3.出院後仍需護理扶持照護者。
4.生活自理需人陪伴之健康老人。
服務項目：
1.護理照護：專業護理人員提供全方位護理照顧。
2.生活照顧：照顧服務員全天候24小時生活照顧。
3.生活娛樂：規劃生動有趣的休閒活動，紓解身心。
4.醫療服務：醫師定期巡診，健康檢查，疫苗注射。
5.飲食營養：專業營養師評估營養狀況，提供衛教。
6.復健治療：依長者個別需要，安排復健職能治療。
※交通指引：
緊鄰一高、二高及中彰快速道路，距高鐵烏日站約五分鐘車程，交通便利。
