Thinking about a hair transplant before age 25? Read this first. 📉👇

Noticing a receding hairline in your early twenties can be emotionally difficult, and it’s natural to want an immediate fix.
However, at HLC Clinic, we believe in being honest: getting a hair transplant too early can often make things worse in the long run.

Here is why timing matters more than you think:
🔹 Hair loss is progressive. Your final pattern usually isn’t clear until your late 20s or early 30s. If we transplant hair now and your native hair continues to fall out behind it, you could end up with an unnatural „island“ of hair surrounded by thinning or bald areas.
🔹 Your donor area is NOT renewable. Every graft we extract is permanent and gone from the donor site forever. If a clinic over-harvests your donor area too early, you may not have enough healthy supply left for future corrections when you actually need them. We cannot create grafts from nothing once they are gone.
🔹 Medication is often the better first step. For many young patients, starting Finasteride or Minoxidil can slow or even halt progression. This gives us time to observe your pattern while preserving the hair you already have.
When does surgery make sense for younger patients? We evaluate every case individually, but we may proceed if:

• Your hair loss has been stable for 1-2 years on medication and you will continue with the hairloss medications.
• You have realistic expectations (designing a hairline for your 40s and 50s, not recreating a teenage hairline).
• The hair loss is already advanced (Norwood 4-5) and the pattern is clearly established.

Don’t rush a decision you can’t undo. 🛑 We regularly see patients who chose „cheap“ clinics at 21 and are now left with visible scarring and depleted donor areas that are nearly impossible to fix.

Our advice? Document your progression with photos every 6 months and see a specialist for a proper diagnosis. We would rather tell you „not yet“ than have you live with a result you regret.

The demand for repair surgeries is still, unfortunately, very high. ⚠️

What’s alarming is that these cases don’t just come from low-cost providers — we often see failed transplants from well-known clinics in Europe and the US too.

In this particular case, we had to completely remove the previously transplanted, incorrect hairline. ❌➡️

Extracting and re-implanting single grafts can be done in one step — but only if the number is small. ✅

However, when 800–1000 grafts need to be removed, like in this case, the process takes 2–3 stages and around 6–12 months. ⏳

Step one: Most grafts are extracted manually
Then we wait 3–4 months for healing

Because the hairline is such a visible area, we must be gentle and patient.

Sometimes, transected hairs grow back — these are removed in a second step, followed by another healing phase. 🔁

If the healing goes well and no visible scars remain, we can either transplant a new hairline — or leave it natural.

In this case, removal was necessary. The hairline was too low, looked unnatural, and the patient was far too young. 🙅‍♂️📉

Here is a result of a natural hairline transplant with high density. The patient initially had an SMP (Scalp Micropigmentation) done for the temples and triangles. . The hair loss continued, and the SMP became visible. Therefore, we always warn against getting an SMP only for the hairline and triangles if the hair loss is ongoing. If you decide on a buzz cut or want more volume on the crown, you can achieve very good results with SMP if it is done correctly. However, if only the temples and hairline are treated with SMP and hair loss continues, the only options left are to transplant these visible SMP areas or remove them with laser treatment.

For this patient, we transplanted approximately 2200 grafts for the temples and hairline. Seven months post-op, the patient had the SMP removed by laser. Normally, if the SMP is done really well, laser treatment is not necessary because the SMP adds extra volume. In this case, however, the SMP had turned green/blue, and this color change bothered the patient, so he decided to have it removed.

The patient is on hair loss medication and may in the future want to slightly lower the hairline
