All improvements were achieved through precise anatomical repositioning and structural restoration.

At 2 months, postoperative edema has significantly decreased, allowing the natural contour of the midface–periorbital unit to emerge.

The goal was not augmentation, but reconstruction of facial anatomy through an endoscopic approach — restoring:

• midface support
• tear trough continuity
• lateral canthal stability

while preserving a natural, non-overfilled appearance.

Sunken eye deformity is often misunderstood as a simple volume deficiency.

In reality, it is most commonly the result of orbital fat displacement and structural imbalance.

In this patient we performed:

✨ Endoscopic Retro-Orbicularis Oculi Fat (ROOF) Transposition
✨ Ptosis Repair
✨ Bella Eyes®️ – Endoscopic Dynamic Canthopexy

⚠️ No fat grafting
⚠️ No fillers
⚠️ No artificial volume augmentation

Instead of adding volume, the patient’s own orbital fat was anatomically repositioned to restore the natural contour of the upper eyelid and periorbital unit.

At 2 months postoperative, swelling has significantly subsided and the structural correction of the hollow upper eyelid is clearly visible.

The goal was not to enlarge the eyelid —
but to restore its native anatomy and youthful convexity.

Results will continue to refine over the coming months.

Postoperative 1-Month Update.

Our patient requested a more lateral almond-shaped eye appearance rather than a vertically lifted look. According to her aesthetic preference, we performed Bella Eyes, achieving a natural lateral elongation of the eye contour.

Our latest publication:
“Endoscopic Retro-Orbicularis Oculi Fat Transposition for Correction of Sunken Eyelid Deformity”
is now published in the Plastic and Reconstructive Surgery (PRS) Journal — one of the most prestigious journals in the world of plastic surgery.

This marks our 5th technique and 5th publication in the field of endoscopic facial surgery.

In this study, we present a refined, anatomically respectful approach to correcting sunken eyelid deformity using retro-orbicularis oculi fat (ROOF) transposition — restoring volume without artificial fillers, and achieving natural, long-lasting results.

Innovation is not just about creating new techniques.
It’s about redefining what is possible — safely, precisely, and consistently.
