Trigeminal neuralgia (TN) is a chronic pain disorder characterized by sudden, severe, electric shock-like facial pain along one or more branches of the trigeminal nerve. The pain is often triggered by routine activities such as talking, chewing, or even light touch to the face. While initial management includes anticonvulsant medications like carbamazepine, patients with refractory or intolerable symptoms require surgical intervention.
Microvascular decompression (MVD) is considered the gold standard micro-neurosurgical treatment for classic TN with around 95-97% success rate, especially when neurovascular compression is identified.
We operated and relieved the severe facial pain of a patient who underwent MVD surgery at another institution and severe facial pain came back 2 months after the surgery.
We identified the teflon granuloma (a very rare case) as the cause of recurrent pain and removed 1 piece of the teflon and re-positioned the other piece so that the trigeminal nerve got relieved.
We confirmed the relief of facial pain by neuro-monitoring and patient got absolutely pain free right after the surgery.
Patient is discharged to home pain-free without any medical pills.
