We are writing to inform you that, effective July 31, 2025, Dr. Talley and Dr. Smurda will no longer be practicing together as Women’s Health Professionals, LLC.
Beginning August 1, 2025:
• Dr. Talley will remain at the current office at 414 5th Avenue in Albany and will continue caring for his patients.
• Dr. Smurda will continue his practice with Southwell Medical at 1010 W. Franklin St. in Sylvester, where he will be happy to care for his patients.
Your medical records will remain available to you. At any time, we are happy to provide a copy of your records—either to you directly or to another healthcare provider of your choice—at no cost, upon your request. You may request your records by phone, in person, or in writing until July 31, 2025, through our current office.
After that date, your original records will be archived by Phoebe Health System. To request records after July 31, please contact:

Please be assured that all record transfer requests will be processed promptly and will not affect your ongoing care or future appointments.
