Meme Protezi Yerleşim Deformiteleri⁣
Mutlu günler,⁣
Meme düşüklüklerinde memeye implant yerleştirilmesi için çeşitli koşulların olduğunu söylemiştim. Memenin üst kısmında bir boşalma olduğu zaman dekolteyi doldurabilmek için bir implant yerleştirebiliyoruz. Ancak, bazı durumlarda hastalar ısrarcı olup eğer doktor da bu konuda çok tecrübeli değilse sadece implant koyulabiliyor. İmplant yukarıda kalıp snoopy deformitesi dediğimiz durum yani meme ucunun aşağıda olduğu sanki bir köpeğin burnu gibi bir görünüm ortaya çıkabiliyor. Burada da görmüş olduğunuz gibi implantın üzerinden aşağıya doğru inmiş memeler ortaya çıkıyor. Bu görüntü, başka türlü de görülebiliyor. Bu ne zaman ortaya çıkabiliyor? When there is a decollete emptiness in the upper part of the breast, we can place an implant to fill the decollete. However, in some cases, if the patients are insistent and the doctor is not very experienced in this field, only the implant can be placed without any lift. The implant can remain above and a condition we call as snoopy deformity, that is, an appearance like a dog's nose with the ni**le down, may occur. As you have seen here, breasts saggings downwards over the implant appear. This image can also be seen in other ways. When can this occur? It can also occur when high profile implants are placed. Meme sarkıklıklarının tedavisinde dekolte bölgesinin dolgunluğu bizler için önem arz ediyor. Özellikle, bazı hastalarımız dekolte bölgesindeki boşalmadan çok rahatsız oluyorlar. Burada evre 2 ve evre 3 meme sarkıklıklarında dekolte bölgesinde ileri derecede boşluk ortaya çıkabileceğini görüyorsunuz. Özellikle evre 3 sarkıklık varsa ve ileri derecedeyse, memenin tüm dokusu da aşağıya inmiş oluyor. Dikleştirme yapılsa bile dekolte bölgesinde bir dolgunluk elde edemiyoruz. Bu hastalarda implant yardımı çok önemli. Hastanın istemiş olduğu dekolte dolgunluğunun derecesine göre özel implantlar seçiyoruz. İmplantın projeksiyonunu ayarlayarak dekolte dolgunluğunun da miktarını artırabilmekteyiz.⁣

Decollete Fullness In Breast Lift Operations⁣

The fullness of the decollete area is important for us in the treatment of breast ptosis. In particular, some of our patients are very uncomfortable with the emptiness in the decollete area. Here, you can see that in stage 2 and 3 breast sagging, there may be a severe hollow in the decollete area. Especially, if there is stage 3 advanced sagging, all the tissue of the breast sags down. Even if a lifting is performed, we cannot achieve a fullness in the decollete area. Implant assistance is very important in these patients. Evre 3 - İleri Derece Meme Sarkıklığı⁣

Meme sarkıklıklarının derecelendirilmesinde evre 3 meme sarkıklığının en ileri derece meme sarkıklığı olduğundan bahsetmiştim. Burada da görebildiğiniz gibi memenin tüm dokuları ve yapıları meme altı oluğunun altına inmiş. Bu hastalara baktığımız zaman dekolte kısmında dışarıya doğru bir dolgunluk olmadığı gibi bazı hastalarda normalden daha da çukur duruyor. Dekoltedeki bu çukurluğun doldurulması için ek prosedürler uygulanması gerekebiliyor. Daha önceki günlerde sizlere bahsettiğim gibi meme ucu ileri derecede düşük hastalarda superior veya superomediyal pedikül dediğimiz meme ucuna kan getiren teknikleri kullanarak meme ucunu yukarıya taşıyabilmekteyiz. Bu şekilde, memeye yuvarlak ve daha doğal bir form verebilmekteyiz.⁣

Stage 3 - Advanced Breast Sagging⁣

I had mentioned that stage 3 breast sagging is the most advanced degree of breast sagging in the grading of breast ptosis. As you can see here, all tissues and structures of the breast have descended below the inframammary groove. When we look at these patients, there is no outward fullness in the decollete part, and in some patients it is even more hollow than normal. Additional procedures may be required to fill this hollow in the decollete. As I mentioned to you in the previous days, in patients with ptotic ni**les, we can move the ni**le upwards by using techniques that bring blood to the ni**le, which we call as superior or superomedial pedicle. By this way, we can give the breast a round and more natural form.⁣

Meme küçültme ve dikleştirme operasyonularında kullanmış olduğumuz önemli bir ameliyat tekniğinden sizlere bahsetmek istiyorum. Superomediyal pedikül dediğimiz bir teknik. Bu teknikte, at nalı şeklinde bir doku tabana kadar çıkarılıyor. Meme ucunu besleyen doku alanı meme ucunun iç ve üst kısmını kaplayacak şekilde oluyor ve kolaylıkla yukarı aşağıya dönebiliyor. Breast Reduction Technique With Superomedial Pedicle⁣

I would like to tell you about an important surgical technique that we use in breast reduction and breast lift operations. It is a technique we call as superomedial pedicle. In this technique, a horseshoe-shaped tissue is removed up to the base. The tissue area that nutrients the ni**le is such that it covers the inner and upper part of the ni**le and can easily turn up and down. After resecting tissue around the ni**le, we turn it upwards. We easily transfer the ni**le to where it should come. Especially as you can see here, we can have the chance to reduce the breast as much as we want by removing advanced tissue from the lower part of the breast.⁣

I have mentioned to you that the selection of the area that provides blood circulation to the ar**la complex in breast lift surgeries can be from different source with different techniques. In these surgeries, we can determine the location of the vessels that bring blood to the ni**le according to different anatomical conditions in different patients and apply the ni**le transport technique accordingly. Here, you can see that the blood vessels that bring blood from upper part, which we call as the superior pedicle, come from both right and left. If we perform a ni**le displacement based only on the blood vessels in the upper and inner part, we can perform ni**le displacement with techniques called as superomedial technique. If we perform it depending on the upper right part, we can perform ni**le displacement with techniques called as superolateral technique.⁣

İnferior Pedikül İle Meme Küçültme Tekniği

Meme küçültme tekniklerinden süperior pedikül yani meme ucunu besleyen kan damarlarının yukarıdan geldiği teknikten sizlere bahsettim. Bugün de sizlere aşağıdan kan damarlarının gelmesi halinde uyguladığımız inferior pedikül tekniğinden bahsetmek istiyorum. Burada görmüş olduğunuz inferior pedikül tekniğini farklı grup hastalarda kullanıyoruz. Hangi tekniği seçeceğimizi genellikle meme ucunun yukarıya doğru ilerleyeceği mesafeye bakarak karar veriyoruz. İnferior pedikül tekniğinde alt kısımdan meme ucuna gelen kan damarlarını koruyarak meme ucunu yukarıya doğru hareket ettiriyoruz. Yukarıda da ona özel bir yuva ve boşluk hazırlayarak oturacağı bölge belirleniyor. Memenin de yanlarından ve gerekli kısımlardan küçültme yapıldıktan sonra meme ucu yukarıya doğru yer değiştirilerek inferior pedikül tekniği tamamlanmış oluyor.

Breast Reduction Technique With Inferior Pedicle

I have talked about the superior pedicle technique, that is, the technique in which the blood vessels feeding the ni**le come from upper part. Today, I would like to tell you about the inferior pedicle technique that we apply if the blood vessels come from lower part. We use the inferior pedicle technique you see here in different groups of patients. We usually decide which technique to choose by looking at the distance the ni**le will move upwards. In the inferior pedicle technique, we move the ni**le upwards by protecting the blood vessels coming from the lower part to the ni**le. The area where it will place is determined by preparing a special area and space for it above. After the breast is reduced from the sides and necessary parts, the ni**le is displaced upwards and the inferior pedicle technique is completed.

Süperior dediğimiz, meme ucunu besleyen kan damarlarını memenin üst kısmından gelen dokuları koruyarak sağlamaktayız. Burada şema üzerinde size anlatmak istiyorum. Aşağıya doğru sarkmış memede yukarıya doğru meme ucunun ilerletilmesi için meme ucuna kan getiren damarlarını ya aşağıdan ya da yukarıdan korumak gerekiyor. Bunun için aşağıdan doku çıkartıp üstteki dokuyu koruyarak ve meme ucunu bu dokudan gelen kan damarları ile kan dolaşımını sağlayarak yukarıya doğru ilerletiyoruz. One of the techniques we apply for the correction of severely sagging breasts is the superior pedicle breast reduction technique. We provide the blood vessels that nourish the ni**le, which we call as superior, by protecting the tissues coming from the upper part of the breast. Here, I would like to explain it to you on the diagram. In order to transfer the ni**le upwards in the ptotic breast, it is necessary to protect the blood vessels that bring blood to the ni**le either from below or from above. For this purpose, we remove tissue from below, protect the upper tissue and transfer the ni**le upwards by providing blood circulation with the blood vessels coming from this tissue. Stage 3 Breast Sagging⁣

The procedures we apply in the correction of breast sagging are called breast ptosis correction or breast lift. We said that the ni**le must fall below the inframammary fold in order to be called as breast sagging. In stage 2 breast ptosis, we said that the ni**le goes below the inframammary groove. However, in some cases, the all tissue of the breast falls below the breast groove. This is Stage 3 which is the most advanced ptosis. These breasts can be considered as breasts looking at the ground. We apply different techniques in these patients. What we do simultaneously to lift the ni**le upwards. We determine the techniques we apply in the correction of breast sagging according to the needs of the person. Especially in breast reconstructions, breast sagging is of great importance. We can also perform sagging breast reconstructions in order to obtain a similar appearance to the opposite breast in procedures with severe sagging and where only one breast is completely resected. For example, the person has a stage 2 breast ptosis. One of the breasts is no need for intervention. One of them is resected and reconstructed again. If we perform a procedure to lift this breast, we can provide breast reconstruction in the position where the breast is down, as there will be a difference between the two breasts. Meme sarkıklıklarının evrelendirilmesine göre tedavi prensiplerinden bahsediyordum. Evre 1 hafif sarkıklıkta meme ucundan da bir toparlama yapılabileceğini söylemiştim. Bu toparlama yapılırken memenin doku miktarı da çok önemli. Eğer yoğun meme dokusu varsa ve bir implant yerleştirildiği zaman meme ucu pozisyonunun aşağıda kalacağı bir görünüm varsa o zaman T izli bir dikleştirme evre 1 hastalarda da gerekebilir. Evre 2 dediğimiz hastalarda meme ucu meme altı oluğunun altına düşmüş oluyor. Bu hastalarda üst pol dediğimiz dekolte kısmında ileri derecede bir boşalma oluyor. Dokuyu da yukarıya doğru çıkarmak için ters T veya lolipop şeklinde meme dikleştirme teknikleri kullanıyoruz. Meme ucunun yukarıya doğru kaldırılması ve implantın ortasına getirilmesi şeklinde bir ameliyat yapıyoruz.⁣

Stage 2 Breast Sagging⁣
