We're celebrating 50 years!! CHP was founded on Dec. 19, 1974 and the first in-home nursing visit was made in March 1975. In 1984, we were the first hospice established in this area and only the seventh in the state at that time. We'll be sharing more of our history as we celebrate this milestone throughout 2025! ❤🥇

CHP Home Care & Hospice is honored to deliver compassionate, quality care to veterans and their families. Hospice is a VA benefit that's delivered through local hospice agencies, like CHP. Home health services may be covered through the VA or through Medicare, or private insurance. Contact us to learn more.

This November, CHP Home Care & Hospice recognizes National Care at Home Month. Home health, private duty home care, palliative care, and hospice care – provide interdisciplinary, holistic care to millions of people, allowing them to recover from illness or injury, receive daily pain relief and support, or spend their final months wherever they call home surrounded by their loved ones.

National Care at Home Month also recognizes the important work of unpaid, family caregivers, who often labor without notice or thanks. While the support of home care provides immense relief for patients’ unpaid caregivers, they continue to engage in acts of love, commitment, and compassion while caring for their loved one, often at great personal sacrifice during an already emotionally and financially challenging time.
