Through this partnership, eligible patients receive their own dedicated care coordinator who works closely with their Oaklawn primary care provider to:

✔️ Coordinate specialist appointments
✔️ Assist with medication refills
✔️ Help with transportation and community programs
✔️ And more—all available 24/7

📌 Cost & Coverage:
This service is covered by Medicare and may be subject to your deductible and coinsurance (usually 20%). Many patients with secondary insurance plans will have little or no out-of-pocket cost. 🗓 New hours starting July 28:
➡️ Monday–Friday: 8:30 a.m. – 7 p.m.
➡️ Saturday: 9 a.m. – 3 p.m.
➡️ Sunday: 11 a.m. – 3 p.m.

📢 Important Update for July 5
Oaklawn Express Care – Albion will be closing early at 2 PM on Saturday, July 5, instead of the usual 8 PM.

If you need urgent care after that time, our Express Care locations in Marshall and Battle Creek will remain open during normal hours, 8 AM – 8 PM.

Albion will resume normal hours on Sunday, July 6.
