Are you an LPN looking to make a meaningful impact in the community? Phoebe Home Health is hiring, and this could be the perfect place for you. We are seeking a Licensed Practical Nurse who is passionate about delivering high‑quality care and making a difference in the lives of every patient we serve.In this role, LPNs provide one‑on‑one skilled nursing care in the patient’s home. Responsibilities include assessing patient needs, developing care plans in collaboration with physicians, and carrying out those plans alongside the patient, their family, and support staff. This position focuses on monitoring treatment responses, medication management, healing progress, and mobility, while overseeing case management. LPNs in home health enjoy the opportunity to spend meaningful time with patients, promote independence, and deliver innovative, patient‑specific care.

Qualifications:
- Vocational/Technical Degree from an accredited Practical Nursing program (required)

Work Experience:
- 1–2 years of practical nursing experience (preferred)

Licenses and Certifications:
- Current Georgia LPN license
- Basic Life Support (BLS)
- Advanced Cardiac Life Support (ACLS)
- Valid Georgia Driver’s License
