*** NOTICE*****
Proposed treatment program for acute renal failure (ARF) in the United Republic of Tanzania as a model for sub-Saharan Africa.

Abstract

To date, no sustainable treatment program exists for kidney failure in countries in sub-Saharan Africa. No data are available on the incidence, prevalence or cause of chronic kidney disease (CKD). The world's focus has been on treating 'priority diseases' as defined by the United Nations Millennium Project and the World Health Organization because of their high mortality rates. This is understandable because HIV/AIDS, tuberculosis, diarrheal diseases and malaria claim majority of the lives; however, in many cases, the mortality is due to acute renal failure (ARF) as a consequence of the progression of these diseases. Moreover, to date, no data are available on mortality rates due to ARF as a consequence of these 'priority diseases.' Sustainable Kidney Care Foundation (SKCF), working with industry, institutions, universities and funding organizations, is in the process of establishing a pilot program for the treatment of acute renal failure (ARF) using peritoneal dialysis (PD) as the modality of choice with a special focus on treating children and women of childbearing age in the United Republic of Tanzania.

3 –day Kampala Renal Week Conference has opened in Kampala with a call for adoption and application of basic preventive measures at community level to reduce the kidney disease burden in Uganda.

The call was made by Ms. Jennifer Musisi Semakula, the Executive Director Kampala Capital City Authority (KCCA) while opening the conference at Grand Imperial Hotel.

She said that with the involvement of the political leadership the City Authority is embarking on a massive campaign to reach the communities to ensure that health aspects of city management are adequately addressed before they become diseases. She said that communities need to be engaged on issues of extent of damage of kidney disease, level of treatment required and the costs involved.

Ms. Musisi said that the city authority is expanding health facilities and note that this is an opportunity to roll out kidney disease management.
The Minister of Health Dr. Christine Ondoa said that kidney disease is one of the Non Communicable Diseases associated with high morbidity and mortality. She said management of diet, to***co and alcohol use will help reduce kidney disease burden.

Dr. Ondoa’s speech was read for her by Dr. James Sekajugo, who is the in-charge Non Communicable Diseases in the Ministry.
In her speech the Minister pointed out the need for equipment in health facilities to enable health workers screen for kidney disease.
Professor Nathan Levine of the International Society of Nephrology noted that the inadequate number of specialists in kidney disease should be handled through shifting responsibilities to nurses and clinicians with supervision by the few nephrologists available.

The conference was the first of its kind in Uganda and it is expected to provide impetus for scaling up the fight against kidney diseases.
Makerere University College of Health Sciences was one of the organisers of the conference that attracted participants from different parts of Africa, Europe, USA and other parts of the world.

Mulago hospital has acquired 10 more dialysis equipments at a cost of shs 150 million each with the help of the Japanese government.

Bateerana Byarugaba the Executive Director says the increment will enable more patients access treatment. Thomson Maseruka a doctor at Mulago says with the increment, the hospital will be able to handle about 14 patients on a daily basis up from the three patients the hospital has been receiving per week.

Bateerana however says treatment is still expensive because the hospital lacks consumables. He says kidney patients in need of dialysis part with shs 250000 every time they go for a session and in the week they would spend about shs 750000 but Bateraana says this is bound to change starting next financial year because the hospital is in the process of acquiring consumables that will be used.

Bateerana says if the hospital gets consumables, patients will pay between shs 25000-30000 per session.

Don't know your blood type?
Before you can test for anyone who needs a kidney, you would need to find out what your blood type
is. One of the things you can do to find out what your blood type is, is go to donate blood and tell
them to let you know what your blood type is - or, you can go to your doctor to have a special
blood test taken to find out what your blood type is. If you are going to do this at a doctor's office,
ask your doctor to put on the request form for the lab, "If A blood type, please subtype", because in
most cases people who are A blood type can only donate to an A or an AB blood type person in
need of a kidney. But if your subtype is A2 you might be able to donate a kidney to someone who is
B or O as well. If you are an A2B you may be able to donate to a B as well as an AB blood type. This
A2 subtype is rare but it can happen. If you are subtype A1, you can only donate a kidney to
someone who is blood type A. There is an alternative treatment to make blood-types that are
different, compatible, such as plasmapherisis, a treatment that would be done on the person in need
of a kidney. But this isn't always the best alternative for the person in need of a kidney.
