Meme Küçültmede Dekolte Dolgunluğunu Sağlama Teknikleri⁣
Merhaba,⁣
Meme küçültmelerde dekolte dolgunluğunu sağlamak için uyguladığımız tekniklerden bir tanesi, memenin kendi dokusundan bir dolgu materyali oluşturmak ve bu dolgu materyalini yukarıya doğru tespit etmek. Burada sarkık olan bir memenin alt kısmından bir doku planlanarak bu doku memenin altından yukarıya doğru yer değiştirilip üst kısımda dekolte dolgunluğunu oluşturacak şekilde tutturulabilmektedir. Bu tekniklerin uygulanması o kadar da kolay değil. Alt kısımdan oluşturulan dokunun kan dolaşımının bozulmadan yukarıya doğru taşınması gerekiyor. Çünkü memenin içinde ortaya çıkabilecek kan dolaşım bozuklukları daha sonra kalsifikasyon dediğimiz ileride tümöral kitlelerle karışabilecek radyolojik görüntüler oluşturabilmektedir. Bundan dolayı bu tekniklerin konunun uzmanı tarafından uygulanması çok önemli.⁣

Decollete Fullness Gaining Techniques in Breast Reduction⁣

One of the techniques we apply to provide decollete fullness in breast reductions is to create a filling material from the breast's own tissue and fix this filling material upwards. Here, a tissue can be planned from the lower part of a sagging breast and this tissue can be moved upwards from the bottom of the breast and attached to form the decollete fullness in the upper part. These techniques are not easy. The tissue formed from the bottom needs to be moved upwards without disturbing the blood circulation. Because blood circulation disorders that may occur in the breast in the future can be confused with tumoral masses which we call as calcification created by radiological images. Therefore, it is very important that these techniques are applied by an expert on the subject.⁣

Meme küçültme ve dikleştirme ameliyatlarında dekolte dolgunluğu kadınların önem verdiği bir konu. Özellikle doğal göğüsten dekolte dolgunluğu sağlamak çok zor. Burada öncelikli olarak meme dokusunun olup olmadığını değerlendiriyoruz. Dekoltesinde potansiyel bir meme dokusu olan bireylerde bu dokuyu koruyucu teknikler seçtiğimiz zaman, meme ucunu da yukarıya taşıyarak aynı anda dekolte dolgunluğunun öncesinden daha iyi bir hale geldiğini izlemliyoruz. Ancak bazı bireylerde üst kısımlar tamamen boş olabiliyor. Bu bireylerde daha farklı teknikler uygulamak gerekiyor. Decollete fullness in breast reduction and lifting surgeries is an issue that women care about. It is very difficult to achieve fullness in the decollete, especially from the natural breast. Here, we primarily evaluate whether there is enough breast tissue or not. When we choose techniques to protect this tissue in the decollete, we observe that the decollete fullness becomes better than before by moving the ni**le up as well. However, in some individuals, the upper parts may be completely flat. These individuals need to use different techniques. I can say that individuals with tissue in their decollete may have sufficient fullness from their own tissue without the need for an extra silicone etc.⁣

Merhaba, Meme ucu küçültme yöntemlerinden bahsediyordum. Bazı tekniklerde meme ucuna gelen süt kanallarının zedelenmesiyle bu süt kanallarının dışarıya direnajının engellendiğini ve buna bağlı olarak mastitlerin ortaya çıkabileceğini söylemiştim. İlla kişinin süt vermesi gerekmiyor. Kadınlardaki süt kanallarının içindeki sıvıların dışarıya atılması için bu kanalların ucunun açık kalması gerekiyor. Burada görmüş olduğunuz gibi uç kısmından tam süt kanallarının boşaldığı kısma çok dokunmadan olan tekniklerle küçültebileceğimiz kadar meme ucunu küçültme tekniklerini tercih ediyoruz. I have said that in some techniques, the drainage of these milk duct is prevented by the damage of the milk channels coming to the ni**le and mastitis may occur as a result. It is not a rule that every women necessarily have to give milk. In order for the functional fluids in the milk ducts in women to be drainaged, the tips of these milk ducts must remain open. As you can see here, we prefer the ni**le reduction techniques as we can reduce it with the techniques that do not touch the part where the milk ducts are drainaged from the tip. İyi günler, Memenin uç kısmının küçültülmesi işleminden bahsetmiştim sizlere. Bu işlemin tekniklerinden de bahsetmek istiyorum. Süt kanallarına zarar vermeden yapılan bazı teknikler var. Bu tekniklerden bir tanesi meme ucunun alt kısmından daire şeklinde bir derinin çıkartılması ve meme ucunun içeriye doğru katlanması. Buradan çıkartılan deriyi ve son halini görüyorsunuz. Burada meme ucunun taban çapı daralmamakla birlikte ileriye olan fırlaklığı azaltılabiliyor. Ancak bazı kişilerde bu meme ucunun taban çapı ve büyüklüğü de çok geniş oluyor. Bu geniş halkayı daraltmak için meme ucundan wedge dediğimiz üçgen şeklinde bölümler çıkarılabiliyor. Bu bölümlerin çıkarılmasıyla buradaki süt kanallarının zarar görmesi söz konusu olabiliyor. Süt kanallarının kapanmasıyla da ileride mastit ortaya çıkabilmesi mümkün olabilmektedir.⁣

Reduction Techniques of The Ni**le⁣

There are some techniques that are performed without damaging the milk ducts. One of these techniques is to remove a circle of skin from the lower part of the ni**le and fold the ni**le inward. Here, you may see the removed skin and its final form. Although the base diameter of the ni**le is not narrowed, its forward protrusion can be reduced. However, in some people, the base diameter and size of this ni**le is also very wide. In order to narrow this wide circle, triangular sections called as wedges can be removed from the ni**le. With the removal of these sections, the milk ducts here may be damaged. With the closure of the milk ducts, it may be possible to develop mastitis in the future.⁣

Meme ucundan yapılacak küçültmelerin zorlanmasının olumsuz sonuçlar doğurabileceğinden bahsetmiştim sizlere. Bazı hastalarımız özellikle büyük göğüslerin sadece meme ucundan bir izle küçültülüp küçültülmeyeceğini merak ediyorlar. Bu konu bize çok fazla soruluyor. Çünkü meme küçültme işleminde memenin iç dokusunu küçülttüğümüz zaman derisini de küçültmek zorunda kalıyoruz. Bu deri küçültmeyi sadece meme ucunun etrafından deri çıkartarak yapmaya çalıştığımız zaman burada görmüş olduğunuz gibi meme ucu etrafında çok ciddi bir kırışıklık ortaya çıkıyor. Yani büyük daire küçük daireye yedirilmeye çalışıldığı zaman pile pile bir görüntü ortaya çıkıyor. Bu gerçekten çok çirkin bir görünüme neden olacağı için bunu da bir sınırda yapabiliyoruz. Sadece her hasta için meme ucundan küçültme yapmak mümkün olmamaktadır.⁣

I had told you that reductions only to be performed from the ar**la with traction can have negative consequences. Some of our patients are wondering whether especially big breasts may be reduced with just a scar from the ni**le ar**la. This subject is asked a lot to us. Meme ucunda küçültme ameliyatları yaptığımız zaman meme ucunun yeniden genişlemesinin önüne geçmek için tanımlanmış en iyi tekniklerden biri “Round Block” tekniğidir. Yani yuvarlak bir şekilde meme ucunun etraf dokuya adapte edilmesi tekniği. Bu ne şekilde yapılıyor? Erimeyen özel Gore-tex dikişlerle deri altından devamlı bir şekile, ince aralıklarla tek dikişle iki yandan ilerleyip yıkarıdan bağlıyoruz. Bir bohçayı sıkar gibi iki yandan deri sıkışıyor. Yukarıdan da düğüm yapıp bu derinin esnemesinin ve açılmasının önüne geçiyoruz. “Round Block” tekniği özellikle meme ucunun çok geniş olduğu ve ileri daraltma gereken vakalarda ciddi derecede iş gören ve başarılı sonuçlar veren bir teknik olarak karşımıza çıkıyor.⁣

“Round Block” Technique in Ni**le Ar**la Reduction⁣

When we perform ni**le ar**la reduction surgeries, one of the best defined techniques to prevent the re-expansion of the ni**le is the "Round Block" technique. In other words, it is the technique of adapting the ni**le to the surrounding tissue in a rounded way. How is this perfomed? With special non-resorbable Gore-tex sutures, we proceed from both sides with a single stitch at thin intervals under the skin continuously and tie it at the top. The skin tightens on both sides, as if squeezing a bundle. We make knots at the top and prevent this skin from stretching and opening. The “Round Block” technique is a technique that works seriously and gives successful results, especially in cases where the ni**le ar**la is very wide and advanced narrowing is required.⁣

Meme ucunun küçültülmesi ameliyatlarında meme ucunun etrafından bu şekilde bir doku çıkarıldığından sizlere bahsetmiştim. Bu dokuyu çıkardıktan sonra etraftaki derinin gerginliğine göre farklı tekniklerle bu aradaki dokunun birbirine yaklaştırılmasını sağlayabilmekteyiz. Özellikle bu konuyla ilgili literatürde tanımlanmış birçok teknik bulunuyor. Aslında buradaki boşluğu ne kadar büyütürsek meme ucunun etrafına gelecek olan çekme kuvveti de o kadar artacaktır. Bu çekme kuvvetinden dolayı meme ucunun yeniden genişleyebilme ihtimalinin olduğundan bahsetmek istiyorum. Eğer bu bölgeden çıkarttığımız dokudan çok fazla miktarda bir gerginlik hissetmiyorsak o zaman normal deri altı dikişleriyle meme ucunu fazla genişletmeden meme ucunu küçültme imkanımız olmaktadır.⁣

Principles of Ni**le Ar**la Reduction⁣

I had told you that as you see here, a tissue is removed from around the ni**le by this way in ni**le reduction surgeries. After removing this tissue, we can bring the skin edges together with different techniques depending on the tightness of the surrounding skin. In particular, there are many techniques described in the literature on this subject. In fact, the more we enlarge the space here, the more the tension force around the ni**le will increase. I would like to mention that there is a possibility of re-expansion of the ni**le due to this pulling force. If we do not feel a large amount of tightness from the tissue we removed from this area, then we have the opportunity to reduce the ni**le without too much expanding the ni**le with normal subcutaneous sutures.⁣
