The most challenging aspect of this case was determining the placement of the ridge located between the distobuccal and distolingual cusps.
Based on the remaining tooth structure after cavity preparation, the presence and general course of this ridge could be identified.

The distolingual cusp corresponds to the hypocone, whose degree of development is known to vary widely in natural dentition.
Anthropological classifications describe the hypocone along a continuum from well-developed to reduced or entirely absent, and this framework guided the interpretation of the remaining structure.
In this case, the morphology suggested a moderately developed hypocone, which informed the positioning of the cusp.

In contrast, the position of the distobuccal cusp was largely dictated by the orientation of the buccal groove and the central fossa.

The ridge was ultimately positioned in a manner that separates the distobuccal and distolingual cusps.
However, a morphology in which the ridge does not completely divide these cusps may also occur in natural teeth.
The prominence and continuity of this ridge likely exhibit considerable individual variation, and further observation of natural dentition will be essential for better understanding the full range of this morphological feature.
