San Luis Care Center San Luis Care Center is a skilled nursing and rehabilitation center located in Alamosa, Colorado.

Surrounded by the majestic Colorado Mountains, San Luis Care Center, a Life Care Centers of America facility, is situated in the world's largest alpine valley, the San Luis Valley in Alamosa, Colorado. Just southwest of The Great Sand Dunes National Park and surrounded by shopping and restaurants, our nursing home is a perfect choice for individuals in need of short-term rehabilitation, long-term care or post-operative recovery. Focusing on inpatient and outpatient rehabilitation with 24-hour skilled nursing care, our in-house rehabilitation team uses state-of-the-art rehab equipment, such as the AlterG® Anti-Gravity Treadmill®. Our caregivers and therapists work together to create individualized care plans for each resident and patient, combining passion and care to provide excellent, individualized care.

👩‍⚕️ Barbara Montoya, LPN, is our Whatever it Takes Champion for September. Barbara was nominated by a resident who shared that Barbara is a dedicated nurse that has been with the San Luis Care Center for 28 years. 💙

Barbara takes the time to visit with each resident. She is a caring, friendly and compassionate. She reaches out to meet each person's care, reaches out to the doctors and families. She does her best to make the San Luis Care Center their home. 🏡
