▫️Hello we receive a lot of complaints about bleeding and spotting after IVF transfer. Is it normal, can you explain a little about it?

🔺We normally do not expect bleeding after IVF treatment. Because first we give medicine to prevent the shedding of the uterus, which will increase the adhesion of the embryo, so that the patient's uterus can be more stabilized and accept the embryo.

🔺Then we give the progesterone hormone to the patients, and especially after we give this hormone, we do not expect any bleeding under normal conditions.

🔺But in some patients, small spotting may occur within a few days after the embryo attaches, but this shouldn’t look like menstruation.

🔺I have to warn you that spotting is not as much as menstrual bleeding. It may appear as bleeding, but if you have more bleeding, you should contact your doctor immediately.

🗣In vitro fertilization (IVF) takes place in two stages. The first of these is to develop eggs, collect these eggs and create embryos, and the second stage is when developed embryos are transferred to the mother.

🗣Therefore, in order to develop the egg, if without medication we do follow the eggs developed within the patient's own physiology,without any interference. Or, At during the follow-up, we prevent the development from stopping with very small touches at certain times, and by helping it with very light medications,we follow the egg development and collect those eggs and perform in vitro fertilization(IVF).

🗣Yes,But it is necessary to apply this to certain group of patients. Especially, no matter how much medicine we give, we may not receive many eggs, we generally prefer it in patients with diminished ovarian reserve.

▫️ Patients with negative IVF trials need to be reassessed when they visit the doctor and their stories are very important, especially in other centers, which is a bit more of a problem for the doctor because the doctor also needs the details of those treatments from other centers.

▫️We need to focus on two phases of IVF treatment. One of them is egg collection and embryo development and the other is the part after the transfer. Some patients who may have obtained very good eggs may have also obtained a very good quality embryo, but after some kind of transfer, these embryos may not hold. If good embryo transfer did not succeed in some way, then it is useful to focus more on the uterus first. We may need to review the inside of the uterus. We perform hysteroscopy on these patients and then try to increase their chances of success.

▫️ If we have not achieved a good embryo or if the embryos transferred in previous trials do not appear to be of very high quality and as a result has failed, it is useful to focus on what we can do to improve embryo quality. Changing the treatment protocols of these patients may significantly affect embryo quality. Genetic problems in both men and women may have negative effects on embryo quality by transferring them to the embryo, which is useful in investigating this situation.

▫️ We also need to check the biochemical and hormonal status of the patient. Thyroid hormone deficiency, diabetes, vitamin D deficiency, anemia, coagulant diseases, and certain drugs that using continuously will decrease both embryo development and the chance of embryos holding in utero.

In this photograph, we see a beautiful intrauterine structure in the form of three lines on ultrasonography. Ultrasonography performed during IVF treatment provides valuable information about the appearance of the uterus. Ultrasonography is used to evaluate the thickness, smoothness and structural conformity of the uterine lining. It also gives us the opportunity to make a definitive diagnosis or suspect if there are any problems in the uterus such as polyps, fibroids or other abnormalities. According to ultrasonography, we can determine whether there is a suitable uterine lining thickness and a proper uterine structure in which the embryo can implant. Therefore, it is an important tool in IVF treatment in order to make the best timing for embryo transfer and increase the chance of success.
