough to thaw and proceed to ICSI procedure.

Therefore, s***m freezing is not a top option. It would be best to use fresh s***m. However, for those with problems stated above, it’s recommended freezing s***m for future use.

Does Irregular Sleep Pattern Affect Infertility Treatment?

The answer is yes. Irregular sleep patterns or insufficient sleep affect both males and females.

In females, irregular sleep patterns affect their egg quality. Insufficient sleep leads to lower egg quality and, thus, poorer response to ovarian stimulation medication.

In addition, if women stress and do not get enough sleep at the same time, it could lead to abnormal ovulation, such as irregular ovulation or irregular menstruation.

At the same time, in males, irregular sleep patterns and insufficient sleep lead to poorer s***m quality. Hence, sleep patterns affect infertility in both males and females.

Nevertheless, in those who have enough sleep at 8 hours but without a regular pattern, it could also affect their fertility. The reason is that irregular sleep patterns affect growth hormone release and changing sleep cycles. Thus, it could leave a negative impact on s***m and egg quality.

Those who have irregular sleep patterns have a poorer ovarian response than those who have a regular one. Hence, though with enough sleep, the irregular patterns could lead to infertility as well.

Can Women Exercise During Ovarian Stimulation?

Ovarian stimulation is another important process for women who receive infertility treatment. It is essential for women to take good care of themselves.

Whether exercising is fine or not depends on the period of ovarian stimulation. It is totally fine to exercise before the stimulation process.

However, if you are in the process of ovarian stimulation, you may do some mild exercise that does not contain a lot of movement. In addition, it should be a low-impact exercise.

This is because, during ovarian stimulation, especially those with many eggs, the eggs will be stimulated and grow due to the exercise, such as shaking or running. The small ovarian will expand in size.

As a result, there is a possibility that the ovarian torsion will occur. Some may experience ovarian rupture due to the size of the eggs, resulting in abdominal pain or bleeding. As a consequence, the patient may need an emergency surgery or ovarian surgery.

Thalassemia and IVF

Thalassemia is a severe anemia that can be genetically inherited. There is a 25% risk that the child born from parents in which one of the couple is a carrier of thalassemia would develop severe thalassemia. The carriers will exhibit no symptom of thalassemia and, thus, may not be aware that they are a carrier.

The way to identify whether one is a carrier of thalassemia or not is to take the blood test.

If both parents are carriers of thalassemia but wish to have a healthy child, firstly, the parents should take the screening test for thalassemia before having a child. The test will be a**lyzed to see if the parents have a chance to pass the disorder to their child or not.

If the result reveals that both parents are the carrier, it is recommended using assisted reproductive technology or IVF. The procedure is done by fertilizing the egg with s***m in the lab until we get the embryo.

Then, the patient may take a screening test for thalassemia using the PGD method (Preimplantation Genetic Diagnostic). After the embryo test, we will transfer the healthy embryo into the uterus and let it develop further.

Is Having Twins Possible for IUI?

Yes, It is possible to have twins by IUI. In fact, ones have 10-15% chance of pregnancy by IUI. There is a 5-10% chance of having twins from this method. The common twins found in IUI are dizygotic twins.

As IUI starts by ovarian stimulation, it is possible for women to have more than 1 egg, and thus have more than 1 child in one pregnancy. In addition, due to the stimulation, there is a likelihood that the embryo could split into twins, but this is not commonly found.

Nevertheless, there is only 1% chance of having triplets in one pregnancy, depending on the standard stimulation. Normally, the doctor will check through the monitor to examine the possibility of twins when the patients have too many eggs.

If ones have too many eggs, the doctor will postpone s***m injection or prohibit s*xual in*******se in that cycle to prevent twins pregnancy. Then, the doctor may adjust drug levels to reduce the number of eggs. The reason is because twins pregnancy has a high chance of miscarriage.

Vitamins to Take Before Ovarian Stimulation

Women should take vitamins for at least 15 days to 1 month before ovarian stimulation. But, which vitamins will be beneficial before the stimulation?

The first one is folic acid, which helps prevent the newborn from developing abnormalities in the brain and spinal cord. Plus, folic acid also helps increase the success rate of infertility treatment.

In addition, women should take antioxidant vitamins because these vitamins help improve egg quality. It is important to note that we cannot increase the number of eggs. Vitamins will only help with the quality of eggs.

Hence, it is recommended taking coenzyme Q10, vitamin C, vitamin E, L-Arginine, L-carnitine, and astaxanthin. These vitamins help improve the quality of eggs.

Moreover, the vitamin that helps improve success rate is vitamin D. It is recommended to take vitamin D3 since it is easy for the body to absorb.

Khun Aungsana (35 yrs.) and her husband had tried to have a child for 1 year. They entered IUI program at another clinic 2 times but still failed. When they changed to Prime Fertility Clinic and consulted with Dr.Mai, Primary exam results revealed that her husband had low s***m count. She herself had an almost 5-cm ovarian cyst. Khun Aungsana went through ovarian stimulation 2 times. There were 2 embryos sent to chromosome testing on the 1st cycle, only 1 was qualified. On the 2nd cycle, 5 embryos were sent to chromosome testing, 2 were qualified. Finally, Khun Aungsana successfully got pregnant in 1 cycle of embryo transfer.

What Does the Color of Semen Say?

Have you ever observed the color of your s***m?

Normally, the color of semen is grayish-white. If the color changes from its usual look, it could be a sign of health problems.

If the color of your semen is different from its usual look, it is recommended seeing a doctor for the examination. If the semen is yellow, we need to check if the semen is mixed with urine. Otherwise, the cause could be jaundice as it causes the body to have a high level of bilirubin, which results in yellow pigment mixed with semen.

If the color of semen is red, you need to take an examination to see if there is a wound, infection, or cancer in the seminal tract that causes semen to be red, or mixed with blood.

If the color of semen is green, there might be an infection such as prostate or seminal tract infection.

The changing color of semen can be a sign of abnormalities in male reproductive system. However, the color cannot tell s***m quality, such as concentration, motility, or s***m morphology.

Hence, it is recommended to see a doctor to examine further abnormalities that may lead to infertility.
