🩺 Septal Perforation: When the Cause Is Not Obvious
Not all nasal septal perforations have a clear or single cause.
While co***ne use and previous nasal surgery are well-known risk factors, many patients do not have these histories.
In daily practice, common non-rheumatologic contributors include:
• Childhood nasal trauma
• Chronic nose picking
• Long-term use of topical nasal sprays
• Recurrent infections
• Subtle mucosal ischemia over time
In some patients, these factors may act silently and cumulatively, leading to perforation years later.
📌 Even when major causes are excluded,
“idiopathic” perforations may still have an underlying trigger — just not an obvious one.
Understanding this helps avoid unnecessary blame, delays in diagnosis, and missed treatment opportunities. ***nenose
