🙌Agenda: Best Atrial Fibrillation Management: Concomitant Atrial Fibrillation Ablation and Left Atrial Appendage Closure-The CMUH Experience
🙌Abstract:
Concomitant atrial fibrillation (AF) ablation and left atrial appendage closure (LAAC) represent a promising integrated approach for patients with symptomatic AF and elevated stroke risk who are either unsuitable for long-term anticoagulation or seeking comprehensive rhythm and thromboembolic protection. This presentation will review the rationale, clinical evidence, and procedural strategies supporting the combined use of catheter ablation and LAAC.
The left atrial appendage is both a common trigger site for atrial arrhythmias and the primary source of thrombi in non-valvular AF. Addressing both the electrical substrate and the anatomical source of embolic risk in a single procedure offers the potential to improve long-term rhythm control while minimizing stroke risk without lifelong anticoagulation.
Recent trials and registries, including the aMAZE, WATCH-AF, and AMULET IDE studies, have demonstrated the feasibility and safety of performing AF ablation and LAAC either simultaneously or in a staged approach. Outcomes from these studies suggest comparable efficacy in arrhythmia control, with favorable safety profiles and acceptable device success rates. However, patient selection, procedural timing, and device choice remain critical considerations.
This talk will outline the procedural workflow, including mapping and ablation strategies, transseptal puncture techniques, and LAA closure device deployment. Special attention will be given to anticoagulation management pre- and post-procedure, as well as imaging protocols for ensuring optimal device placement and long-term closure success.
Attendees will gain insight into current guidelines, ongoing clinical trials, and evolving best practices. The goal is to provide electrophysiologists with practical, evidence-based strategies for implementing concomitant AF ablation and LAAC in appropriate patients, ultimately aiming to improve clinical outcomes through a tailored, multidisciplinary approach.
