S***M SLOWTH, a selection of mature s***m, increases fertilization rate

How to use S***m SlowTH for ICSI
S***m SlowTH contains Hyaluronan which has a property for mature s***m selection for fertilization.

Once a mature s***m nucleus approaches Hyaluronan, it will decrease s***m motility.

When s***m selection from S***m SlowTH proceeds to ICSI, the scientists will consider s***m with declining motility together with s***m morphology to select the most mature one for ICSI. It helps increase fertilization rate and also the growth of embryos.

Tips to improve s***m quality for father-to-be!

For men who are planning to fulfill a family, a good way is a preparation by which you can get a check up and semen analysis to see whether your s***m is healthy or not. This way, you will know an appropriate method to prepare yourself.

Also, we would like to provide some tips to improve s***m quality as follows:

· Exercise to control your weight – Since being overweight or obesity will affect testosterone hormone that may decrease s***m production. Recommended exercises such as running, aerobic dance, riding a bike at least 30-45 minutes per day.
· Refrain from alcohol drinking – Since alcohol will affect testosterone hormone that may decrease s***m production.

What is Dermoid Cyst?

Dermoid cyst is a type of ovarian cyst that has a low chance of transforming into cancer in the future. Only 1-2% of dermoid cysts are cancerous. It may contain structures such as fat, hair, and teeth inside.

It causes developmental anomalies that the cells of hair and teeth are developed in the cyst. It can be said that dermoid cyst is derived from stem cells that develop to the form of cyst with hair and teeth.

If you ask whether it affects fertility or not? We would say it would cause some effects in fertility. A big size of dermoid cyst can interrupt o***y functions which needs to be removed since it cannot be treated with medication.

It means that the surgery damages the affected o***y which results in low ovarian reserve.

Most importantly, it causes frequent complications. A big size of dermoid cyst causes torsion which stops ovarian functions.

Finally, if you go to the hospital too late, the thing is that we might need to perform a surgery to remove the o***y which affects infertility since you will have only 1 o***y left.

Should patients, during fertility treatment, eat vegetarian food?

For this coming vegetarian festival, does eating vegetarian food affect patients during egg stimulation or embryo transfer? We would say yes. It causes some effects to the patients.

General practice of vegetarian festivals, people will need to refrain from eating all animal-based foods such as meat, seafood, eggs, dairy, oil, and butter which are derived from animals.

Also, animal products are the source of protein. Protein comprises amino acids which some of amino acids are not in plants. Hence, human body cannot synthesize the amino acids on their own and nutrition deficiency can affect human’s health.

We also have some substitution which is plant protein such as nuts, seaweed and mushroom. It can be a substitution of protein but high consumption is needed.

However, there is no reliable research to support that eating vegetarian food will decline egg quality.
Since it takes around 10 days for the festival, we recommend better skip to maintain health nourishment. For medical purposes, it would be best to eat 5 food groups to gain nutrition from plants and meat especially eggs and dairy products which are protein sources.

A lack of nutrition may decline egg quality and s***m quality. This is what we need to be concerned about.
Also, food groups such as tofu, soy milk and textured vegetable protein which most of them are made from nuts.

Especially soy beans that contain female hormones which is called Phytoestrogen. In case of too much consumption, it can stimulate female reproductive organs such as abnormal endometrial conditions. It can also affect those who are during embryo transfer preparation in case that the patients are sensitive to hormones.

Therefore, for those who eat too much vegetarian food and nuts, some problems may arise during embryo transfer and implantation.

However, it is also possible to eat vegetarian. Then, you can proceed to egg stimulation and endometrial preparation next cycle that you can eat 5 food groups.

In any cases, please consult with your doctor for more suggestions if you should avoid vegetarian food during the treatment or not.

Preparation of the Endometrium: What to prepare and nourish?

There are various factors to prepare the endometrium to be proper for embryo transfer such as physical preparation and lifestyle. Selecting vitamins for nourishment is also important.

Here is a checklist for physical preparation and nourishment:
- Get a lot of rest. It is best before 4-5 pm
- Refrain from stress
- Refrain from alcohol, smoke, pollution and chemicals
- Work out
- Eat 5 food groups: fresh, cooked and clean
- Refrain from food that can cause diarrhea or any sickness. If your health is declining, it will affect uterine quality.

For food, we would like to recommend food that contains lactobacillus such as fermented milk and yogurt making a proper condition of pelvic for embryo implantation.

For vitamins, it is recommended taking 5mg of folic acid/day or 1 tablet/day including other vitamins such as vitamin D for 1,000 – 2,000 IU and vitamin E for 200 – 400 IU per day. This will help with better embryo implantation.

To be sure, it is recommended consulting with the doctor before taking any vitamins.

How does chocolate cyst affect women?

Chocolate Cyst is a type of ovarian cyst. It is a subgroup of endometriosis and it is found mostly in women of reproductive age.

There are effects such as hurt and pain. It causes gradually severe menstrual cramps and women can be less responsive to painkillers. The symptoms can also be more severe.

It can also spread out and cause pain in the pelvic area such as tenesmus, stomach pain, hip pain and abdominal adhesions. Since chocolate cyst or endometriosis will grow randomly in the stomach area causing adhesions on the fallopian tube, ovaries and hydrosalpinx, it means that it can cause long-term impacts and also affect fertility.

In addition, the egg quality will be worse if the chocolate cyst becomes bigger since it will damage ovaries causing low egg count. If women need surgery, it will rapidly damage the ovaries.

Most importantly, those who have chocolate cysts should be aware that there is a 50% chance that it can reoccur within 5 years after the surgery in case the patients skip their medication. Also, the medication is recommended until menopause starts. Women can stop the medication once they would like to have a baby. However, after giving birth, they will need to continue taking medication.

Therefore, it is recommended seeing the doctor if you have some suspicious symptoms.

What would be the criteria for Semen Analysis according to WHO 2021?

Semen Analysis according to WHO 2021 is a semen examination in both quantitative methods (Volume and motility) and qualitative methods (Morphology). The latest WHO criteria are in the year 2021

It is to determine the cause of male infertility and also to follow up those who received vasectomy.

The analysis is divided into 2 sections as follows:

Macroscopic examination: It is a visual inspection of semen on liquefaction, viscosity, appearance, volume pH etc.
Microscopic examination: It is a method which uses a microscope to examine s***m on total count, motility, viability, morphology, total white blood cell count, total white blood cell count etc.
In order to examine s***m abnormalities, WHO 2021 specify the criteria of semen analysis as follows:

Volume should be equal or more than 1.4 milliliters
Total count should be equal or more than 39 million
Motility should be equal or more than 42%
Viability should be equal or more than 54%
Morphology should be equal or more than 4 %
If there are any abnormal indicators after the result is released, men can get next semen analysis in next 90 days since the full cycle of semen production takes 3 months starting from cell regeneration until travel to the tube.

What is ERA TEST?

Even though a healthy embryo with normal chromosomes is being transferred, many couples are still facing disappointment of not getting pregnant. It is true that healthy embryos enhance pregnancy rate. However, endometrial condition is also an important factor for embryo implantation.

ERA TEST or Endometrial Receptivity Analysis Test, developed and patented by Igenomix (PCT/ ES2009/000386 is a test designed to evaluate endometrial receptivity by gene expression. About 200 genes will be evaluated to help plan a personal window of implantation for the most suitable embryo transfer timing.

It is found that the era test, evaluating the best timing for embryo transfer, enhances the pregnancy rate approximately around 25% and pregnancy rate in women receiving era test is higher than those without the test. Also, it reduces the loss of healthy embryos from unsuccessful implantation.

Nevertheless, the era test is not necessary for all patients since there are various factors affecting pregnancy success rate. Therefore, it is recommended seeing a specialist for suitable advice further.

Severe Menstrual Cramps, can it increase the risk of having other diseases?
Severe menstrual cramps may lead to other diseases as below:

Endometriosis: The endometrial lining tissues grow outside of the uterus area. This can also grow randomly in the stomach and on the ovaries. Some may cause adhesion inside. It may cause chocolate cyst in some cases.

Myoma uteri: It is a tumor that develops in the muscle tissue of the uterus and it enlarges in the uterus. It can also twist the endometrium depending on its size and its position that may press other organs.

Adenomyosis: It is the same type of endometriosis. Instead of growing randomly in the stomach area, it grows in the muscle tissue of the uterus. It enlarges the uterus and causes menstrual pain and severe bleeding.

Does it affect fertility? We would say yes. We recommend having pelvic examination and ultrasound to see if there are any cysts or any tumors. In case of chronic menstrual pain, the concern will be about stomach adhesions.

Therefore, if you have abnormal menstrual cramps, we would recommend seeing the doctor is the best way.

In some cases, the patients with low progesterone symptoms which is called Luteal phase defect. It means that secretion of progesterone is lower than normal. This leads to improper condition of uterine cavity for implantation. As a result, it is difficult for those with low progesterone to conceive.

How do we know if we have low progesterone? In most cases, women need a medical checkup to check their hormone levels. Nevertheless, we could say that these symptoms are very rare.

In general, women have normal levels of progesterone. However, if we find out that progesterone levels tend to be low, we will need to use Luteal phase support which is the administration of progesterone. For example, oral medication, vaginal progesterone and injection.

What are causes of degenerate cells in blastocyst?

Embryo cells that get damaged or cell death in blastocyst stage during the assisted reproductive technology or embryo which is from natural fertilization. It can occur from 2 natural processes which are Necrosis and Apoptosis.

The damages that occur from Necrosis process are involved in swelling of cells and breakage of cell walls. The damages that occur from Apoptosis process is involved in development stage of cell including few cells and egg shell which is called Zona Pellucida: ZP

Also, cells will stop developing in a blastocyst stage which is due to the shrinkage of cells. It leads to damages in nucleus area, cell wall area and DNA.
